AGREEMENT FOR PROFESSIONAL SERVICES
THIS AMENDMENT TO THE AGREEMENT FOR PROFESSIONAL SERVICES ("Agreement") entered into .
by and between the City of Carmel and VS Engineering, Inc. (the "Professional'), as City Contract dated June 1,
2616 shall amend the terms of the Agreement by adding the additional services to be provided by Professional
consistent with the Scope of Work attached hereto and incorporated herein as Exhibit "A11. The terms and conditions
of the Agreement shall not.otherwise be affected by this Additional Services Amendment and shall remain in full force
and effect.

SCOPE OF SERVICES
I. PROJECT DESCRIPTION
Project Name: Jordan Woods Stormwater Improvements
Owner: City of Carmel, Indiana
Engineer: VS Engineering, Inc.
A. Project Location
The City of Carmel proposes to construct storm drainage improvements within the Jordan Woods
Neighborhood and the Woodland Country Club golf course, which is located north of 106th St. and
west of Keystone Pkwy. The proposed project is located within the City of Carmel and generally
includes Jordan Rd., Timber Ln., Hamilton Ln., 109th St.,.and 110th St. Detention facilities are
proposed to be installed on the golf course, just.north of the neighborhood. The project limits are
shown in Attachment No. 1.
B. Existing Conditions
The Jordan Woods Neighborhood experiences flooding several times during a typical year. This is
mainly due to the lack of any defined roadside drainage and is further exacerbated by concentrated
flows discharged from the golf course. The existing open channel through the neighborhood allows
for direct conveyance of these flows; however, several culverts on this channel have deteriorated and
several appear to be undersized to convey this flow.
C. Proposed Improvements
The Engineer shall design detention facilities, hybrid ditch systems, traditional storm sewer systems,
roadside ditches, and culverts to provide adequate drainage during storm events. These systems shall
connect to the existing 60" storm sewer at 106th St. and Keystone Pkwy. The hybrid ditching, storm
sewer, and roadside ditches limits shall be determined during design and will vary depending on site
conditions. A wet detention pond facility, with a storage capacity of approximately 2.7 acre -ft, will
be installed on the golf course to reduce concentrated peak flows discharging into the neighborhood.
Success shall be measured by significantly reducing the occurrence of localized flooding due to
precipitation events with the neighborhood. —
II. GENERAL REQUIREMENTS
A. Applications and Approvals - ENGINEER shall prepare and submit all permit applications
and obtain on behalf of OWNER all approvals or consents from authorities having
jurisdiction or agencies having facilities within the limits of the project. The ENGINEER
shall prepare a list of permits or approvals and indicate their applicability to this project.
B. Registered Engineer — ENGINEER shall prepare bid documents for drainage improvements,
including plans and contract documents, plus the final opinion of probable construction cost
by quantity and unit price which are stamped by a Professional Engineer registered in the
State of Indiana.
C. Design Requirements — All work by the ENGINEER shall be in accordance with, but not

