ADDITIONAL SERVICES AMENDMENT TO
AGREEMENT FOR PROFESSIONAL SERVICES
THIS AMENDMENT TO THE AGREEMENT FOR PROFESSIONAL SERVICES ("Agreement') entered into
by and between the City of Carmel and United Consulting Engineers, Inc. (the "Professional'), as City Contract dated
June 1, 2016 shall amend the terms of the Agreement by adding the additional services to be provided by
Professional consistent with the Scope of Work attached hereto and incorporated herein as Exhibit "A". The terms
and conditions of the Agreement shall not otherwise be affected by this Additional Services Amendment and shall
remain in full force and effect,

SUR VEY—SCOPE OF SERVICES
GENERAL REQUIREMENTS: The survey shall be collected in ENGLISH FORMAT and shall contain
the following minimum information:
Original survey field book containing the following minimum information:
a. Index.
b. General site information.
c. Complete listing of utilities, including addresses, with facilities in the area.
d. Horizontal control points with references.
e. _Level datum notes with vertical temporary benchmarks.
f. Storm and sanitary structure details.
2. Electronic data and drawing files suitable for use on a WINDOWS based computer system using the
most current version of Autocad including the following:
a. Points file of entire survey with hard copy output.
i. (single spaced, ASCII format, with data separated by spaces)
ii. point number
iii. northing
iv. easting
v.. elevation
vi. description
b. A point description index/library shall be provided and a current copy maintained on file
with UNITED.
c. Topographic drawing file including:
i. (2-D drawing, Autocad format)
ii. property lines, existing right-of-way, easements and property owners' names.
iii. Above ground utilities and buried utilities marked by Indian Underground Plant
Protection Services.
iv. topography shown with appropriate symbolic representation.
d. Break -line drawing file indicating slope change information.
i. (3D T.I.N. in XML format)
ii. Contours at 1 foot intervals
PROJECT LIMITS AND COVERAGE OF SURVEY:
1. City Center Drive: Beginning at Clark Street and extending 775 feet northeast to Carmel Garden. The
coverage width shall from the southeast curb of City Center Drive to 10 feet beyond the existing
fence.
2. Clark Street: Beginning at City Center Drive and extending 325 feet southeast. The coverage width
shall be from the northeast curb of Clark Street to 10 feet beyond the existing fence.,
3. Sufficient information will be gathered by use of a data collector to provide complete topography and
equivalent cross-sections at 50 foot intervals along all referenced alignments.
