Westfield Blvd Carmel, IN 46032 Plan Review Comments The undersigned has reviewed the proposed plans for the above listed project and is forwarding the following comments for your review. All comments are requirements of the Indiana Fire Code 2008 Edition 6751AC 22-2.4 or the 2008 Indiana Building Code unless noted. 1. 907.2.10.1.2 Single-or multiple-station smoke alarms shall be installed in Groups R-2 R-3, R-4 and I-1 regardless of occupant load at all of the following locations: 1. On the ceiling or wall outside of each separate sleeping area in the immediate vicinity of bedrooms. 2. In each room.used for sleeping purposes. 3. In each story witfiin a dwelling unit, including basements but not including crawl spaces and uninhabitable attics. In dwellings or dwelling units with split levels and without an intervening door between the adjacent levels, a smoke alarm installed on the upper level shall suffice for the adjacent lower level pro4ided that the lower level is less 4han one full story 6elow the upper level. ( 9072.10.1.2 Groups R-2, R-3, R-4 and I-1. ) Additionally, all smoke alarms shall receive their primary power from the buildings power soUrce, shall be ecjuipped with a battery backup, a�d be interconnected in such a manner that the activation on one alarm will activate all the alarms within the individual unit. All apartment units shall have single station smoke detectors installed and shall comply with this section. 2. Provide full fire alarm system plans for each building for review. 3. Full automatic fire sprinkler system plans shall be provided for review for each building. 4. 905.3.1 Building height. Class I standpipe systems shall be installed throughout buildings where the floor level of the fiighest story is located more than 30 feet(9144 mm)above the lowest level ofthe fire department vehicle access, or where the floor level of the lowest story is located more than 30 feet(9144 mm) below the highest level of fire department vehicle access. N A full standpipe system shall be installed in building A. The installation shall comply with the 2008 Indiana Fire Code and NFPA 14. 5. 1208.2 Attic spaces: An opening not less than twenty (20) inches by forty (40)inches (Five hundred eight (508) mm by one thousand sixteen (1,D16) mm shall be provided to any attic a�ea having a clear height of over thirty (30) inches (seven hundred sixty-two (762) mm). A thirty (30) inch (seven hundred sixty-two (762) mm) minimum clear headroom in the attic space shall be provided at or above the access opening. When the access opening penetrates fire-resistive construction, the access opening closure shall provide the fire- resistive construction as requi�ed for the fire-resistive construction that is penetrated for the opening, and may be manufactured or field assembled, and shall be tight fitting. 6. 1024.6 Access to a public way. The exit discharge shall providea direct and unobstructed access to a public way. Sidewalks shall be provided from each required exit to a public way. 7. 1410.1 Required access. Approved vehicle access for firefghting shall be provided to all construction or demolition sites. Prior to construction beginning above grade and the arrival of combustible materials on site, Emergency Vehicle access shall be provided by roads with at least an asphalt binder surface capable of supporting vehicle loading under all weather conditions. Vehicle access shall be maintained unobstructed at all times. 8. 1412 WATER SUFPLY FOR FIRE PROTECTION. An approved water supply for fire protection, either temporary or permanent, shall be made available as soon as combustible material arrives on the site. Fire Hydrants shall be fully tested/inspected, accessible, and placed in service prior to combustible material arriving on site. 9. 506.1. Key Boxes Whe� Required.Whenever the servicing fire department has instituted a key box emergency access system, a key box compatible with that system shall be installed in an accessible location if: (1) the building is protected with an automaticsprinkler system equipped with a local or transmitted water- flow alarm, or (2) the building is provided with any fire alarm system equipped with an outside audible/visual signaling device, or (3) the building is provided with any fire alarm system where the alarm is transmitted to an off-site location, or to the fire alarm center for the servicing fire department. Each individual building shall be provided with its own Knox Box at an approved location. 10. 912.3.1 �ocking fire department connection caps. Provide app�oved locking caps on fire department connections for water-based fire protection systems where the responding fire department carries appropriate key w�enches for removal. Knox locking caps shall be provided for each fire department connection. S Please respond to the above noted condition(s) in writing and fonvard this to my attention. So I replied to all. Please find the following comments regarding this site that need to be addressed prior to the Department of Engineering supporting the release of the ILP. 1. A right-of-way permit with a $5,000 license and permit bond is required for construction equipment access to the site from the public right-of-way. We will need to know where this will occur. 2. A pre-construction meeting is required prior to starting any work on-site. 3. An estimate of the value of the construction phase erosion and sediment control measures shall be provided for review. Upon approval of the estimate, a performance bond equal to the value of the estimate shall be provided to the City. 4. Please indicate the construction limits on all sheets. 5. If the perimeter of the overall pad is flat (all at the same elevation) and certain parts of the pad near the tower are to be elevated for drainage away from the tower, there will.be an area of ponding water along the northern limits of the overall pad. 6. Piease indicate the intent of the second Sheet C-5. 7. Please indicate the slope from the proposed access drive to the existing grade. There appears to be several feet of fall to the existing flowline. 8. Detail 3-D/2 shall be replaced with the City standard detail for such curb. 9. The City curbing policy shall be added to the plan set. 10. The Department assumes that a concrete washout will be needed. Please indicate the location and provide a detail of the area. The loeation must be interior to the perimeter erosion control measures. 11. The permanent erosion stabilization measures shall be indicated.At a minimum, mulched seeding is required. 12. In generel, the erosion and sediment control measures need to be more clearly defined on the plans. 13. The City standard is for all slopes 6:1 and greater be stabilized with an erosion control 6lanket. 14. Scour protection is needed on each end of the proposed drive culvert. 15. A check dam is required on the downstream end of the proposed culvert. 16. The silt fence along the southern limits of construction shall extend across the entire limit of construction and may tie into each side of the check dam. 17. The western end of the siit fence along the southern limits of construction sha�l be extended and run perpendicular up the slope for a minimum distance of 8-feet (if that is the standard section of the silt fence being used). 18. The sides of the swale on the east side of the overall pad shall be stabilized with erosion control blankets. 19. The silt fence shall be wrapped to run parallel to the construction entrance a minimum of 16-feet (or two section of the silt fence being used) on each side of the entrance. 20. The City minimum cover does not appear to be provided over the proposed culvert. The Department is OK with this as this is a private culvert under a private drive. 21. The proposed culvert does not meet the minimum culvert size of 18-inches. Was the 12-inch culvert determined to be adequate for the watershed that will drain through this culvert?