limited to the current editions and the requirements of the following:
1. City of Carmel Stormwater Technical Standards Manual
2. City of Carmel Standard Drawings
3. Indiana Department of Environmental Management (IDEM)
4. U.S. Army Corps of Engineers (USACE)
5. Indiana Department of Natural Resources (IDNR)
6. Indiana Department of Transportation (INDOT) Standard Specifications
7. Other pertinent regulatory design standards
D. Source of Data — ENGINEER shall identify in all plans and specifications data which is
measured and data which is assumed.
III. COORDINATION AND QUALITY CONTROL
A. Project Meetings — ENGINEER shall attend a total of eight (8) project meetings, including
one (1) 50% Design Review Meeting, one (1) 90% Design Review Meeting, one (1) 100%
Design Review Meeting, three (3) progress meetings, one (1) utility coordination meeting,
and one (1) Woodland Country Club golf course coordination meeting. Coordination with
the Woodland Country Club can also occur during other project meetings.
IV. DATA COLLECTION AND ANALYSIS
A. Data collection and analysis shall .include gathering and analyzing maps, reports and data
including but not limited to the following:
1. Review existing GIS data available within the PROJECT area.
2. Collect existing topographic, utility, floodplain, soil, wetland, and other applicable
maps for PROJECT area.
3. Collect existing hydrologic and hydraulic information including, but not limited to,
stage discharge relationships, hydraulic models and flood plain studies.
4. Evaluate all collected data and identify required survey for the project area.
B. ENGINEER shall plan, coordinate, monitor and document all data collection and analysis
activities.
V. FIELD SURVEY
ENGINEER shall provide field surveying services for the above referenced project. ENGINEER
shall furnish all labor, materials, and equipment to perform the surveying services described below
for the fee identified in Attachment No. 3.
A. BASIC SERVICES
1. ENGINEER shall survey the project location within the defined project limits to
satisfactorily complete the basic field survey services described herein. ENGINEER shall
also provide one set of original field notes, all field survey data collected via electronic
media, and one set of master drawings. Work shall be in accordance with Indiana Code
(I.C. 25-21.5); and Indiana Administrative Code (865 I.A.C. 1-12)
2. This Survey Scope does not include the preparation of a Location Control Route
Survey (LCRS). In the event that right-of-way or easements will be purchased to
sufficiently complete the project, a LCRS may be required to fully conform to 865
I.A.C. 1-12.

3. Electronic files including the following shall be prepared and submitted by ENGINEER
as directed by OWNER:
1. Finished plan view of topographic survey in AutoCAD .dwg or .dxf format
2. 1 -foot contours in AutoCAD .dwg format
3. TIN used to create contours in .xml format
4: Electronic points files in .txt coordinate or .xml formats
5. Survey Book in .pdf format
4. ENGINEER shall complete the field survey as summarized below and as directed by
OWNER. The project area to be field surveyed is shown in Attachment No. 1 and described
as follows :
a. Jordan Woods Subdivision Survey —12,660 feet
Jordan Road
Beginning at the centerline of 106'h Street, survey northerly along Jordan Road
2,825 feet to 111' Street.
Jordan Court
Beginning at the centerline of Jordan Road, survey southeasterly along Jordan Court
300 feet to the cul-de-sac at the end of Jordan Court.
1111" Street
Beginning at the centerline of Timber Lane, survey east along 111"' Street 285 feet.
Timber Lane
Beginning at the centerline of Jordan Road, survey north along Timber lane 2,100
feet to 1110' Street.
1101h Street
Beginning at the centerline of Jordan Road, survey west along 110"' Street 2,850 feet
to a point 200 feet west of the centerline of 109'h Street.
Hamilton Lane
Beginning at the centerline of Timber Lane, survey west along Hamilton Lane 1,750
feet to the centerline of 109"' Street.
109`h Street
Beginning at the centerline of 110"' Street, survey south and east along 109"' Street
2,100 feet to the cul-de-sac at the end of 109t" Street.
Stormwater Tie-in Locations
Two locations will be surveyed off of the mainline road limits described above. The
approximate locations are as follows; approximately 150 feet south of the
intersection of 111'll Street and Jordan Road, survey east 150 feet to the west edge of
pavement of the southbound lane of Keystone Parkway, and approximately 225 feet
west of the intersection of Hamilton Lane and Timber lane, survey north 300 feet.
The total lineal feet of these two locations amounts to 450 feet.

