Description of Change:
1) The Installation of(12) 12" and (2) 18" PEF style plug valves in lieu of PEC style valves. Total cost for
this item $1,500.00. FD#1
2) Contractor to change the grade on the vactor dumping station slab to allow the placement of the
dumpster on a flat surface. Total cost for this Item $1,475.00 FD#2
3) The removal of buried concrete, from the site, impeding the installation of the 6" watennain. Total cost
for this Item $3,598.50 FD#3
4) Due to poor soils, over excavation at the footer for the vactor receiving station. Total cost for this Item
$3,375.00 FD#6
5) The installation of control logics plc in lieu of the slc5/05. Total cost for this Item $12,287.00 FD#7
6) The installation of asphalt wedge in the NE parking lot. Total cost for this Item $2,156.00 FD #8
7) The deletion of reinstalling heater AHU-I in the GBT building. Total deduct for this Item ($2,150.00)
8) The use of Zinc Bolts in place of Stainless Steel as embeds on the center column. Total deduct for this
Item ($1,223.00)
9) The deletion of the Aluminum cover for the supernate tank. Total deduct for this Item ($98,860.00)

Dear Mr Grimes—
Alfa Laval Ashbrook Simon Hartley, Inc. ("Alfa Laval") has reviewed the GBT control panel review
comments dated 5/29/13 for the above mentioned project and would like to bring.to your attention
two comments from the review in regard to products being supplied:
• Alfa Laval will provide the PanelView Plus 6 OIT screen as requested in lieu of the PanelView
+ 1000 as shown.in the'submittal as we have confirmed the +1000 is in fact discontinued
• Alfa Laval included the Allen Bradley SLC 5/05 PLC processor per our proposal, for use in
the GBT control panelras iris the same model currently being used with the GBT equipment
• on-site It has been brought to our attention that the ControlLogix PLC is<being requested
due to its listing in specification 16905, however, it was our understanding that this section
described the main control panel in •the plant, and not our GBT control panel. The GBT
specification section 11850 does not reference any other sections and also does not specify:a
particular model of PLC in our specific control panel section, therefore we priced a control
panel equivalent to what is currently in use
Alfa Laval kindly requests a change order in the amount of$11,170.00..in order to upgrade to the••ControlLogix PLC model •
1756-L71 •(this •model is proposed in lieu of the model in 16905, 1756 L63,••
as it is equivalent and newer, therefore will be supported longer). This change request is at cost, but •
we would like to point out that it does include additional engineering and programming costs from the
panel supplier to accommodate the change in processor.

Please advise as to how we should proceed. Alfa Laval will need to receive a change order prior to'
revising the submittal.documents and proceeding with fabrication of the GBT panels if we are to go.;

Installation labor is not included in the scope of our base proposal
• If supervision is required during installation, Ultraflote can provide an experienced supervisor at the rate of
$1,000.00 per day, from portal-to-portal. This rate is based on a five- (5) day at (8) hours per day normal
workweek, for a total of forty (40) hours. Should more hours a day or for the week be required,they will be
charged at the rate of$ 187.50 per hour. However, it is not recommended to exceed twelve(12) hours per day for
efficiency and safe working conditions for a supervisor. In addition, all travel and living expenses will be for the
customer's account.
• The estimated Man-hours are based on the productivity of an experienced Ultraflote crew. The installation contractor
should consider carefully the productivity of his work force, the site conditions and the construction strategy to be
employed when evaluating these estimates.

INSTALLED OPTION MATERIALS & LABOR
Total Installed Price $ 113, 876.00
Our Installed Option Price is based upon delivery all materials at one time with installation being done in a continuous
construction program with one UNREPRESENTED field crew at prevailing wage rate.
Ultraflote Corporation is an unrepresented company offering installation of these aluminum flat covers on a non union
basis. In the event there is a claim for the work by others Ultrafiote will exit the site and invoice customer for our costs,plus
overheads and profit to date.
If customer elects to award the order for this project to Ultraflote Corporation, such award will be made according to these
conditions. Accordingly,if any labor difficulties result from the performance of this work,Ultraflote Corporation will exert
its best efforts to assist the customer in resolving any such dispute. Any expenses incurred by Ultrafiote Corporation as a
result of such labor difficulties shall be for the account of the customer.
IF INSTALLATION LABOR IS PROVIDED BY ULTRAFLOTE CORPORATION THEN
THE FOLLOWING ARE TO BE PROVIDED BY THE PURCHASER OF THE FLAT PANEL COVER:
• Crane and required rigging for off loading materials.
• Crane for setting of flat panels and trusses (if required)
• Electric service (115 volt/30 amp) for small tool operation.
• Adequate, level dry working area as close as possible to the tank.
• Structure or site modifications other than construction of the Quad-Seal Welded Aluminum Flat Panel Cover are not
included in our scope of work.
• Any work of other trades, such as electrical, pipe fitting, painting, ductwork, or miscellaneous metal work (e.g.,
handrails, platforms, etc.).
PRICING IS BASED AND CONTINGENT UPON:

