THIS AMENDMENT TO THE AGREEMENT FOR PROFESSIONAL SERVICES ("Agreement) entered into
by and between the City of Carmel and American.Structurepoint, Inc. (the "Professional"), as City Contract dated
April 6, 2016: shall amend the terms of the Agreement by adding the additional services to be provided by
Professional consistent with the Scope of Work attached hereto and incorporated herein as Exhibit"A". The terms
and conditions of the Agreement shall not otherwise be affected by this Additional Services Amendment and shall
remain in full force and effect.

City of Carmel Program Management Scope Items
Rangeline Road Phase II Environmental Site Assessment
The tasks as outlined below constitute the proposed scope of professional services for assessment of
potential contaminated soils and/or groundwater (i.e., Phase II Environmental Site Assessment (ESA))
for the Rangeline Road Improvement project in Carmel, Hamilton County, Indiana. A Phase I ESA
conducted for the project corridor resulted in the following findings:
• Eleven sites along the corridor with recognized environmental conditions(RECs)were identified
due to current or historical land use.
• Two of the eleven sites identified have the potential to have released contaminants to soil and/or
groundwater in the vicinity of the project and warrant additional investigation in order to
determine whether the property to be acquired as part of the Rangeline Road improvements is
contaminated. Further investigation would facilitate determining whether soil or groundwater
contaminants are present, the concentration of the contaminant(s), evaluating worker safety
during construction, quantifying the contaminated media requiring off-site disposal, and
determining the type of disposal required.
The following table summarizes the sampling recommended at the two REC sites recommended for a
Phase II investigation, which may affect the proposed construction activities on Rangeline Road and
adjacent right-of-way(ROW) (see Exhibit 1 for proposed boring locations).

Task 1—Health and Safety Plan
A site-specific Health and Safety Plan (HASP) will be prepared by American Structurepoint that meets
the accepted industry standards.The HASP will,at a minimum,state the scope of services,identify known
hazards on the site, outline any special monitoring protocols, and identify all emergency contingencies.
Task 2—Utility Locates
Prior to site activities, the Indiana Underground Plant Protection Services (IUPPS) will be utilized to
locate public utility lines in the vicinity of the site. Private utility location services are not anticipated,as
the proposed boring locations are within existing ROW.Locates will be requested from IUPPS a minimum
of 48 hours prior to commencement of field investigations. All proposed boring locations will be staked
or marked prior to contacting IUPPS,including a 10-foot diameter area around each stake or mark.
Prior to commencement of drilling work, each marked boring location will be field checked and photo-
documented to verify utilities in the vicinity of the staked/marked boring locations have been located. If
appropriate utility markings are not present,IUPPS will be notified and a subsequent utility locate request
will be submitted.

Task 3—Soil and Groundwater Sampling
The subsurface investigation will consist of the completion of four environmental soil borings at locations
deemed appropriate based on a desktop evaluation of potential contaminant sources, and/or the fmdings
from the Phase I ESA investigation. Borings will be completed via Geoprobe®direct push technology,to
a maximum depth of 30 feet, depending on geologic conditions and groundwater availability. Each soil
boring core will be visually inspected by a field geologist on site. Soil samples at 4-foot intervals will be
field screened using a photo-ionization detector(PID). One soil sample from each boring will be collected
from the interval exhibiting the greatest potential for contamination (e.g., PID readings, visual
observation, odor, etc.). The soil sample containers will be labeled, placed in an ice-packed cooler, and
retained for laboratory analysis using appropriate chain-of-custody protocol. If conditions permit, i.e.,
sufficient groundwater is encountered, the soil borings will be converted to temporary 1-inch PVC
piezometers for the collection of groundwater samples.
One soil sample and one groundwater sample (if available) from each boring will be submitted to an
accredited laboratory for analysis of the parameters listed below.
A. Volatile Organic Compounds (VOCs)via EPA SW846 Method 8260B
B. Semi-VOCs via EPA SW846 Method 8270
C. RCRA 8 Metals via EPA SW846 Method 6010
D. Polycyclic Aromatic Compounds (PAHs)via EPA SW846 Method 8270
E. Percent Moisture via EPA SW846 Method 1664(soil only)
Soil and groundwater sample analysis will utilize analytical methods, detection limits, and QA/QC
reporting protocol consistent with the Indiana Department of Environmental Management(IDEM)2013
Remediation Closure Guide or other appropriate IDEM guidance. Upon completion of site activities,
temporary piezometers will be abandoned by a licensed driller and all boreholes will be backfilled with
bentonite and capped to match original conditions.
Task 4—Prepare Investigation Report
A technical report describing and documenting the activities performed during the investigation, a
summary of field activities, and the laboratory analysis results will be provided. Any opinions and
conclusions regarding the presence and extent of contaminants in the soil and/or groundwater within the
project boundary will be included in the report.
If, during consultation with the client, it is determined that the proposed project will require additional
soil/groundwater characterization or remediation, the services required to complete a Further Site
Investigation (FSI), a Remediation Work Plan (RWP), or Special Provisions related to the handling or
disposal of contaminated materials will constitute an Additional Service.
We shall have no responsibility for oversight or supervision of the contractors or their employees, for the means
and methods of construction,or for the safety of persons on or off the job site. We shall have no responsibility to
remove hazardous materials. If petroleum hydrocarbons are encountered in the soil and/or groundwater samples
during the Phase II ESA,we recommend the responsible party report the suspected release to the IDEM. We will
not be responsible for reporting to any regulatory agencies.
We will provide the client with a draft investigation report for review and incorporate comments received into a
fmal investigation report. We will perform these services in a manner consistent with the degree of care and skill
ordinarily exercised by members of the same profession currently practicing under similar circumstances.

PROJECT DELIVERABLES
Our deliverable will be a Phase II ESA Investigation Report,which will include the following:
A. Summary of the analytical results for soil and groundwater
B. Recommendations for further investigation, if necessary, including proposed locations for additional soil
borings and/or monitoring well installation
C. Maps and exhibits of the project area depicting sampling locations and other pertinent data
D. Analysis of the subsurface materials,including USDA or USCS classification of soils,estimated groundwater
depth, and other pertinent geologic data
E. Appendices with soil boring logs,laboratory analytical report, and appropriate supporting documentation
Utility locates will be performed within 5 days from our Notice to Proceed. We will perform the Phase II ESA
field work and data collection within 3 days following utility clearance of the project area. We will submit a draft
investigation report to the client within 30 days of completion of the field investigation. We will submit a final
report within 5 days following receipt of comments from the client.
ADDITIONAL SERVICES
The services listed above represent the total scope of services as understood at this time. Should the need for
Additional Services arise,we are willing to provide them for a supplemental fee. Fees for the following Additional
Services are not included in the fee for Basic Services. These services will be provided only if authorized in
writing in advance:
A. Studies: Further Site Investigation(FSI)or Remediation Work Plan(RWP
B. Sampling of media from underground structures,including oil/water separator(s),septic lines or septic tanks,
and/or Underground Storage Tanks(USTs)
C. UST closure activities and/or excavations
D. Private Utility Location or Subsurface Utility Engineering(SUE) services
E. Services resulting from changes of scope or magnitude of the project as described above