Note: The width of survey will be 45 feet on each side of the centerline of the roads,
except in the two stormwater tie-in locations which shall be 30 feet on each side of
the apparent property line.
b. Jordan Woods Ditch Survey — 4,215 feet Mainline Ditch
Beginning at a point 125 feet north of and 65 feet west of the centerline of the
wishbone round -a -bout of 106`x' Street with the centerline of Keystone Parkway,
begin survey at the east edge of pavement of the southbound off -ramp of Keystone
Parkway and head northwesterly following the meanderings of the creek through
Jordan Woods 3,800 feet to the south property line of the golf course that is north of
Jordan Woods.
Tributary Ditch
Beginning at a point 215 feet west of and 85 feet south of the intersection of
Hamilton Lane and Timber Lane, survey south 415 feet.
Note: The width of survey will be 10 feet beyond each side of the top of bank of the
ditch and above described centerline of the tributary.
C. Golf Course Pond Survey — Approximately 4 acres
Approximately 4 acres of golf course property will be surveyed for the design of
detention pond. See Attached No. 1 for the approximately location of the pond.
d. Total Survey includes approximately 16,875 feet by varying width of roadway
and ditches, and approximately 4 acres for a detention pond located on golf
course property.
5. Obtain last deed of record, subdivision plats, and section or auditor plats for all properties
within the project limits from local and state agencies. The property information shall
include property owner's name, instrument number, and date of recording.
6. Establish Primary Horizontal Control within the project limits such that the survey base
line(s) can be re-established during construction. Horizontal Datum will be the East Zone of
the Indiana State Plane Coordinate System, NAD83 (2011).
7. Establish on-site elevation using NGS, DNR or Hamilton County benchmarks or GPS
observations. Set temporary bench marks within the project limits such that elevation datum
can be re-established during construction.
Tie in the survey base lines to available USPLSS section corners and/or existing
property/right-of-way monumentation. All necessary section corners will be located or re-
established to adequately define property lines along the limits of the project.
Plot right-of-way and property lines based on observed physical evidence and record
documents acquired from local government agencies.
10. Coordinate with all utility companies to locate and mark their utilities in field. ENGINEER
shall notify the utilities via the call before you dig notification system (Indiana Underground
Plant Protection Service (I.U.P.P.S.). ENGINEER shall verify that each utility has field

located their facilities during the course of the design survey. The existing facilities located,
at the time of the field survey, shall be incorporated into the design survey. In addition,
ENGINEER shall provide listing of all utilities and all information available for that utility
including address, and telephone number.
11. Perform design survey in sufficient detail to obtain topographic data, buildings, walls, walks,
signs, vaults, and natural and man-made features, as evidenced by facilities at the ground
surface and marks by others, necessary for the development of project plans.
12. Take cross sections at specified intervals across the right-of-way of public roadways and or
the project limits (as described above) whichever is further out. Additional cross sections
shall be taken at intersection of streets, roads, railroads, driveways, etc. Obtain elevations of
all existing structures such as drainage culverts, utilities and other structures.
13. Indicate spot elevations at all, break in grade, ramps, area ways, tree grates, etc. within the
project limits, and at top and bottom of curb.
14. Provide a listing of all symbols, notations and legends used in the field survey. ENGINEER
shall furnish a hard copy together with all field survey information collected on electronic
media. ENGINEER shall also prepare master drawings (1 -foot contours) from data collected
in topographic survey using AutoCAD Civil 3D and shall submit a hard copy together with
electronic format. ENGINEER shall delineate and label the location of all buildings,
structures, fences, railings, signs, walls, walks, paved areas, curbs and other permanent
structures and existing improvements. ENGINEER shall outline all building edges, insets
and projections, and below grade structures such as vaults, basements, and areaways where
applicable, as evidenced by facilities at the ground surface and marks by others.
15. Prepare easement descriptions and plats for land acquisition on a per parcel basis.
Note: Total number of easements to be increased or decreased based on design needs.
VI. PRELIMINARY DESIGN
A. ENGINEER shall plan, coordinate, monitor and document preliminary design activities.
B. ENGINEER shall be responsible for completing preliminary design services for the
PROJECT as defined by the following items:
1. Drainage improvements will be designed to a point where 50% Design Review plans
can be developed and coordination with appropriate regulatory agencies can be
completed before submitting for applicable permits.
2. Preliminary hydrologic and hydraulic calculations.
3. Coordination of the PROJECT with OWNER, State of Indiana government agencies,
and all other local governmental and non-governmental agencies.
4. ENGINEER shall submit two (2) half-size sets of 50% Design Review plans to
OWNER. PDF copies shall also be uploaded to the American Structurepoint
ShareFile site.
5. Submit estimates of probable construction costs.
C. ENGINEER shall complete site walk-thru visit with OWNER to determine critical areas for
construction. Document existing conditions with colored photographs.

