THIS. AMENDMENT TO THE AGREEMENT FOR PROFESSIONAL SERVICES ("Agreement") entered into
by and between the City :of Carmel and United Consulting (the "Professional"), as City Contract dated June 1, 2016
shall amend .the terms of the Agreement .by adding the additional services to be provided. by, Professional consistent
with the Scope of Work attached hereto and incorporated herein as Exhibit "A". Furthermore, the terms of the
Agreement shall be amended to include the E -verify requirement as stated in. Indiana Code § 22-5-1.70t seq., which
is attached hereto and incorporated herein as Exhibit "B", as well as the required E -verify. Affidavit, attached hereto
and incorporated herein as Exhibit "C".Also, pursuant to I.C. § 5-22-16.5, Professional shall=certify that in signing this
document, it does not engage in investment activities within the Country of :Iran. The terms and conditions of the
Agreement shall not otherwise be affected by this Additional Services Amendment and shall remain in full force and
effect.

RANGELINE ROAD. RECONSTRUCTION
From 136th Street (Smokey Row) to U. S. 31
Carmel, Indiana
Description of Proposed Project
1. GENERAL
The proposed project includes constructing a boulevard section with new sidewalk and multi -use path
for 0.43 miles of Rangeline Road, which will begin at the north approach of the roundabout at 136th
Street and extend north to a proposed roundabout just south of US 31. The proposed roundabout at
the north end of the project will connect Rangeline Road with the Lowes Way Connection project
proposed by Hamilton County. Additionally, the existing curb lines at the 136th Street roundabout will
be modified to eliminate the damaging effects of trailer off tracking. This project will include
modifications to Hamilton County Bridge No. 194 over Little Cool Creek, which was replaced by the City
of Carmel in the Fall of 2008. The City of Carmel will fund this project with 100% local dollars; with an
anticipated bid date in April of 2017 and construction completion in November of 2017. This project is
located in Sections 19 and 24 of Township 18 North, Ranges 3 and 4 East, Clay Township, in the City
of Carmel.
H. EXISTING FACILITY
Rangeline Road currently consists of a two-lane section. from 136th Street to the U.S. 31 overpass,
with a 600 -foot section at the north end accommodating two northbound lanes. The existing
pavement consists of a 24 -foot wide asphalt overlay over concrete pavement. A 4 -foot shoulder exists
at various locations throughout the project length, and acceleration and deceleration lanes exist at
most of the businesses and developments throughout this stretch of roadway. The existing pavement
along the corridor has deteriorated due to large traffic .volumes and water infiltration. Sidewalks exist
on the east side between the 136th Street roundabout and Little Cool Creek and adjacent to an
apartment complex at the north end of the project. Storm water runoff is drained adjacent to the
roadway by either shallow ditches or swales or by enclosed pipe ditches that flow into Little Cool
Creek. The Hagen -Burke Trail crosses Rangeline Road at grade, just north of the Little Cool Creek
Bridge. The posted speed limit is 30 mph.
III. TOPOGRAPHIC SURVEY
Survey History
The majority of the proposed project was previously surveyed by United Consulting in 2006.
However, several construction projects and. changes to the topography have taken place over the last
decade. These changes include but are not limited to:
1. The construction of the roundabout at Rangeline Road and 136th Street.
2. Razing of structures and grading of the southwest and northwest quadrants of Rangeline Road
and 136th Street.

3. Grading, storm sewer and sidewalk construction along the east side of Rangeline Road from
136th Street to Little Cool Creek.
4. Construction of the bridge and approaches carrying Rangeline Road over Little Cool Creek.
5. Channel work within Little Cool Creek, east of Rangeline Road, including removing of the
arched structure for the former drive to the City of Carmel property.
6. Razing of the structure and grading immediately north of Little Cool Creek on the east side of
Rangeline Road.
7. Grading and new topography along the Rangeline Road frontage of Butler Collision Center and
Dairy Queen.
8. New and relocated underground utilities for the entire project length.
9. Completion of the US31 at the north end of the project limits, including the razing of the former
Speedway station.
In our experience with urban projects such as this, having an accurate and up to date survey is critical
for design and avoiding costly change orders during construction due to unknown changes in the
existing cross section. Therefore, we believe it is in the best interest of the City of Carmel to have a
new topographic survey performed at this time.
Proposed Survev
The topographic survey will follow all the requirements of the "Indiana Department of transportation
Design Manual, Part III — Location Surveys (June 15, 1998), and all the requirements of Title 865 IAC
1-12 et sequential. The limits of the survey will be as follows:
1. Rangeline Road: Beginning 150 feet south of the center of the roundabout at 136th Street and
extending 2,750 feet north to the north end of the southbound US31 bridge over Rangeline
Road. The reference alignment will be established on the existing centerline. The coverage
width will be a minimum of 75 feet left and right of the reference alignment, or 25 feet beyond
any top of bank or toe of slope.
2. 136th Street: Beginning 125 feet west of the center of the roundabout at Rangeline Road and
extending east 250 feet to 125 feet east of the center of the roundabout. The coverage width will
be a minimum of 75 feet left and right of the centerline of 136th Street, or 25 feet beyond any top
of bank or toe of slope.
3. Little Cool Creek: Beginning 300 feet upstream from the upstream face of the Rangeline Road
bridge over Little Cool Creek and extending 675 feet downstream to 300 feet downstream of the
downstream face of the Rangeline Road bridge. The coverage width will be 50 feet beyond the
banks of Little Cool Creek.
4. Frontage Road at The Range Pet Lodge: Beginning at the centerline of Rangeline Road and
extending northeast 450 feet. The coverage width will be a'minimum of 100 feet left and right of
the centerline of the Frontage Road, or 25 feet beyond any top of bank or toe of slope.

