Re: Health and Wellness Suites of Carmel – TAC Review

Dear Mr. Hartman:
American Structurepoint, Inc., has revised the design, construction plans and drainage report for the
Health and Wellness Suites of Carmel project per the review comments listed in the letter dated June
10, 2013. Each comment listed from the review letter contains a response addressing each issue.
Below are the original comments with the response in italics.
1. We ask that the prefix for the manhole be labeled as MH#MWS1.
Manhole labels reflect the naming example and can be found on the Sanitary Sewer
Plan & Profiles on sheets C.303 – C.304.
2. Existing MH#S2 should be labeled MH#MC-25 on Sheet C.303.
Existing manholes have been renamed. See revised Utility Plan on sheet C.300 and
Sanitary Sewer Plan and Profile on sheet C.303.
3. Please show profile view of 8” lateral with all utility crossings.
Sanitary laterals have been revised and lateral plan and profiles can be found on the
Sanitary Sewer Plan and Profile on sheet C.304.
4. Sheet C.303 plan and profile view please specify length of Jack and Bore and include casing
material, thickness and diameter.
Typical Boring Detail provided by Clay Township Regional Waste District has been added to
sheet C.606. The appropriate notes regarding the jack and bore method, casing mater,
thickness and diameter have been added to sheet C.303.

5. If Jack and Bore is utilized, granular backfill callout on profile view is not necessary.
A note stating granular backfill is not required if jack and bore method is utilized has
been added to Sheet C.303.
6. Min. FFE of the structure must be set 1’ above TC of up or downstream manhole
(Min: 856.61)
According to agreement made, a manhole structure will be added for each sanitary
lateral with a casting grade no higher than 853.50 (one foot lower than the finished
floor elevation)
7. Please add a note to grease interceptor to refer to detail drawing sheet C.601
Note added to the grease interceptor keynote #5 on revised Utility Plan, sheet C.300.
8. Please add hardscape/pavement bump out at interceptor location for serviceability.
The grease interceptor is now located within the paved drive.
9. Please send to CTRWD a full copy of the IDEM Application and Allocation packet.
IDEM Application and Allocation packet will be sent to the Clay Township Regional
Waste District next week.
