This Addendum is issued in accordance with the provisions of The General Conditions of the Contract for
Construction, Article 1 Contract Documents, and becomes a part of the Contract Documents as provided therein.
The information contained herein modifies the original Bidding Documents dated 03.13.2017. Requirements of the
original Bidding Documents and previous Addenda remain in effect except as modified by this Addendum.
Acknowledge receipt of this Addendum in the space provided on the Bid Form. Failure to do so may subject the
Bidder to disqualification.
PART 1 – PROJECT MANUAL CHANGES
1. SECTION 004101 – BID FORM
a. Replace Bid Form with new Bid Form (attached). Note that there are now two options for
bidding, one with the Performance Bond and Payment Bond and one without these Bonds.
Contractors shall have the option to bid either with or without these Bonds (or bid both ways).
2. SECTION 006000 – PROJECT DELIVERY FORMS
a. Article 1.2.B. Delete this paragraph. A Bid Bond will not be required.
PART 2 – DRAWING CHANGES
1. SHEET G-002 – SHEET INDEX, PROJ LOCATION, CODE SUMMARY, AND LIFE SAFETY PLAN
a. Code Summary has been revised. Replace existing sheet with new attached sheet.

2. SHEET AD-101 – FIRST FLOOR DEMO PLAN
a. Existing safe door is to be removed. Change keynote to “03” as indicated on attached Sketch
ADD1-1.
b. Existing slab depression at safe door is to be filled as required for installation of flooring. Add
keynote “20” as indicated on attached Sketch ADD1-1.
c. Add keynote “20” to Keynote Legend to read as follows: “Infill existing slab depression as
req’d for installation of new flooring”.
3. SHEET A-101 – FIRST LEVEL FLOOR PLAN
a. Existing safe door has been deleted as indicated on attached Sketch ADD1 -2. Wall type S1FA.97
now extends across existing safe wall to wall type S3C.P.
b. Slope flooring up at old safe door as required. Maximum slope to be 1:20.
4. SHEET A-601 – DOOR SCHEDULE AND DETAILS
a. Add Detail A7/A-601 – “CASED OPENING JAMB @ OLD SAFE DOOR (HEAD SIM.)” as
indicated on attached Sketch ADD1-3.
PART 3 – CLARIFICATIONS
1. The specifications indicate that we are required to provide proof from State of Illinois that we have
workman’s compensation insurance. Since we are an Indiana company and the project is in Indiana, can
you correct/confirm that the requirement is from the State of Indiana? Correct…the requirement would be
for the State of Indiana, not Illinois.

5. ACKNOWLEDGMENT
The undersigned Bidder understands that the Owner reserves the right to reject this bid, and that this bid shall
remain open and shall not be withdrawn for a period of sixty (60) calendar days from the date prescrib ed for its
opening.
6. TIME OF COMPLETION
If awarded this Contract, the undersigned Bidder acknowledges that work of the Base Bid and accepted Alternate
Bids shall commence and be completed, including all punch list items in accordance with work sequence
described in Division 1 Section “Summary”, with Substantial Completion no later than July 28, 2017 and Final
Completion no later than August 18, 2017.
7. SUBSTITUTIONS
The undersigned Bidder has based his bid upon the materials, products, articles, equipment, brands, manufacturers
and processes described in the Bidding Documents or upon approved equivalents. Proof of equivalency of
substitutions is the responsibility of the Bidder, but the Architect shall be the sole judge of equivalency. Proposed
equivalent substitutions shall be equal in all respects to the requirements of the Bidding Documents, including but
not limited to the design, quality, physical size, performance characteristics, strength, previous history of use, and
to the method of installation, attachment, or connection to related or adjoining work. Determination of
equivalency of proposed substitutions shall be made by the Architect before the bid opening date, as described in
paragraph “Substitutions” in the Division 0 Section “Instructions to Bidders.”

B. Explanation of Bid Process
1. Bids due at 2PM on Thursday, April 13, 2017 by email to JGilbert@RATIOdesign.com
2. The bid opening will be private.
3. Bids to be held for 60 days.
4. Complete all blanks on bid form, including alternate bids. Sign and properly execute all
documents.
5. Deadline for submitting questions in writing: 5PM on Friday, April 7, 2017.
a. Please submit all questions in writing, if possible.
b. Phone calls are acceptable.
c. Responses will be published via addendum for all bidders to see.
6. Deadline for submitting substitution requests: 5PM on Friday, April 7, 2017.
7. Final addendum shall be issued no later than 12PM on Tuesday, April 11, 2017.
8. Status of Building Permit:
a. A code interpretation is in the works regarding the fact that the entire building is
non-sprinklered.
b. Once we have the interpretation, drawings and specs will soon be submitted to
State of Indiana and City of Carmel.
C. Project Information

3) Sawcutting, using the jack-hammer, cutting/demo piping and metal
ductwork will need to be performed before/after normal business
hours of Monday-Friday 8:00 am - 5:00 pm.
F. Security and housekeeping procedures
1. GC is responsible for keeping a clean and safe site.
2. Contractors are responsible for securing their materials.
3. GC is responsible for maintaining building / construction site security at all times.
G. Responsibility for life safety
1. Leave site in safe condition at the end of each day - replace barricades and safety
barriers, etc.
2. Notify Architect if hazardous materials are identified. Owner will provide applicable
reports when requested.
3. Maintain building life safety systems for occupied areas such as smoke and fire alarms,
emergency lighting and power, intercom, etc.
4. The existing two-story building was the original building built in 1974. The
adjacent six-story tower was built in 1978 as a free-standing tower. A connector
for the two buildings was built in 1979.
5. Do NOT cut anything (conduit/cable/piping/ductwork/etc.…) that supplies the
tower.
6. Jeff Foreman plans to be on-site everyday during construction. He will be
available for questions of what can and cannot be cut. Jeff will work together with
the contractor on a daily basis.
H. Temporary facilities and use of Owner's facilities
1. Facilities provided by SePRO available for contractor use at no charge: water, sanitary,
toilets, drinking fountains, HVAC, and power.
I. Work by others, coordination between prime contractors and others.
1. Furniture (separate contract) with Stocks Office Furniture.
2. Audio/Visual (separate contract) with Tandy & Associates.
3. Security (separate contract) with FE Moran.