5. Additional 350 ft x 350 ft area at the southwest quadrant of the intersection of Rangeline Road
and 136' Street (as shown on the attached exhibit).
6. Additional 200 ft x 750 ft area of the former Ritter's lot on the west side of Rangeline Road (as
shown on the attached exhibit).
7. Sufficient information will be gathered by use of a data collector to provide complete topography
and equivalent cross-sections at 50 foot intervals along all referenced alignments.

1V. GEOTECHNICAL EVALUATION
A Geotechnical Evaluation will be required to evaluate the subsurface conditions and to provide the
necessary information for pavement design, storm sewer construction, and earthwork considerations.
This evaluation will include soil borings and a formal geotechnical report. As part of the 2006
roundabout project, soil borings were acquired to a point approximately 550 feet north of 136th Street.
V. ENVIRONMENTAL
1. Environmental Documentation:
A NEPA Document will not be required for this project because it is funded without the use of
federal funds.
2. Phase I Environmental Assessment
The former gas station at the northern end of the project has been demolished. The site was
acquired as part of the US 31 reconstructions and it is expected that the documentation of
existing conditions at the site can be obtained from the Indiana Department of Transportation.
Therefore no Environmental Assessment will be included.
3. Historic Properties
The Hamilton County Interim Report (pages 109-110) shows only one historical site within the
project limits. It is the "Carmel Cemetery" in the northeast quadrant of Rangeline Road and
136th Street. It dates back to circa 1833 and is listed as a "contributing" historic site, as an
example of Exploration/Settlement and Religion. The design of this project will not disturb this
potentially historic resource, or acquire any right-of-way from this parcel.
4. Wetland and Stream Impacts:
The National Wetlands Inventory Map shows no wetlands or streams within the project limits.

5. Regulatory Permits:
An IDEM Rule 5 permit will be required since the project will be disturbing greater than 1 acre
of land. The project will also include the following permits: IDEM Section 401 Water Quality
Certification, US Army Corps of Engineers Section 404 Permit, and a Construction in
Floodway Permit issued by DNR. A Hamilton County Outlet permit will not be required as
Little Cool Creek is not a regulated drain in this location.
6. Cemetery Management Plan
Two Cemetery Management Plan will be required for the work being done within 100 -feet of
the Carmel Cemetery and Old Carmel Cemetery.
Vl. HYDRAULICS
It is anticipated that the existing profile grade of Rangeline Road will need to be raised in order to
construct the proposed roadway cross section and the current published FEMA Flood Insurance
Study shows a water surface profile for Little Cool Creek that may overtop Rangeline Road during a
Q100 flood event. Jon Clodfelter met with Dave Knipe from the IDNR on April 1, 2016 to discuss the
currently published modeling and what level of effort would be required to meet IDNR requirements
for a Construction in a Floodway permit (see Meeting Minutes).
Maintaining the existing bridge over Little Cool Creek and raising the profile grade of Range Line
Road will require a detailed hydraulic model for the IDNR Construction in a Floodway permit
necessary for the construction of the proposed project. The detailed hydraulic model for Little Cool
Creek will confirm if there is any roadway overtopping for the existing bridge and if the proposed
project can receive a permit within IDNR backwater requirements. If the backwater requirements can
be met with the existing structure, the hydraulic modeling will be finalized and submitted to IDNR with
the Construction in a Floodway permit application. If the backwater requirements can't be met with
the project as proposed, additional hydraulic analysis and project scope review will be required which
is beyond the scope of this proposal.
It is anticipated the proposed hydraulic modeling will have a total study reach of approximately 1400
feet extending from the most downstream cross section from the FIS modeling to just downstream of
the structure under the Monon Trail. The starting water surface elevation for the modeling will not
include backwater from Cool Creek and the Q100 discharge will be 1530 cfs per the current published
models. The hydraulic modeling will include models for Duplicate Effective, Corrected Effective,
Existing Pre -Project and Proposed Project models in or order to comply with IDNR backwater
requirements.
Vll. UTILITIES
Overhead power lines currently run along both sides of Rangeline Road. A Duke substation is located
south of the Little Cool Creek Bridge on the west side of the roadway. High voltage lines (69kv) run
from the substation along the western side of Rangeline Road, and then northeastward along Little
Cool Creek. There is an existing pole in the southwest quadrant of the bridge crossing Little Cool
Creek. During utility coordination for Hamilton County Bridge No. 194, Duke Energy placed a $1
million value on its relocation. There are also buried AT&T fiber lines running along the eastern side of
Rangeline Road at the edge of the turning lanes. It is anticipated that relocation of portions of these
lines as well as buried utilities (water, sewer, gas, cable, etc.) will be required to accommodate the
construction of this project. Utility coordination will follow an accelerated version of the INDOT
procedure.