D. Review of project documents shall be completed by OWNER or OWNER's representative.
Comments shall be generated and resolved accordingly between the parties at -each submittal
stage.
VH. DETAILED DESIGN
A. ENGINEER shall plan, coordinate, monitor and document detailed design activities.
B. ENGINEER shall plan and coordinate one (1) Public Meeting, the location of which shall be
established by OWNER. ENGINEER shall complete a summary of the meeting and submit
to OWNER for review and comments. ENGINEER shall complete adjustments to the design
according to comments which are approved by OWNER.
C. In accordance with the project's preliminary design task, the ENGINEER shall perform a
detailed design of the proposed PROJECT. As part of the detailed design phase, the
ENGINEER shall complete the plans to a point where a 90% Design Review may be held
and applicable permits can be applied for. Once all permits are secured and all comments
from the 90% Design Review meeting are addressed, final plans shall be completed. The
following items, at a minimum, shall be included in the final plans:

Contract Documents
1. City of Carmel Technical Specifications shall be the basis of design and documents.
ENGINEER shall provide to the OWNER any supplemental technical specifications
necessary to cover all design elements as well as identify project quantities and Pay
Items to be used.
D. ENGINEER shall complete the necessary permitting to construct the project.
E. ENGINEER shall be responsible for all hydrologic and hydraulic calculations required to
complete all applicable permits from regulatory agencies.

F. ENGINEER shall provide the OWNER with the estimates of probable construction costs of
the PROJECT. ENGINEER shall advise OWNER of any adjustments to the probable
construction costs and provide design options, if any, to remain within the total project
budget.
G. Review of project documents shall be completed by OWNER or OWNER's representative.
Comments shall be generated and resolved accordingly between the parties at each submittal
stage.
H. ENGINEER shall deliver to OWNER the following items as final design documents that
shall become the property of OWNER:
1. Two (2) sets of original As -Bid reproducible tracings of the contract plans including
addenda on standard 11 "x 17" sheets
2. Two (2) copies of the final contract documents.
3. One (1) copy of final contract documents and contract plans in Adobe PDF format.
4. All AutoCAD files in 2013 format with all supporting Civil 3D files.
VIII. UTILITY COORDINATION
A. ENGINEER shall be responsible for all coordination with utilities.
B. ENGINEER shall coordinate a minimum of one (1) meeting with utility companies before
advertising for bidding occurs.
C. ENGINEER shall provide utilities with plans and specifications in sufficient detail that the
utilities may relocate their facilities without conflicting with the PROJECT.
IX. GEOTECHNICAL SERVICES
A. ENGINEER shall provide geotechnical services for subsurface soil and groundwater
conditions at the site to the depths that may be significantly affected by the construction.
Provide soil boring services for a maximum of 10 borings. Soil sampling shall be performed
at intervals that will optimize the subsurface information needed but not greater than 1,500
foot spacing. Borings shall be extended to auger refusal or a maximum depth of 10 feet,
whichever is encountered first.
B. ENGINEER shall determine the engineering characteristics of the subsurface material
encountered. Evaluate slope stability of the proposed opencut improvements.
C. Complete geotechnical report of all soil boring data. The report shall include complete
boring logs, pipe and/or culvert foundation recommendations, trench stability/support
recommendations, groundwater construction management recommendations, and other
relevant information.
X. BIDDING ASSISTANCE
A. ENGINEER shall assist the OWNER in addressing bidder's questions for the Jordan
Woods Neighborhood Improvements Project and submit information for addenda with
applicable design modifications prior to bid opening date. OWNER will distribute Notice to
Bidders; distribute plans and complete bid collection and opening. OWNER will address
bidder's questions and submit information for addenda (if needed). OWNER will discuss