♦ The tank/basin coating system damage, or repair thereof, resulting from the installation of the cover, or use of
erection equipment, that must be bolted to the wind girder and tank shell.
• Published prevailing wage rates (if applicable) for estimated time of erection. Erection at later dates (i.e. higher
prevailing wage rates)will be charged to the purchaser of the cover.Notice of correct prevailing wage rate is required.
• The pipe work that penetrates the Flat Panel Cover being routed to avoid interference with its structural members.
Ultraflote will provide information to the pipe supplier at time of submittal so he can coordinate his pipe work. Pipe
connection/penetrations at the Flat Panel Cover will be made(by others)such that the pipes penetrate the cover surface
perpendicularly.

other metal components in our products are increasing dramatically.These increases could force price adjustments for

• Since Ultraflote Corporation is not in charge of the project's schedule Ultraflote is not in a position to accept any type of
liquidated damages.
• Unless otherwise stated all trusses will be above panel surfaces.
• Cover to be mounted on angle at center bridge and two piece adjustable z-bend at stem wall.
TERMS: Ultraflote's Standard Terms and Conditions for the domestic purchase of the Ultradome roof are made a part of
this proposal.
DELIVERY:
Based upon current material inventory and space presently available in our schedule, it is estimated that the proposed
Flat Cover Roof could be in manufacturing and shipped within 10 to 12 weeks after receipt of your order and approval.
Assume 6 to 8 weeks after receipt of complete tank information after receipt of complete tank information for approval
drawing preparation and submittal. Please contact us if our proposed delivery schedule does not meet your operational
requirements.

