Dear Mr. Thomas:
American Structurepoint, Inc., has revised the design, construction plans and drainage report for the
Health and Wellness Suites of Carmel project per the review comments listed in the August 1, 2013
letter/e-mail. Each comment listed from the review letter contains a response addressing each issue.
Below are the original comments with the response in italics.
SWPPP
1. Please place the SWPPP narrative in the City of Carmel format (attached). Item #5 of the
post-construction SWPPP is the requirements of the O&M manual.
All available information provided and is in the City of Carmel format. See sheets
C.500-C.507 (Erosion Control Plan and Stormwater Pollution Prevention Plan). See
revised O&M manual.
2. Construction entrances for sites greater than 2 acres must be a minimum of 150’L x 20’W.
Please revise proposed entrance.
Revised construction entrance is approximately 165’L x 30’W. See Erosion Control
Plan, sheet C.500 and detail found on Sheet C.503.
3. Concrete washout area, trash containment, and port-o-let should be located within the site
construction limits.
See revised locations on the Erosion Control Plan, sheet, C.500.

4. Label/Hatch the method of stabilization of all disturbed areas that will not be impervious
surfaces. Seed and Straw is required at a minimum.
See labeled and hatched methods of stabilization on the Erosion Control Plan, sheet
C.500. See and mulching requirements are outlined on Sheet C.504.
5. Unless pond will be filled by on-site well, scour protection at outlets into pond should extend to
the bottom of the pond. Rip rap over geotextile. This is to prevent scour of the bank and
undermining of the end sections during pond re-charge.
Rip rap outlet protection has been extended to the pond bottom as shown on sheet
C.500.
6. A protection is needed along the top of bank of the proposed pond where it is adjacent to
disturbed site areas (south and west perimeters) to prevent bank erosion.
Erosions control blankets to be used on slopes of pond. See revised Erosion Control
Plan, sheet C.500.
7. Is there a reason that two storm lines drainage the site don’t meet before the pond and then get
treated by one larger water quality unit instead of two?
Two lines are needed because joining the lines would cause capacity issues. Upsizing
the pipes caused coverage issues, thus two lines and two aqua-swirls are needed.
8. Show limits of native plantings in the pond and use different hatching to show limits of wetland
shelf and native banks on SWPPP. Native planting surface area must equal to or larger than 1/3
the surface area of the pond’s normal pool.
Plantings in the pond will be located on the 17 feet ledge and are identified with a
hatch on sheet C.500. The Pond Cross Section can be found on the Grading Plan,
sheet C.401. The native planting surface area is larger than 1/3 the surface area of the
pond’s normal pool (normal pool area of 0.52 acres, planting area of 0.22 acres). The
planting details will be located on the Landscape Plans.
9. Provide species list for the native plantings.
The planting details are located on the Landscape Plans (included in this submittal).

10. Include a description of the Naturalized wet pond with Native Banks and Wetland Shelf in the
post-construction SWPPP narrative.
See revised SWPPP on sheet C.505.
11. Label all BMPs on the SWPPP. The wet pond label should be modified to indicate that there
are native banks and a wetland shelf.
See revised O&M manual and revised SWPPP sheet C.507.
12. Protection signage is required around native planting areas. Please show the location of the
signage on the SWPPP and provide a detail for the sign on the plans.
Location of the Do Not Mow signs are shown on the landscape plans.
13. Storm inlets castings must be stamped with a “No Dumping, Drains to Waterway” message.
Please provide note on plans.
Note is provided on the revised Storm Sewer Structure Data Table, sheet C.411.
14. Provide pond cross-section showing the limits of the native planting and wetland shelf. Show
location of protection signage on the banks and reference the species lists.
Pond Cross Section is provided on the revised Grading Plan, sheet C.401 as well as
the revised Drainage Report. The protection signage is shown on the Landscape
Plans.. Signage and planting details will be provided on the Landscape Plans.
15. Storm inlets on existing road leading to the construction entrance should have inlet protection.
Please show.

16. Consider moving construction staging area closer to the construction entrance to prevent
contractors from driving through disturbed ground to park.
Revised construction staging area is shown on sheet C.500 to be closer to the entrance.

17. Temporary soil stockpile should be surrounded with a perimeter control.
Silt fence added to revised Erosion Control Plan, sheet C.500.
18. Access and BMP/Drainage easements are required for all BMPs per the specification in
Chapter 700 of the Storm Water Technical Standards manual. Please show on SWPPP.
BMP/Drainage easements are shown on the revised Site Plan, sheet C.200 and
Erosion Control Sheet C.500. The BMP Location Map provided in the Drainage
Report and O&M Manual also shows the easement.
O&M Manual
19. Include cross-section of pond showing the limits of the native plantings.
Pond Cross Section is provided on the BMP4 sheet and shows the planting shelf.
20. Update site map with modified pond name indicating the native banks and wetland shelf.
See revised pond label (with native banks and wetland shelf) on the BMP Location
Map on sheet BMP1.
21. Update site map with “Do Not Mow or Spray” sign lcoations.
See revised BMP Location Map for sign locations.
22. Include native bank and wetland shelf planting species lists.
Native planting species lists will be provided by the Landscape Plans and have also
been included in the BMP Manual.
23. Provide sample inspection for the naturalized pond.
Sample Tabular Maintenance Schedule (Wet Detention Pond) has been provided with
the revised BMP Manual.

24. Annual reports can be submitted to: City of Carmel Engineering Department, Attn: Storm
Water Administrator, One Civic Square, Carmel, IN 46032
Note added to revised O&M Manual.
25. Record O&M as an instrument with the final plat.
After discussing with planning, a final plat will not be prepared for this project. If
required, the O&M Manual can be recorded separately.
26. Label existing wetlands on the site map.
Labels for existing wetlands have been added, see revised O&M Manual.
27. Update Site Map with the limits of the native banks and wetland shelf.
See Pond Cross Section on the BMP Details sheet for bank and wetland shelf limits.
28. Indicate in the vegetation management section of the naturalized pond that the pond may be
mowed once per year in early spring or late fall. All cuttings must be removed from the pond
area and disposed of properly.

29. Update site map with BMP/Drainage Easement limits.
