Dear Mr. Jenkins:
Re: Completion of Water Works Operator
Certification Operator In Training
This is to advise that you have satisfactorily completed the requirements for
certification as a Grade WT5 Water Works Operator as outlined in 327 IAC 8-12-
3.2(a)(2)(A)(B). This is based upon the documentation maintained by you and
verified by Mr. Jerry A. Cloud.
Based on the approved duty logs you have submitted documenting your hands-
on training in the operation of a Class WT5 water treatment plant, and by obtaining
the required continuing education hours, you have met the qualifications for
certification as a WT5 operator in the State of Indiana. The final duty logs, however,
were not received in this office prior to the expiration date of June 30, 2016. You are
currently in a one-year grace period which ends June 30, 2017, and the WT5
certification is not valid.
Your WT5 certification cannot be activated and placed in a current renewal
period until a$30 renewal fee is received by this office. The fee can be paid by check
or money order in the amount of$30 and payable to Indiana Department ofWq,�
Environmental Management.
Once the fee has been received and processed, your certificate of competency
indicating the class water works you are certified to operate along with a wallet card
will be sent to you under separate cover. The certificate has been designed to fit a
conventional 5x7 frame. The renewal card will have an expiration date of June 30,
2019.

You will need to acquire 30 hours of approved continuing education by June 30,
2019. Rule 327 IAC 8-12-7.5(c)(1) (enclosed) states that 70% of the required
continuing education contact hours shall be obtained from a technical category. If
