In accordance with Seclion 19 of tfie General Administralive Rules(675 IAC 12-6-19)a complete set of plans and specifcations that conform
exaaly to the design that was released by the office of the state building commissioner shall be maintained on the conshuction jobsite as well as a
copy of the design release.

authority of the Fire Prevention & Building Safety Commission. As such, it is not comprehensive
with respect to the Americans with Disabilities Act or Fair Housing amendment, and what is

4G0603AE Detailed plans and specifications of the fire suppression system shall be filed with the required
application and appropriate fees in accordance with 675 IAC 12-6-3(a), 675 IAC 12-6-7(g)(17), and

Good morning Chris,
Thank you for your prompt responses. The details are acceptable, please insert them into the 0&M as indicated. The
engineer's estimate is also acceptable. I have no further comments on the SWPPP, 0&M manual, and storm water
control engineer's estimate.
The following will need to submitted before I can issue the storm water permit;
1. 9 final sets of plans for stamp and signature by the City Engineer and Carmel Utilities. (thought we might need
11, but county is not involved so only 9 are required)
2. Submit storm water control bond to Dave earnes in our office for 100%of the approved engineer's estimate.
3. Pay all fees to Utilities.
4. Submit Right of Way performance bond to Dave Barnes.
5. Obtain Right of Way permit from Fred Glaser in our department. flgaser@carmel.in.gov
6. Record (example attached) and submit final, bound O&M manual.
7. Submit completed storm water permit application.
When items 1-7 are complete, I can issue the storm water permit. After it has been issued, a pre-construction meeting
must be scheduled and held with Dan Greskamp before any construction can commence (d�reskamp@carmel.in.ROV or
317-571-2307).

Thanks for the review. Following please find our response of your review.
1. You are correct about the Aqua Swirl price. I used a price of an Aqua Swirl that we put in Greenwood, It was
much smaller.Attached please find the revised cost estimate for the SWPPP items and the email from the
manufacturer with [he Aqua Swirl A56 unit price.