Post -bid utility coordination has not been included in this scope of work. It is anticipated these
services will be provided as part of construction inspection.
Vlll. PROPOSED FACILITY
Design Histo
As part of the original 2006 contract, United was directed to develop design plans to 60%.complete,
based on a project scope that would make the best use of the, budget limits for the corridor. However,
during the design phase, the scope was refined several times and ultimately only 20% plans were
ever developed. Unfortunately, based on several topographic changes, as discussed in the survey
section, and a revised typical section based on the proposed project, the work completed in 2006 has
little to no value for this project.
The subcompact roundabout design criteria for Rangeline Road and 1361 Street was based on the
desire of the City to limit truck access in the area. This design criteria ultimately dictated the inscribed
diameter of the circulatory roadway. Regrettably, the limited size of the roundabout has not deterred
trucks as desired and has led to significant off tracking problems in the northeast and southeast
quadrants of the roundabout which will be mitigated as part of the proposed project.
Proposed Design
This project will be, designed using the INDOT design criteria for a minor arterial as well as NCHRP
Report 672 — Roundabouts: An Informational Guide, Second Edition.
To the extent possible the proposed profile will be raised to achieve 100 year serviceability for
Rangeline Road, while maintaining the existing bridge. The typical cross section for the project will
consist of two 14 -foot travel lanes separated by an 11 -foot median (see Typical Sections). Curb and
gutter will be utilized to establish both the exterior curb lines as well as the center median. Proposed
median opening locations are as described on the plan view exhibit. A sidewalk measuring 5 -feet in
width will be provided behind a 5 -foot grass buffer strip adjacent to the curb on the east side of
Rangeline Road. An 8 -foot wide multi -use path will be placed behind a 5 -foot grass buffer on the west
side of Rangeline Road. Ornate parapet walls will be constructed separating the sidewalk and multi-
use path in front of both cemeteries (see Scope of Work Exhibit).
The existing flasher at the Hagen -Burke Trail will be removed and replaced with ground mounted
pedestrian signs and in -ground, pedestrian activated flashing lights, similar to what is in operation at
the Clay Terrace shopping center. The existing retaining wall along the north side of the Hagen -Burke
Trail, on the east side of Rangeline Road will turn 90 degrees and be extended approximately 50 feet
north (see Scope of Work Exhibit).
The surface of the bridge over Little Cool Creek will be modified to match the proposed roadway cross
section (see Typical Sections). The west sidewalk will be widened by 9 feet and an 8 foot concrete
median will be added across the bridge. It is anticipated the existing bridge railing can be maintained
by removing the sidewalk concrete to sufficient depth to expose the top mat of steel in the sidewalk.
The sidewalk extension will be carried over the top of the existing sidewalk where the existing
sidewalk elevation at the railing will be maintained. Finally the bridge will be load rated based on the
proposed modifications.

