Re: Clearing quotes for the Main and Keystone Parkway Intersection in Cannel. IN
The following is a Request for Quotes by the City of Carmel for tree clearing and
building demolition necessary for utility relocations at Main Street for the Keystone
Parkway Reconstruction Project in Cannel. IN. The City of Cannel is pursuing and
accepting quotes for review and anticipating awarding the project for construction at the
December 2. 2009 Board of Public Works Meeting. If awarded. the City would require a
contract with the awarded contractor. All right-of-way is currently staked and a field
visit to review the areas of clearing is suggested but not required. The following should
also be considered in the quote:
I. The quote should he for all work shown in the Contract Items and Unit Prices
Summary Table for the entire intersection as directed by the City of Carmel and in
conjunction with the attached plans.
2. Tree clearing shall consist of any tree shown within the right-of-way and per the
plans and any trees designated otherwise by the Engineer. Tree removal shall
include stump removal unless directed by the City of Carmel or the Engineer.
3. Anything smaller than 6" will be considered brush removal and should be
removed, including the stump.
4. Restoration will he required at the time of clearing. The City requests a separate
price for -Mulched Seeding. U", including fertilizer and grading if necessary.
This shall be paid by the square yard (SYS).
5. The areas of removal will be staked with -light-of-way" or "clearing limits"
stakes. Any removal outside of these stakes without prior approval of the City of
Carmel will he the responsibility of the contractor, The City of Cannel will not
he liable for any unauthorized removal.
6. The cost for all Maintenance of Traffte necessary to complete the project shall not
be paid for directly but be included in the cost of other items. No road closures
wiEl be considered and any restrictions shall be coordinated with the City of
Carmel at least 48 hours in advance of the restriction.
7. All incidental labor; equipment, materials and any other incidental work necessary
to complete the work as described above shall he included in the Lump Sum cost
for the work.

8. All work performed on this project shall be in compliance with the 2010 INDOT
Standard Specifications Book.
Thank you for your consideration of this project. The deadline for submitting a quote is
9:00 am on November 18, 2009. Quotes must be hand delivered to the City of Cannel

DEMOLITION OF EXISTING STRUCTURES
1.4 THE WORK
1.1 The Structures. The successful Bidder(Contractor) shall provide all labor,
material, equipment, machinery, tools and services necessary for the complete
dismantling, demolition and removal of the following structures located in the
City of Cannel hereinafter referred to as the "Work":
(1) Structure A, (Parcel No. 66), 9 Carlin Drive Lot 43 in
Carmel Meadows Section 2. A property description is
included in Exhibit A. City has possession of this
structure.
(2) Structure B, (Parcel No. 75), 918 Main Street E. Lot 188 in
1:Iarrowgate Section 3. A property description is included
in Exhibit B. Current Property owner to vacate the
property on or before November 24, 2009.
(3) Structure C, (Parcel No. 74), 810 Main Street E. Lot 174 in
Harrowgate Section 3. A property description is included
in Exhibit C. Current Property owner to vacate the
property on or before December 1, 2009..
(Hereinafter referred to as the "Structures"). For purposes hereof, each of the
Structures consists of the entire structure, including all pieces, parts, components,
and materials thereof, the building foundation, basement walls, roof, concrete
pads, and all components, attachments and fixtures thereto and equipment and
machinery therein, including appurtenant structures such as chimneys, stairways,
and porches. In addition, all nonattached improvements on the lot including, but
not limited to, sheds, garages, sidewalks, fences, and driveways shall be removed
unless otherwise directed by the City. All such pieces, parts, components,
materials, attachments, fixtures and equipment of the Structures shall hereinafter
be referred to collectively as the "Property."
SECTION 202, BEING LINE 232, DELETE AND INSERT AS FOLLOWS:
The houses and buildings shall be demolished and removed completely. down to a
point 1 ft-(O.3m)below the original ground level or the subgFadc elevation,
whichever is lower.
SECTION 202, BEGIN LINE 235, DELETE AND INSERT AS FOLLOWS:
Prior to starting demolition operations, or when directed, all existing utilities shall
be terminated and all floor drains sealed in a satisfactory manner. Water and
sewer lines shall be capped at the main and removed completely as directed by
the Engineer and the City of Carmel Utilities Department,

1.2 Time of Work.
Structure A - Contractor shall commence the Work on Structure A within seven
(7) days of receipt of a notice to proceed from the City of Carmel. Expected
completion time for the removal of Structure A is seven (7) days or less.
Structure B —Contractor shall commence the Work on Structure B within seven
(7) days of receipt of a notice to proceed from the City of Carmel. Expected
completion time for the removal of Structure B is ten (10) days or less.
Structure C- Contractor shall commence the Work on Structure C within seven
(7) days of receipt of a notice to proceed from the City of Carmel. Expected
completion time for the removal of Structure C is seven (7) days or less.
1.3 Proposed Work Plan. Bidders shall submit a proposed work plan, including all
procedures proposed for the accomplishment of the Work in a safe and workman
like manner. If the successful Bidder determines that the proposed work plan
must be altered, a revised work plan shall be submitted for approval of the City of
Cannel before the Work may begin.
1.4 Utilities. Contractor shall disconnect each building, both above and below
ground, from all utility services, including electric, water, steam, sewer (sanitary
and storm) and natural gas prior to commencement of any demolition activities.
Utilities facilities within each building shall be removed as part of the Work.
Water, steam and sewer lines shall be capped and/or removed to the main facility
as directed by the City Engineer for the City of Carmel, Indiana. Contractor shall
have no claim to any right, title or interest in a utility facility upstream of the
location where such utility is disconnected. In the event that Contractor discovers
any utility service that has not been turned off, Contractor shall immediately
contact the Engineer so that appropriate arrangements may be made to have the
service turned off.
1.5 Well Abandonment. All existing wells on a given property must be properly
capped and abandoned prior to demolition of the structure. Well abandonment
must be performed by a state licensed and certified well driller who is also a
registered well driller in the City of Cannel, and should be completed in
accordance with all applicable State and local regulations. Well abandonment
will not be paid separately, but should be included as a part of the Work.
1.6 Septic Tank Removal. if present, Contractor shall pump all private septic tanks so
they are free of any waste and debris. The existing tank shall be pumped and
completely removed and hauled away from project site prior to demolition of the
structure. Existing finger systems may be capped and abandoned in place.
Disposal of all waste and materials shall be completed in accordance with all
applicable state and local regulations. Contractor will be responsible for