STANDARD TERMS AND CONDITIONS FOR DOMESTIC PURCHASE OF THE,ULTRA DOMED ROOF
I. ACCEPTANCE: The Standard Terms and Conditions set forth herein for purchase of the ULTRADOME from ULTRAFLOTE CORPORATION,(hereinafter call UC)shall be
applicable to all orders accepted by UC and shall cancel and supercede any terms and conditions on purchase order forms or contract fonns submined by Buyer,whether or not
acknowledged or accepted by UC. No modification of the Standard Terms and Conditions shall be effective unless agreed to in writing by an Officer of UC_ These terms and
conditions shall become a binding contract between Buyer and UC upon acknowledgment of Buyer's order by UC.
2. BUYER OBLIGATIONS:To insure optimum performance and proper installation of the ULTRADOME.Buyer shall at its own expense perform the following tank preparation
work and supply all necessary materials therefor,prior to the scheduled arrival of UC and/or subcontractor installation crews. Buyer shall provide to UC and/or subcontractor
satisfaction,suitable and safe 115 volt 50 amp 60 Hz electrical service at the site for use with 110-volt power tools. Buyer shall provide a suitable unobstructed crane and truck
(tractor/40'trailer)access way to tank,which access is to be kept continuously above water,accessible,and passable during erection until all work by UC has been completed and
accepted. Should Buyer so desire,UC shall arrange to door have done the above work,all of which is for the account of the Buyer at a price to be agreed upon by UC and Buyer.
Buyer shall maintain the work area in safe and suitable condition throughout the course of the project. Buyer shall be responsible for storage and protection of materials and UC
equipment during times when UC and/or subcontractor erection crews are not at the construction site. Buyer shall not alter or modify the completed ULTRADOME unless such
modifications have been discussed with UC and agreed to in writing It shall be Buyer's responsibility to comply with all local building codes and to obtain all required building
permits.
3_ PRICES:Unless otherwise stated,prices do not include any license or permit fees,sales,excise,use or other such taxes now in effect or hereinafter levied. All such fees and taxes
are for the account of Buyer and shall be paid for by him. Our prices are based on current material prices,present freight rates and our current rates for shop and field labor. Should
- there be any changes in any of these prices or rates,our proposal shall be adjusted by the difference in our costs,including overhead and profit resulting from such changes.
4. TERMS OF PAYMENT:The full contract price shall he due and payable as follows. The material portion,or 75%of the contract price for each dome shall be due and payable by
the Buyer within 15 days after the dome materials have been delivered to the Buyer's tank site by UC. The remaining 25%of the contract price,or the labor portion for each dome
shall be due and payable within 15 days after the dome installation has been completed. Interest shall he payable on any over-due amounts at the rate of 1 1/2%per month or portion
thereof or the maximum rate allowable by law whichever is lower. If UC shall retain a collection agency and/or attorneys to collect amounts overdue,all collection costs,including
anorney's fees,shall be payable by Buyer. UC shall give at least 10 days written notice of its intention in this respect. Progress payments are required for orders in excess of
$500,000.00.
5. CREDIT:Fabrication,delivery,and erection of the ULTRADOME shall at all times be subject to the continuing approval of financial responsibility of Buyer by UC. If financial
responsibility and/or condition of Buyer reasonably appears to call for such action,UC may decline to fabricate,deliver or erect,or may require payment in advance or a suitable
guarantee that all invoices will be paid when due. If Buyer becomes insolvent or for any reasons fails to make payment when due,UC reserves the right to terminate the Contract
forthwith upon written notice to Buyer,without prejudice to any other remedy available to UC.
6. LOSS OF MATERIALS:All risks concerning damage to or loss of materials furnished to the Buyer under the Contract shall transfer immediately to the Buyer upon delivery of the
materials to the Buyer's facility or other point specified in the Contract.
7. WARRANTY:UC warrants free and clear title to all products delivered hereunder. On those installations where erection is performed by UC personnel and/or authorized
subcontractor,UC further warrants that the ULTRADOME shall be free from defects in materials or workmanship for a period of 12 months from the date of completion of work;
misuse,negligence,or ordinary wear and tear excepted,and UC agrees to repair or replace without expense to Buyer any workmanship or materials furnished hereunder that may
prove defective within said period. UC shall not be responsible for any consequential,special or contingent damages. Any claim by Buyer under this warranty shall be deemed
conclusively waived unless written notice is given to UC within 10 days after discovery of any claimed defective workmanship and/or materials. This warranty shall not apply to any
dome,which has been repaired,altered,or modified by anyone unless specifically authorized by UC so to do. The warranty as set forth herein is in lieu of all other warranties either
expressed or implied. ALL IMPLIED WARRANTIES,INCLUDING MERCHANTABILITY AND FITNESS FOR PURPOSE,ARE HEREBY DISCLAIMED AND EXCLUDED.
8. DELAYS:UC shall not be liable for any delays or failure in performance caused by any condition beyond our direct control including but not limited to Acts of God,unusual
delays in manufacture,unusual delays in transportation or shipments,strikes,lockouts,or other labor difficulties,fires floods,unavoidable causes not due to fault or negligence of
UC. UC shall not be liable for any delays or failure in performance caused by any act or neglect of Buyer,or by delays of our suppliers in furnishing materials or supplies due to one
or more of the foregoing causes.
9. PERFORMANCE:Unless otherwise specified in writing,UC shall famish all necessary materials,labor supervision and equipment for installation of the ULTRADOME in
compliance with the terms for this contract between Buyer and UC_ All materials shall be new unless otherwise specified and he suitable for the intended purpose. UC shall have the
right to subcontract all or any part of the work to one or more subcontractors of its selection provided each such subcontractor agrees to assume all the responsibilities and obligations
of UC with respect to said work. UC shall not permit any third party attachment or lien arising out of work to be performed hereunder to be imposed upon property of Buyer. UC
shall protect Buyer's property and adjacent property from injury or loss arising from UC and/or subcontractor performance of the work hereunder. UC shall not be responsible for
any injury or loss caused by Buyer or its representatives,employees,or contractors. UC shall at all II times keep Buyer's premises free from accumulations of waste materials
resulting from its operations. UC shall take all necessary precautions for the safety of employees on the work and shall comply with security,safety and fire regulations of Buyer in
force at the site of work. Buyer shall furnish all fire watch and lire protective services,all at its own expense. UC shall comply with all governmental laws relating to the
performance of work hereunder and certifies that all materials purchased by Buyer hereunder were produced in compliance with all applicable requirements of the Fair Labor
Standards Act of 1938,as amended. UC shall have the right to use its own discretion in the employment of labor. In case of interference in the work hereunder by reason of changed
conditions,local labor conditions,or any requirements on the part of buyer with reference to labor or other conditions,UC shall have the right to stop the work without prejudice until
such interference is satisfactorily adjusted. Work shall be performed on the basis of a normal 8 hour day and 40 hour work week,excluding holiday's,and weekends. Additional costs
incurred from working overtime at Buyer's request will be for Buyer's account. Should Buyer be responsible for any cessation of normal work procedure by UC and/or subcontractor
erection crews. Buyer shall accept billing and pay UC for all extra costs resulting therefrom. UC and/or its subcontractor shall indemnify Buyer against any valid claim for patent
infringement or trademark infringement. By notice in writing to the other party hereto,UC and Buyer shall have the right to make changes in the nature or extent of the work
required by this Contract within its general scope but no additional charges will be allowed unless authorized in writing by Buyer. If any such changes materially affects the
completion or costs of the work,UC shall promptly notify Buyer,and the parties hereto shall negotiate an adjustment. The method and agency of transportation and the routing will
be designated by UC. Due to a policy olcontinuous improvement,UC reserves the right to modify the design of the ULTRADOME_ Any such modifications shall not incur any
obligations to previous ULTRADOME owners.
10. INSURANCE:UC and/or subcontractors shall maintain the following insurance coverage in amounts not less than that indicated. Certificates of such insurance will be furnished
to Buyer on request.