any design modifications with the ENGINEER prior to finalizing said modifications.
B. ENGINEER shall assist the OWNER in review of bids for the Jordan Woods
Neighborhood Improvements Project. A bid tabulation, evaluation, and recommendation
for award shall be completed.
C. ENGINEER shall assist the OWNER in addressing bidder's questions for the Woodland
Country Club Golf Course Detention Project and submit information for addenda with
applicable design modifications prior to bid opening date. OWNER will distribute Notice to
Bidders; distribute plans and complete bid collection and opening. OWNER will address
bidder's questions and submit information for addenda (if needed). OWNER will discuss
any design modifications with the ENGINEER prior to finalizing said modifications.
D. ENGINEER shall assist the OWNER in review of bids for the Woodland Country Club
Golf Course Detention Project. A bid tabulation, evaluation, and recommendation for
award shall be completed.
XI. CONSTRUCTION PHASE SERVICES
A. ENGINEER shall attend a pre -construction meeting, review shop drawings and assist the
OWNER in interpretation of contract documents during construction of the Jordan Woods
Neighborhood Improvements Project.
B. ENGINEER shall prepare construction As -Built digital plans for the Jordan Woods
Neighborhood Improvements Project based on the red -lined drawings provided by the
contractor. The contractor shall provide as -built survey information, in accordance with the
requirements of the Contract Documents. Note: Pipe and swale profile shall be
determined at every 100 feet and cross sections shall be surveyed at 500 feet intervals to
verify the final scope and conformity to the construction plans. As -Built plans shall
include culvert crossing and inverts, pipe size, and bank locations.
C. ENGINEER shall attend a pre -construction meeting, review shop drawings and assist the
OWNER in interpretation of contract documents during construction of the Woodland
Country Club Golf Course Detention Project.
D. ENGINEER shall prepare construction As -Built digital plans for the Woodland Country
Club Golf Course Detention Project based on the red -lined drawings provided by the
contractor. The contractor shall provide as -built survey information, in accordance with the
requirements of the Contract Documents. Note: Cross sections shall be surveyed at 500
feet intervals to verify the final scope and conformity to the construction plans. As -
Built plans shall include culvert crossing and inverts, pipe size, and bank locations.
XII. OTHER AND SERVICES
A. Additional services as may be identified during the course of the PROJECT, which are not
listed in Sections I through XII, shall be performed by the ENGINEER only upon written
authorization by the OWNER.
XIII. ASSUMPTIONS -
This Scope of Services was prepared with the following assumptions. Should the scope of the project

differ from these assumptions, ENGINEER shall submit written notification to OWNER of the
differences and the impacts. ENGINEER shall prepare written justification for any adjustments in fee
resulting from changes in the project scope for review by OWNER.
A. The detention pond on the Woodland Country Club Golf Course is assumed to be
approximately 1.5 acres with up to 3.5 acres of impacted area.
B. The location of the pond(s) is to be determined. If two ponds are necessary, additional piping
will be required to connect the system. It is assumed there will be one pond.
C. The improvements to the neighborhood ditch are assumed to be minimal. It's assumed
improvements will occur within the existing ROW, except for the culvert replacement on the
west side of Timber Lane between Hamilton Lane and Jordan Road, and cost less than
$100,000. If there other- improvements outside the ROW, significant tree clearing is likely
and additional design efforts will be necessary.
D. No stormwater modeling will occur. Traditional hydrologic and hydraulic calculations will
occur within the neighborhood to determine demand and sizing requirements.
E. Easement Documents and Easement Staking fees are provided per parcel and are estimated
for a simple easement within the neighborhood. Should an easement be necessary that is
more complex, additional easement preparation efforts are likely.
In fulfillment of this Agreement, the ENGINEER shall comply with the requirements of the
appropriate regulations and requirements of the OWNER.