contacting the Hamilton County Health Department prior to and upon completion
of'septic tank removal. Septic tank removal will not be paid separately, but
should be included as a part of the Work.
1.7 Site Vacancy and Final Grade. Contractor shall remove all salvageable Property,
unsalvageable Property, waste,rubbish and debris, and equipment within ten (10)
days of final completion of the demolition of the buildings. Contractor shall leave
the Work sites) in a smooth and level graded condition, which includes the
application of mulched seeding Type U in accordance with INDOT specifications.
2.0 STANDARDS, LAWS AND PERMITS.
2. 1 Standards. Contractor and all subcontractors shall perform the Work in
accordance with the requirements of applicable standard specifications of the
Indiana Department of Transportation,
2.2 Laws. Contractor and all subcontractors shall comply with all applicable
requirements of state and federal laws, regulations, rules, and ordinances
including, but not limited to, those concerning occupational safety and health and
environmental protection.
2.3 Permits. Contractor shall obtain any and all local or state permits, licenses or
other approvals necessary for the performance of the Work, including hut not
limited to a demolition permit from the City of Cannel.
3.0 WORK SITE REQUIREMENTS.
3.l Boundaries. The boundary of the Work site for each structure shall he clearly
marked by the City of Carmel.
3.2 Site Safety. Contractor shall protect the site(s) of the Work with barricades,
fencing, temporary barriers, electric flashing lights, and/or any other equipment or
signals that, in the judgment of the Engineer, adequately guards excavations, piles
of rubbish and debris, partially dismantled portions of the Structures, equipment,
loose materials, walkways, streets, or any other potentially hazardous conditions.
Contractor shall immediately notify the Engineer of the discovery or creation of
any condition that maybe hazardous to the public.
3.3 Protection of Surrounding Properties/Structures. Contractor shall perform the
Work and coordinate its Work with that of others, if any, in such a manner as to
protect surrounding real estate, property and streets from damage and disruption.
Contractor shall ensure that structural elements of any of the Structures being
demolished are not overloaded, and shall construct and maintain all appropriate
shoring, bracing and supports. The use of explosives is strictly prohibited.
3.4 Environmental Protection. Contractor shall perform the Work with due
consideration and all practicable precautions to prevent, abate or control noise,

dust, wind-borne debris, smoke, and pollution of the air, water and land
surrounding the site of the Work. Contractor shall immediately notify the
Engineer of the discovery or creation of any environmentally hazardous
condition. Burning or other use of fire at the Work site is strictly prohibited.
3.5 Erosion Control. If determined to be necessary by the City, an appropriate
erosion and sediment control plan meeting the requirements of Indiana
Administrative Code, Title 15, Chapter 5 shall be submitted to the City of Carmel
and the Soil & Water Conservation Service for approval before the
commencement of any Work.
4.0 CONTROL AND HANDLING OF SALVAGED PROPERTY AND WASTE.
4.1 Salvaged Property. Upon commencement of the Work, Contractor shall obtain all
right, title and interest in the Property. Accordingly, Contractor may salvage for
reuse or resale any or all of the Property; provided, however, no person who is not
an employee of Contractor shall be give permission to remove any of the Property
from the site of the Work without prior approval and coordination of the City of
Carmel. Contractor shall remove all such Property from the Work site(s) by the
completion date of the Work. Any of the Property removed from the Structure
site for reuse or resale by the Contractor shall be so removed, reused and resold in
accordance with all applicable state, local and federal laws and regulations. The
City of Carmel may, in its discretion, require Contractor to document the ultimate
destination of any or all materials removed from the Structure site by or at the
direction of Contractor, whether for reuse, resale or disposal.
4.2 Waste. Any and all of the property that is deemed by Contractor to be
unsalvageable and all rubbish and debris generated by the Work shall be removed
from the Work site(s) and properly disposed of on a daily basis, unless otherwise
directed or authorized by the Engineer. In the event that any rubbish, debris or
other waste cannot be removed daily, then such items shall be stored in areas
designated by the Engineer and stored in such a manner as to avoid the creation of
dust and wind-borne debris. All waste materials removed from the Structure site
for disposal shall be transported and disposed of in accordance with all applicable
state, local and federal laws and regulations.
5.0 MEETING ATTENDANCE. Contractor shall attend meetings with the City of
Carmel, or their agents or representatives, as may be requested to explain the
Work from the Work site(s) by or at the direction of Contractor.
