EMERGENCY SPILL RESPONSE PLAN
Giant Eagle Station
146th and Gray Road, Carmel, IN
I. PURPOSE
This plan includes critical activities to be completed in the event of a
petroleum hydrocarbon fuel release.
II. SCOPE
This policy applies to the Giant Eagle, Inc. Fuel & Convenience Store
located at 146th and Gray Road, Carmel, Indiana.
III. PROCEDURES
Critical training response procedures contained within this Spill Response
Plan are provided below for quick reference in the event of an emergency.
1. In the event of an active fuel spill, trigger the emergency shut-off switch.
Clear the area of running vehicles and open flame.
2. If the amount of fuel spilled appears less than five (5) gallons, cordon off
the area of the release using cones, cover vulnerable drains with drain
boots, and proceed to contain and clean up the release using materials
provided within the on-site spill response kit (See Section IV).
3. In the event of a fuel spill greater than five (5) gallons, the following
entities should be contacted immediately:

4. In the event of a fuel spill greater than fifty-five (55) gallons, the following
additional entities will be contacted by the Giant Eagle, Inc. Loss
Prevention Department. Be aware that personnel from the following
organizations may contact you or the facility manager for details.

Please be ready to assist the Loss Prevention Department in completing
the spill reporting form provided in Appendix A.
5. Take measures to contain the spread of releases greater than five (5)
gallons only after establishing that containing the release can be
performed without risk to life and/or property; and
a. When advised to do so by the fire and/or police department, and
b. When advised to do so by the Giant Eagle, Inc. Loss Prevention
Department.
IV. SPILL RESPONSE KIT CONTENTS
A Universal Non-aggressive Spill Kit shall be placed within the station building in an
accessible location. The kit shall include the following materials, at a minimum:
i. Fifty (50) absorbent pads,
ii. Eighty (80) linear feet of sorbent socks
iii. Ten (10) 18 x 18" sorbent pillows
iv. Nitrile gloves
v. Disposal bags
vi. Drainage boots of sufficient quantity and size to seal/cover all
drains within the fueling area.
V. TRAINING REQUIREMENTS
Facility staff shall receive annual training on spill response procedures. New
employees will be trained as they are hired in. Each training event will be recorded in
the training log included in Appendix B and retained on-site.
VI. SUPPLIER/TRANSPORTER NOTIFICATION
The fueling facility is in a W-5 of the City of Carmel Wellfield Protection Zone. All fuel
and chemical product/waste suppliers and/or transporters should be notified of the
placement of the facility within a wellfield area. Such notification will make
suppliers/transporters aware of the sensitivity of this area so that they might
exercise an added degree of care and precaution in completing work activities. A
notification form is provided in Attachment C.

XI. PLAN OPERATIONAL LIFETIME
This policy/plan has an operational lifetime of three (3) years, after which time it
must be reviewed, revised, and again signed by the above party.
ATTACHMENT A
SPILL REPORT FORM
(According to 327 IAC 2-6.1-4 Definitions)
Report the following information about a reportable spill orally within two (2) hours of
discovery to the Indiana Department of Environmental Management (1-888-233-7745, in-
state; 1-317-233- 7745, out-of-state) to the extent that the information is known at the time
of the report:
(A) The name, address and telephone number of the person making the spill report.
(B) The name, address and telephone number of a contact person, if different from clause
(A).
(C) The location of the spill.
(D) The time of the spill.
(E) The identification of the substance spilled.
(F) The approximate quantity of the substance that has been or may further be spilled.
(G) The duration of the spill.
(H) The source of the spill.
(I) Name and location of the waters damaged.
(J) The identity of any response organization responding to the spill.
(K) What measures have been or will be undertaken to perform a spill response.
(L) Any other information that may be significant to the response action.

DOCUMENTATION OF TRAINING - REVIEW EMERGENCY
RESPONSE/SPILL PREVENTION PLAN IMPLEMENTATION
The following facility employee(s) of the Giant Eagle, Inc. Fuel and Convenience Store
located at 146th and Gray Road, Carmel, Indiana, and any product delivery
personnel have received training in implementing the site Emergency Response/Spill
Prevention Plan as required by the City of Carmel Wellhead Protection Zoning
Ordinance.

ATTACHMENT C
NOTIFICATION TO CHEMICAL
PRODUCT/WASTE SUPPLIERS OR
TRANSPORTERS
City of Carmel Wellfield Protection Information
Giant Eagle, Inc. Fuel and Convenience Store located at 146th and Gray Road,
Carmel, Indiana is situated within a designated W-5 of the City of Carmel Wellfield
Protection Zone. As such, the use, storage and management of chemical products or
the generation of wastes that can potentially contaminate the underlying groundwater
is closely monitored and controlled within this area to protect the City's drinking water
supplies. As a supplier and/or transporter of chemical products or wastes to or away
from the facility, you are being made aware of the sensitivity of this area in order to
exercise an added degree of care and precaution in completing your work activities.
The following restrictions shall apply:
1. Personnel involved in the delivery and transport of products or materials to or
away from the site shall have proper training in handling of chemical product or
waste spills.
2. During delivery or transport, any liquids or solids that could potentially become
groundwater contaminants shall be properly sealed in appropriate containers and
handled to prevent accidental leakage or spillage.
3. In the event of a chemical or waste spill or discharge, personnel shall
immediately implement the required spill prevention plan in effect for the activity.
4. The appropriate state and local regulatory entities shall be notified of the spill or
release.
