CONSENT FORM
The undersigned, T.M.H., LLC , being the owner of the property commonly known as 3225 West
146th Street, Carmel. Indiana (Parcel #17-09-20-00-00-004,0001 j hereby authorizes Lombardo Homes of
Indiana LLC and Faeare Baker Daniels LLP to file land development petitions necessary for the
aforementioned address. The foregoing consent is limited solely to the petitions and other filings
necessary to entitle the real estate so that the petitioner may develop a single-family residential
development. Nothing herein shall be construed as authorizing the aforementioned to encumber the
real estate with any commitments or other restriction.
This consent shall remain in effect until revoked by a written statement filed with the Department of

Range 3 East in Clay Township, Hamilton County, Indiana, being more particularly described as

Commencing at the Northeast corner of the Northwest Quarter of Section 20,Township 18
North, Range 3 East, Hamilton County, Indiana; thence South 89 degrees 39 minutes 18 seconds
West (assumed bearing) on the North line of said Northwest Quarter a distance of 712.15 feet

Lombardo Homes of Indiana -Tallyn's Ridge
Variances of Development Standards
Lombardo Homes of Indiana LLC ("Lombardo") respectfully requests five (5) variances from the
Development Standards of the Carmel Zoning Ordinance so that Lombardo may develop a high-quality
28-lot residential subdivision on the 18.5 +/- acre site located at 3225 West 146th Street, Carmel,
Indiana. The lots are proposed to be large (100' wide and 120' wide lots), the homes proposed are
expected to range from $400,000 to $600,000, and the proposed neighborhood will offer a lower
density than the nearby existing subdivisions (please see attached the illustrative home elevations, aerial
location map, and the proposed concept plan).
Details of Variance Requests:
In order to develop Tallyn's Ridge as proposed, Lombardo requests the following variances:
\00\006-\ 1. Section 5.01.02—Maximum Density: permits a Maximum Density of 1.0 lots per acre.
Lombardo requests a variance to permit 28 lots on this 18.5 acre site, which results in a
Maximum Density of approximately 1.51 lots per acre (lower than the nearby existing
subdivisions).
��6`��52. Section 5.04.02(D) — Minimum Lot Area: permits a Minimum Lot Area of 15,000 square feet for
lots with community water system and community sanitary sewer system.
Lombardo requests a variance to permit a Minimum Lot Area of 14,000 square feet.
3. Section 5.04.03(A)—Minimum Front Yard: permits a minimum front yard of forty (40)feet.