It CANCELLATION:In the event that Buyer fails to accept materials ordered or advises UC of its intention not to accept delivery,buyer shall be liable for UC cancellation charges
which shall not exceed the purchase price of all undelivered materials and labor expended on the order at the time of cancellation plus 10%thereof,together with all handling and
shipping costs incurred by UC as a result of such cancellation.
12. CONSTRUCTION:this contract shall be construed under laws of the State of Texas.

♦ If supervision is required during installation, Ultraflote can provide an experienced supervisor at the rate of
$1,000.00 per day, from portal-to-portal. This rate is based on a five- (5) day at(8) hours per day normal
workweek, for a total of forty (40) hours. Should more hours a day or for the week be required, they will be
charged at the rate of$ 187.50 per hour. However, it is not recommended to exceed twelve (12) hours per day for
efficiency and safe working conditions for a supervisor. In addition, all travel and living expenses will be for the
customer's account.
• The estimated Man-hours are based on the productivity of an experienced Ultraflote crew. The installation contractor
should consider carefully the productivity of his work force, the site conditions and the construction strategy to be
employed when evaluating these estimates.

Our Installed Option Price is based upon delivery all materials at one time with installation being done in a continuous
construction program with one UNREPRESENTED field crew at prevailing wage rate of$45.69 per hour.
Ultraflote Corporation is an unrepresented company offering installation of these aluminum flat covers on a non union
basis.In the event there is a claim for the work by others Ultraflote will exit the site and invoice customer for our costs,plus
overheads and profit to date.
If customer elects to award the order for this project to Ultraflote Corporation,such award will be made according to these
conditions. Accordingly, if any labor difficulties result from the performance of this work,Ultraflote Corporation will exert
its best efforts to assist the customer in resolving any such dispute. Any expenses incurred by Ultraflote Corporation as a
result of such labor difficulties shall be for the account of the customer.
IF INSTALLATION LABOR IS PROVIDED BY ULTRAFLOTE CORPORATION THEN
THE FOLLOWING ARE TO BE PROVIDED BY THE PURCHASER OF THE FLAT PANEL COVER:
• Crane and required rigging for off loading materials.
• Crane for setting of flat panels and trusses (if required)
• Electric service (115 volt/30 amp) for small tool operation.
• Adequate, level dry working area as close as possible to the tank.
• Structure or site modifications other than construction of the Quad-Seal Welded Aluminum Flat Panel Cover are not
included in our scope of work.
• Any work of other trades, such as electrical, pipe fitting, painting, ductwork, or miscellaneous metal work (e.g.,
handrails, platforms, etc.).
