ADDITIONAL SERVICES AMENDMENT TO
AGREEMENT FOR PURCHASE OF GOODS AND SERVICES
THIS AMENDMENT TO THE GOODS AND SERVICES AGREEMENT ("Agreement") entered into by and
between the City of Carmel and American Structurepoint, Inc. (the 'Vendor"), as City Contract dated March 20, 2013
shall amend the terms of the Agreement by adding the additional services to be provided by Vendor consistent with
the Scope of Work attached hereto and incorporated herein as Exhibit"A". Furthermore, the terms of the Agreement
shall be amended to include the E-verify requirement as stated in Indiana Code § 22-5-1.7 et seq.,which is attached
hereto and incorporated herein as Exhibit "B", as well as the required E-verify Affidavit, attached hereto and
incorporated herein as Exhibit "C". Also, pursuant to I.C. § 5-22-16.5, Vendor shall certify that in signing this
document, it does not engage in investment activities within the Country of Iran. The terms and conditions of the
Agreement shall not otherwise be affected by this Additional Services Amendment and shall remain in full force and
effect.

PROJECT ENGINEERING ASSISTANCE
A. Engineering/Technical Services/Scope of Services Contract Development
I. The ENGINEER shall evaluate the schedules of the approved program projects and assist
the OWNER with the development of the necessary consultant engineering and/or technical
scope of services required in conjunction with the procurement of these services related to
specific program projects and consultants providing these services. The ENGINEER shall
evaluate each specific project scope of services at the appropriate time prior to procurement
of the professional services.
2. The ENGINEER shall provide the consultant scope of services to the OWNER for final
approval. Once approved, the consultant scope of services shall be provided by the
OWNER to the specific consultant. If modifications of the consultant scope of services are
required due to project negotiations, the ENGINEER shall make such modifications.
B. Scoping/Engineering Reports
1. The ENGINEER shall develop project scoping reports for projects assigned by the
OWNER, based upon the function, scale, cost, and operations identified by the OWNER.
This stage of the project development is referred to as the Scopmg Report (SR).
2. The ENGINEER shall prepare SRs outlining the scope of services for each capital project
within the Program. as assigned to the ENGINEER by the OWNER. The SRs for those
project(s) assigned to the ENGINEER shall vary as to the level of detail, depending upon
the nature of the project.
3. An SR to be developed by the ENGINEER shall be focused predominantly on one category
(e.g., roadway, storm water conveyance system, multi-modal facilities, or other
transportation infrastructure). The SR shall be prepared by the ENGINEER after an
evaluation of the budget. schedule, and goals anticipated for the project. The SR shall be
reviewed and approved by the OWNER.
4. The ENGINEER's SR shall be prepared in a fashion. and with sufficient information, so
that the report may provide the OWNER with the basis for the preparation of an Engineer's
Report (ER) in the event such is required as a part of a federal-aid project. The report shall
also provide the OWNER with sufficient information to hire a designer to proceed with the
more detailed design elements of the project.
5. For either an SR or an ER, a preliminary project design will be developed to a ten percent to
thirty percent complete level in order to evaluate the project elements to a reasonable level
of accuracy The OWNER will direct the ENGINEER on the level of reporting required
and the percent complete of design that is appropriate for the specific project. r�

REIMBURSABLE EXPENSES
Reimbursable expenses include direct expenses incurred by American Structurepoint, Inc., or our
consultants in the performance of work which is directly related to the project. These expenses are in
addition to compensation for Basic and Supplemental services. Reimbursable expenses will be invoiced
at 1.0 times our direct costs. These expenses include, but arc not limited to, the following:
• Renderings, models, or colored elevations
• Governmental agency review or permit fees
• Reproduction of documents for governmental agency review, bidding, or construction
• Reimbursable expenses charged to us by subconsultants
• Airline tickets, car rental, mileage, and per diem expenses for out-of-town travel
• Couriers and overnight deliveries, including FedEx, UPS, or similar carriers
The following expenses, if incurred in the process of providing professional services included in basic
services, are included in the fee noted and are not considered reimbursable expenses:
• Printing for in-house purposes and progress meetings
• Plotting expenses
• Computer charges
• Long distance phone calls
• Faxes
• Postage and handling
