ADDITIONAL SERVICES AMENDMENT TO
AGREEMENT FOR PURCHASE OF GOODS AND SERVICES
THIS AMENDMENT TO THE GOODS AND SERVICES AGREEMENT ( "Agreement ") entered into by and
between the City of Carmel and Cross Road Engineers (the "Vendor"), as City Contract dated May 1, 2013 shall
amend the terms of the Agreement by adding the additional services to be provided by Vendor consistent with the
Scope of Work attached hereto and incorporated herein as Exhibit "A ". The terms and conditions of the Agreement
shall not otherwise be affected by this Additional Services Amendment and shall remain in full force and effect.

Fee Proposal for Professional Services
Dear Jeremy:
We are pleased to submit this proposal to provide engineering services for development of this
Guilford Road Project. Through our meetings and preliminary research, we have prepared a
detailed scope of services needed for completion of the project.
In anticipation of project construction receiving Federal Funding administered by INDOT, the
professional services will be performed in accordance with FHWA and INDOT guidelines and
requirements, and are described in the attached Description of Anticipated Work Elements.
Following the description of work elements is a Fee Schedule that presents each service, the
proposed fee budget for that service, and the basis of payment.
We will utilize subconsultants to perform some of the services. The functions expected to be
performed by subconsultants include Section 106 Documentation, Geotechnical Investigation,
Pavement Design, Title Research, Appraising, and Buying services.

PROJECT DESCRIPTION
This project involves construction of Guilford Road from City Center Drive to Main Street. It is
anticipated that Guilford Road will be a mill and overlay section from City Center Drive north to
126th Street. From 126th Street north to the intersection of Main Street, the project is anticipated
to be total reconstruction with curb and gutter and associated drainage structures. A two way
left turn lane would also be added through this section. In addition, the Main Street intersection
would be converted to a roundabout, with total reconstruction on the north, east, and west legs
extending approximately 300 feet in length. The existing crossing culvert that carries Guilford
Road over the adjacent tributary to Little Cool Creek will be replaced.
DESCRIPTION OF ANTICIPATED WORK ELEMENTS
Supplemental Topographic and Route Survey — This project was originally surveyed in 2006.
That survey data will be checked and all new construction areas will be surveyed and imported
into the project so revisions can be made. The survey will be performed in accordance with
Indiana Code and the INDOT Design Manual, which will include the addition of a Certified Route
Survey Plat (not previously completed for this project). The Route Survey Plat will be prepared
in accordance with Section 865 of the Indiana Administrative Code.
Environmental Document Preparation — This task includes the services necessary to complete
the environmental documentation required by the National Environmental Policy Act (NEPA) and
other pertinent and applicable laws and regulations. It is assumed that a Level 2 or 3 Categorical
Exclusion document will be required. Included is the early coordination with the various
regulatory agencies that is required. In order to satisfy the Section 106 documentation
requirements, several analyses will be performed, including an archaeological records check and
an historical / architectural records check. No Phase la field reconnaissance nor additional
archaeological studies are included.
Road Design and Plan Preparation — In general, this task involves the preparation of design plans
and bidding documents to allow the proposed improvements to be constructed. All of this work
will be performed in accordance with FHWA, INDOT and City of Carmel guidelines and standards.
Additionally, INDOT's LPA process will be followed. In an effort to keep the City as informed and
involved as possible, and in an effort to receive INDOT approvals as the project develops,
submittals will include Stage 1 Submittal and Stage 2 Review, in addition to the required Stage 3
and Tracings submittals.
Geotechnical Evaluation & Pavement Design — A geotechnical report was previously completed
by Alt & Witzig Engineering, Inc. (A &W). A &W will update the existing tests and report as
necessary to comply with current INDOT Standards and Specifications. A &W will supplement its
work by performing the borings, subgrade investigation, and laboratory testing necessary for a
proposed roundabout at the intersection of Main Street and Guilford Road. Please see the
aof
attached proposal from A &W for the detailed scope of work and estimated fee. Pavement
design, in accordance with current INDOT procedures, will be included and performed by Earth
Exploration, Inc. (EEI). Please see the attached proposal from EEI for the detailed scope of work
and estimated fee.
Utility Coordination — This work will follow the current INDOT process for federally funded
projects. Included will be research to confirm all of the utilities located within the project limits.
We will coordinate with the representatives from each of those utility companies though the
development of the project. We will communicate any relocation of facilities that may be needed
and then review the relocation plans that the utility companies prepare. We will prepare the
reimbursable agreements for the affected utility companies and review their proposed
reimbursable relocation costs. This task does not include work associated with field locating the
vertical depth of any utilities. Although not anticipated (since the utilities should perform this
work on their own), if any 'potholing' of facilities will be required, this service will be performed
separately and is not included.
Public Hearing / Public Information Meetings — Although there are federal public involvement
requirements that must be met, an official public hearing may not be required. Regardless, we
propose holding a total of two (2) public meetings if desired by the City. These meetings will
provide a forum for sharing project information between all stakeholders. One public meeting
will be held during the design phase and will include satisfying the public involvement
requirements, if necessary. The second will be held just prior to the start of construction. The
costs that are presented for this effort include preparation for the meetings as well as attendance
at the meetings by CrossRoad personnel. It is expected that the City will provide a facility for
holding the meetings as well as a transcript of the public hearing proceedings, if one is required.
Permit Application — It is anticipated the construction of the project will disturb more than one
acre of ground and therefore an Indiana Department of Environmental Management (IDEM) Rule
5 Notice of Intent will be required. Included is the preparation of a Storm Water Pollution
Prevention Plan, submittal of public notice, and permit application forms. No other permits are
anticipated to be required.
Right of Way Engineering —This work will include performing the right of way engineering as well
as the preparation of property plats and legal descriptions for those right of way parcels that
need to be acquired for the project. This effort also includes a 20 year title research for the
permanent right of way to be acquired.
Right of Way Acquisition Services — This work includes the management and all of the activities
necessary to procure the required parcels of right of way. These activities include the appraisal
problem analyses, the appraisals, appraisal reviews, buying of right of way, and right of way
management. It also includes a title update and title insurance commensurate with the offer
amount. Transfer documents will be prepared and recorded. If requested, the proposed right of
way will be field staked.

Inc. for pavement design. The final cost will consist of the actual invoiced amounts from the subconsultants.