Roadway runoff will be contained by curb and gutters combined with a storm sewer drainage system.
A storm water quality treatment unit will be provided at the end of the system prior to discharging into
Little Cool Creek as required by the cities Stormwater Technical Standards Manual.
The City has identified a portion of Rangeline Road, just south of the 136th Street roundabout, where
storm water flows from the east to west across the roadway and ponds in back of the parcels in the
southwest quadrant. United will evaluate the contributing drainage, provide a recommendation and
incorporate the recommendation into the plans.
A single lane roundabout will be constructed at the north end of the project. The north and west
approaches will have multi -lane entrances which will tie directly into the existing four lane section at
the US 31 bridge. The curb lines for the roundabout at 136th Street and Rangeline will be modified to
eliminate off tracking from truck trailers.
IX. MAINTENANCE OF TRAFFIC
Due to the proposed lane widths in conjunction with the raised grass median, it will be very
challenging to maintain two-way traffic and provide an adequate work zone throughout construction.
In order to maintain access to all of the businesses throughout the corridor and provide a safe and
efficient work zone, we propose maintaining only northbound traffic along Rangeline Road throughout
Phase I of construction. Traffic will shift from one side of Rangeline Road to the other as construction
of progresses. The detour route for southbound traffic will utilize 1461 Street, US 31, and 136th Street
(see Detour Route Map). The Hagen -Burke Trail crossing will be maintained at Rangeline Road
throughout construction.
Following construction of the boulevard section, the north end of Rangeline Road will be closed to
traffic to construct the proposed roundabout. Temporary access will be maintained to Public Storage
and The Range Pet Lodge. The same detour route established for Phase I, will be utilized for this
phase of construction.
X. LIGHTING
Lighting will be included with this project and is anticipated to match the style and spacing established
along Rangeline Road between Main Street and 136th Street.
Xl. LANDSCAPING
Landscaping is not included in this scope, but can be integrated into this project if the City desires.
XII. LAND ACQUISITION
Land acquisition will be funded using 100% local funds. United Consulting will complete the following
services associated with land acquisition: title research, right-of-way engineering and right-of-way
staking as needed. The City of Carmel, through the use of another consultant, will -complete initial
appraisals, review appraisal, and negotiations and buying with property owners.
It is estimated there will be 14 affected parcels within the project limits. Three of which were
previously impacted by the 2006 roundabout project (see Land Acquisition Map). All but one of the
impacted parcels are commercial.

All existing right-of-way will be verified during right-of-way engineering. If recorded documents do not
exist, or differ from the apparent right-of-way, it may be necessary to re -acquire existing right-of-way,
which could increase the number of parcels.
Xlll. PROJECT SCHEDULE
The project schedule is based on a Notice to Proceed (NTP) of April 25, 2016. Survey and utility
coordination beginning immediately following NTP. The bid date is anticipated in April 2017.
XIV. PROJECT BUDGET
A detailed Project Development Cost Estimate has been included herein to highlight the breakdown of
individual design costs and all construction activities. The construction cost was developed based on
current cost information and inflated for construction in 2017 at 3% per year.

EXHIBIT `B"
E -verify requirement
All terms defined in I.C. § 22-5-1.7 et seq. are adopted and incorporated into this section of the Amendment.
Pursuant to I.C. § 22-5-1.7 et seq., Vendor shall enroll in and verify the work eligibility status of all of its newly -hired
employees using the E -Verify program, if it has not already done so as of the date of this Addendum. Vendor is further
required to execute the attached Affidavit, herein referred to as "Exhibit C", which is an Affidavit affirming that: (i) Vendor is
enrolled and is participating in the E -verify program, and (ii) Vendor does not knowingly employ any unauthorized aliens. This
Addendum incorporates by reference, and in its entirety, attached "Exhibit C." In support of the Affidavit, Vendor shall
provide the City with documentation that it has enrolled and is participating in the E -Verify program. This Agreement
shall not take effect until said Affidavit is signed by Vendor and delivered to the City's authorized representative.
Should Vendor subcontract for the performance of any work under this Addendum, the Vendor shall require any
subcontractor(s) to certify by affidavit that: (i) the subcontractor does not knowingly employ or contract with any unauthorized
aliens, and (ii) the subcontractor has enrolled and is participating in the E -verify program. Vendor shall maintain a copy of such
certification for the duration of the term of any subcontract. Vendor shall also deliver a copy of the certification to the City
within seven (7) days of the effective date of the subcontract.
If Vendor, or any subcontractor of. Vendor, knowingly employs or contracts with any unauthorized aliens, or retains
an employee or contract with a person that the Vendor or subcontractor subsequently learns is an unauthorized alien, Vendor
shall terminate the employment of or contract with the unauthorized alien within thirty (30) days ("Cure Period"). Should the
Vendor or any subcontractor of Vendor fail to cure within the Cure Period, the City has the right to terminate this Agreement
without consequence.
The E -Verify requirements of this Agreement will not apply, should the E -Verify program cease to exist.

As we discussed, the City of Carmel has a need for additional professional services and pursuant to our current
contract with United, Contract #060116.01, dated June 1, 2016, I have requested and received from you the
proposed scope of services and fee estimates for the following work task:

Professional Engineering Services
Not to Exceed: $499,500.00
Per our contract procedural requirements, I hereby approve these Additional Professional Services estimates, and
request that the Board of Public Works ratify the above referenced Contract Amendment.
