HomeMy WebLinkAboutRe Carmel Senior Living July 7 2015.msgHello Nick,
I just spoke with Jim Blanchard and he requested that I reach out to you to schedule a "final" inspection for tomorrow morning at our Carmel Senior Living facility located at 13390 N.
Illinois Street, permit #1310019. The purpose of this inspection is to allow our sales staff into the building as well as the general public visitors (to meet with our sales staff,
NO residents) on the 1st floor.
We also discussed that there will be three (3) items incomplete from the Phase I walk-through:
1. Exit signs with Braille. These were ordered on July 2nd and will arrive onsite and be installed next Friday, July 17th.
2. Landscaping. We have already tagged all of our landscape material to be installed onsite, we just cannot get it installed due to all the rain. We have our landscaping team on notice
so once weather breaks, they will jump on the remaining landscaping.
3. Increase Elevator Cab Size. I am working on an action plan that the fire department is aware of to have a custom elevator cab fabricated to fit our elevator shaft. I should have
a complete action plan by the end of this week to submit to the elevator company regarding the custom cab installation. Please note this only affects one of the two functioning elevators
we have onsite.
Please contact Robert Terral, our project superintendent, when you get onsite for the inspection. His phone number is: 720-301-0481. He is also cc'd on this email.
Please let me know if you have any questions or concerns.
Thank you,
Bernida Egging Chapman

Hi Bernida-
I just got your message. You will need to get approval from Jim Blanchard the Building Commissioner for this. I have copied Jim on this email. Also, his phone number is 317-571-2450.
There is already a temporary C/O for furniture and fixtures, so this would be adding the guests/workers on top of that.
Thanks,

Hello Nick,
I just left you a (unfortunately long) voicemail regarding our Carmel Senior Living facility.
Below Bruce Knott has recommended approval to allow our staff into the building as well as public visits. He mentions that this approval needs to be confirmed by the Building Commissioner.
Can you please confirm with me the process for getting the Building Commissioner's approval on this request?
We are hoping to move our staff into the building to allow us to pull our sales trailer and begin landscaping at that area.
Thank you in advance for your response. I appreciate it.

Based on today’s meeting with Bernida Chapman and our follow up inspection with Robert Terral, the following allowances are approved by Carmel Fire but still require approval from
D.OC.S. (building commissioner).
At your request, staff members can start working in the facility.
At your request visitations from civilians can start.
Based on the current elevator problem, we are not approving resident occupancy at this time.
NOTE: If staff and civilian visitation is allowed by D.O.C.S. , then this facility is no longer a construction project, but now becomes a working facility with some construction work
being done. All emergency systems i.e. Alarm, sprinkler, etc. must remain operational. All exits must remain unobstructed including the work being done on the floor near the formal
staircase.
Any hot work will require notification and approval from the fire department. Any deviation of the emergency systems need to communicated to Fire Marshal’s office of the Carmel Fire
dept.
Please provide the following information for our pre-plan information
After hours contact information
Maintenance #
Secondary contact name and #
Facility main phone #

Subject: Carmel: Formal response to Fire Department Inspection on 6.30.15
Dear Mr. Knott,
Sorry for the delay in responding to the open items from the Fire Department Inspection on 6/30/15, please see the attached for our formal response.
There are only Two (2) items that remain open, everything else was completed by 7/2/15. I believe you’ll be able to review our completed items on the re-inspection, however we have
photo documented the repairs and we can make these available if needed.
Thank you for the support your department has given us during this project.
