Company Amount Alternative Alternative Page 1 of 6 CITY OF CARMEL ENGINEERING DEPARTMENT ONE CIVIC SQUARE CARMEL,IN 46032 REQUEST FOR QUOTE FOR 116TH STREET AND EDEN GLEN DRIVE DRAINAGE IMPROVEMENTS September 16, 2011 To: Yardberry Landscape Excavating Co. 9940 South 1000 West Anderson,IN 46017 Contact: Tim Yarberry Ph# 765-378-5783 Fax#765-378-0376 tyarb(a,aol.com Re: Request for Quote for Drainage Improvements at 116th Street and Eden Glen Drive at 11601 Eden Glen Drive and 3528 E. 116th Street, Carmel, IN 46032 To whom it may concern: Notice is hereby given that the Board of Public Works and Safety for the City of Cannel, Hamilton County, Indiana will receive sealed quotes for the "116TH STREET AND EDEN GLEN DRIVE DRAINAGE IMPROVEMENTS" at the office of the Clerk Treasurer, One Civic Square, Carmel, Indiana (City Hall) anytime prior to, and no later than 10:00 a.m. Local Time on, October 5, 2011, and commencing as soon as practicable thereafter during the Board of Public Works Meeting on the same date, such quotes will be publicly opened and read aloud in the Council Chambers of City Hall. No late bids will be accepted. All bids and proposals shall be properly and completely executed on the proposal forms provided with this request and specifications and submitted as follows: 1. Bid Proposal Summary Page and Bid proposal Sheet,pages 5 and 6. 2. Followed by a copy of this request, sheets 1-4. The bid envelope must be sealed and have the words "BID — 116TH STREET AND EDEN GLEN DRIVE DRAINAGE IMPROVEMENTS". It is intended that actual construction of all work divisions shall be started as soon as practicable or as directed, and each bidder shall be prepared to enter promptly into a construction contract, furnish a performance bond, and begin work without delay in the event the award is made to him. Page 2 of 6 The Project consists of,but is not necessarily limited to,the following: Installation of pump drainage system for drainage and driveway culvert pipe including excavation, grading, pipe and restoration as outlined in the provided construction documents. Bidders shall assure that they have obtained complete sets of drawings and Contract Documents and shall assume the risk of any errors or omissions in bids prepared in reliance on incomplete sets of drawings and Contract Documents. This Project will be funded by City of Carmel. For special accommodations needed by handicapped individuals planning to attend the public bid opening meeting, please call or notify the city of Carmel, Engineer's Office, at (317) 571-2441 at least forty-eight (48)hours prior thereto. The City of Carmel reserves the right to reject and/or cancel any and all bids, solicitations and/or offers in whole or in part as specified in the solicitations when it is not in the best interests of the governmental body as determined by the purchasing agency in accordance with IC 5-22-18-2. Specifications: Please refer to INDOT 2010 Standard Specifications. 1, General: Please find construction drawings and details included. Construction consulting and coordination information shall be provided to the awarded firm with the lowest and most responsive quote. Quotes shall be based on itemized proposal items and shall include all incidental work to complete the improvements required and this shall include removals and any work for which no specific pay items exist. Inspection and consultation for the project shall also be provided by CrossRoad Engineers. Coordination shall not be paid separately but shall be included within the cost of the items in the itemized proposal. The Work shall consist of installation of a pump and piping as shown in the plans including electrical supply to the pump from the existing service at the house located on the property. The work shall also include directional drilling of a culvert pipe and any grading and restoration to return the property to a satisfactory growth of lawn in the work areas or any disturbed areas to complete the work. 2. Common Excavation and Grading: All removals and costs associated with such removals shall be included in the items in the itemized proposal. The items shall include common excavation, saw-cutting, or any other removals necessary for the installation of the improvements. 3. Restoration shall include mulched seeding of any excavated or graded lawn areas. Top soil shall be placed at a minimum depth of three inches and fine graded for the suitable growth of vegetation. The top soil shall be per INDOT specifications and graded such that settlement to the surrounding grade of the excavated area is accounted for. 4. Concrete: In addition to INDOT Standards, the City of Carmel Standards and concrete paving policy shall apply to all concrete work. Any damage to pavement to remain in place shall be repaired at the contractor's expense and at Page 3 of 6 the Engineers/City of Carmel's discretion unless damages occur beyond the control of the contractor's reasonable protection,care and diligence to maintain. 5. Protection of existing structures: protection of existing structures to remain in place including but not exclusive to the following: concrete curb or walk, mailboxes, fences, building structures, infrastructure, etc. shall be the responsibility of the contractor. Damages to structures to remain in place due to any portion of the work shall be replaced or repaired at the contractor's expense unless found to be in direct conflict with the improvements. It is not anticipated that any permanent structures will be in the way of the work. It shall also be the contractor's responsibility to protect the work during non-working hours, including industry standard methods for protecting the work for general safety. 6. Coordination: the contractor shall coordinate all work with the property owners and CrossRoad Engineers at: contact info to be provided upon acceptance of quote and Notice to Proceed. 7. Schedule and work restrictions: The selected contractor shall coordinate work with the property owner,the City of Carmel and CrossRoad Engineers. No work shall take place after 7:00 PM in the evening to the following morning at 7:00 AM or on Sundays unless prior approval is obtained from the property owners or the City. The selected contractor shall have the improvements completed within 30 calendar days of a Notice to Proceed unless receipt of materials becomes an issue and can be proven to not be available until a date beyond the completion date provided. If this becomes the case a schedule extension will be provided as necessary. Completion shall include grading and lawn restoration. If lawn restoration cannot be completed by the completion date due to specification and weather requirements, these shall be completed as soon as feasible and achievable within specification or at Engineers/City of Carmel's direction. Retainage held will be released upon full completion of any remaining temperature sensitive work and acceptance of the completed project. 8. Maintenance of Traffic: It will be the selected contractor's responsibility to establish temporary traffic control or maintenance of traffic in accordance with the Indiana Manual on Uniform Traffic Control Devices for Streets and Highways. Any maintenance of traffic necessary per the Indiana Manual on Uniform Traffic Control Devices for Streets and Highways shall be included within the items in the itemized proposal. 9. Utilities and permitting: it shall be the contractor's responsibility to coordinate all utility locates. It shall be the contractor's responsibility to coordinate and acquire any necessary permits, A City of Carmel Right-of-way permit will be necessary for any work in the right-of-way and contact information for this permit will be provided to the selected contractor. In the event that a permit fee is required it will be change ordered into the contract but no fee should be anticipated for quoting purposes. 10. Notes: The above is a brief description of the work and the attached drawings and details shall supplement the description herein.
