(The location on the sign can just read Carmel City Hall.) 3. The Filing Fee and 1 digital copy of the BZA Info Packet must be delivered to BZA administrative assistant Joe Shestak no later than Noon, Friday, July 17. (Please provide us with the certificate of mailings for the file on or before Wednesday, July 22.) e) Please provide the filled out & notarized Placement of Public Notice Sign Placement Affidavit form (see application) once the sign is placed. f) Please provide the filled out & notarized Petitioner’s Affidavit of Notice of Public Hearing form (see application). g) Please provide the filled out Notice of the Public Hearing form/template (see application). h) Please provide proof of notice, such as Publisher’s Affidavit from the newspaper, once you receive them. i) Please make sure to list out the true practical difficulties and hardships associated with having to comply with the ordinance. j) Is the Village of WestClay Owners Association & Architectural Review Board supportive of the proposed site plan layout?
