THE LAKES AT HAZELDELL SECTION 3
AGREEMENT
The developer shall be responsible for any drainage problems includ-
ing standing water, flooding and erosion control, which arise or
bedome evident at any time during the 3 year maintenance period after
release of subdivision bonds, and which is attributable to a
deficiency in subdivision drainage design or construction of drain-
age improvements. This shall include. all pipes, structures, swales,
ditches and ponds which are pertinent features to the proper drain-
age of the subdivision.
This responsibility of the developer shall not, however, include
problems which are created, subsequent to the completion of the
subdivision improvements by the improper grading by individual
builders, or structures and improper grading installed or accom-
plished h}- individual homes•-ners.
It is the intent of this requirements, that the developer shall,
specifically, provide such sub-surface drains, or storm sewers
ditches as are required to properly rectify any drainage problem
or sub-surface water problem which was not contemplated in the
original approved subdivision design, including, but not restricted
to, disposal of sub-surface water from footing drains of individual
lots.
ROAD IMPROVEMENTS: The typical agreement for. existing contiguous
Hamilton County Roads which are substandard, with Hamilton County
is: NONE - —
The developer will put in 12 inch stone base to the required width
of the road, and grade the shoulders to a minimum of 6 feet width
and construct proper side-ditches, or, provide storm sewers and
curbs. This will be the black top grade during construction of
subdivision. Then the county will come in and cut out 3 inches
and put in 3. inch binder .alt after which the developer will be
responsible for 1 inch ofasphalt topping. This is being done as
joint projects between county and developer. An agreement shall be
executed, in writing, between the developer and Hamilton County
specifically detailing the exact work to be accomplished by the
developer and that to be accomplished by the County, and shall also
state the road or roads or portions thereof which are included.
If the subdivision is contemplated to be annexed to the City of
Carmel, then the agreement shall include the City of Carmel Board
of Public Works as a signatory, which shall be for the purpose of
the City of Carmel agreeing to accept for maintenance and operation,
the improved road facility upon completion of said improvements
and completion of annexation.

Dear Keith:
We have reviewed the revised plans for the Lakes at Hazel Dell, Section 3, which were submitted to this
office on March 12,2001. The revisions are acceptable. Therefore, as of this date, the drawings are
approved.
The following requirements must be satisfied:
• Certified Engineer's Cost Estimates.
• Performance Guarantees
• Minimum 5-sets Certified Construction Drawings.
• Water/Sewer Availability Fees.
The Board of Public Works and Safety approval of Water and Sewer Availability has expired and must be
resubmitted. Availability was originally approved on March 1, 2000 with the stipulation that construction
or development of the project must begin within twelve months or said approval expires and the developer
or builder must reapply to the Board. The next available Board meeting is April 18. 2001. 1t also appears
the 327 IAC Sanitary Sewer Permit expired at midnight on April 1,2001.
I am enclosing a copy of the project status report and a copy of the calculation of Availability(acreage)
Fees due on this project.

This office has reviewed revisions,which were incorporated into your comment letter,and revised drawing
dated February 10, 2000. We believe you have addressed our concerns and requests as outlined in our
letter dated January 18, 2000. Therefore,we approve the drawing submitted on February 10,2000. This
approval is contingent upon our understanding that all drawings submitted for these projects have no
modifications or changes except those items reviewed by this office. It is critical that our office be made
aware of all modifications made on plans submitted for review,particularly changes that are considered
"new"or falling outside of our initial review.

Dear Mr. Lash:
Our office has reviewed the Secondary Plat application filed for The Lakes at Hazel Dell,
Section 3. The following are comments that need to be addressed and plans updated accordingly:
Application:
• The filing fee for the Secondary Plat is$500.00.
• The Subdivider Agreement Form for this Section needs to be filed.
Secondary Plat:
• The type and size of the various monuments markers are not indicated.
• Please provide a Key Map illustrating the location of this Section within the overall Lakes at
Hazel Dell project.
• Please assign the designation "Block 3a" to the Common Area along the west side of this
section.
• The year on the Surveyor's Certificate needs to be changed to"2000."
• The note to"Remove Ex.Fence" on Lot 87 should be deleted.
• The Source of Title has not been included on the plat.
•Please us the Administrative version of the Plan Commission Certificate.
Construction Plans:
• The street trees to be planted in this Section have been omitted from the Landscape Plan.
Please provide a revised Plan detailing these.
• There has not been any Plan Commission approval granted for Subdivision signage for the
Lakes at Hazel Dell Subdivision, nor has signage been proposed. Visits to this site reveal that
several permanent Subdivision signs have been established without approval or permits. In
addition, there is a good deal of temporary signage established on this site without permits.
Failure to rectify this situation rapidly could cause delays in further approvals and in the issuing

of other permits or certificates for this project.
• Where sidewalks meet curbs handicap-accessible ramps need to be established.
• Any further correspondence from the Technical Advisory Committee will need to be
submitted to this Department.

Dear Keith:
We have reviewed the plans submitted for this project. We offer the following comments:
1. All infrastructure improvements are City of Cannel jurisdiction.
2. The project will require board of Public Works and Safety approval of Water and Sanitary Sewer
Availability. Plan Commission and/or BZA approval is required prior to Board of Works submissions.
This office will not submit Board of Works items based upon Plan Commission approval on the
Tuesday prior to the Wednesday morning Board of Public Works and Safety meeting.
3. The project will require IDEM permits for Water and Sanitary Sewer. The 330 IAC construction
Permit will not be signed by the City Engineer until availability has been approved by the Board of
Public Works and Safety.
4. The project will require Board of Works approval of the secondary plat. All required performance
guarantees must be posted prior to submission of the plat to the Board. The plat must be recorded
before building permits will be issued.
5. Please review the storm sewers at structure numbers 713,711,707,706, 705,and 704. Our office
would prefer alignment of the tops of the inlet and outlet pipes rather than inverts when possible. If it
is not possible to align the tops of pipes,we would request the inlet pipe invert to be raised above the
outlet pipe invert.
— 6. An easement will need to be provided for all offsite storm and sanitary sewer work. If sections 3 and
4 are platted at the same time,an easement will only be required for the offsite sanitary sewer.
7. Please provide additional elevation and drainage pattern information for the common areas along the
western boundary of section 3 and 4 and the southern boundary of section 4 to insure that existing
drainage problems are remedied,and any possible future problems are avoided.
8. The demolition of the existing asphalt path along Hazel Dell Parkway and the relocation and
reinstallation of the new path must be coordinated to minimize the period of time the path is not
accessible to the public. Our office would request the asphalt path be re-opened to pedestrian traffic
within 2 weeks,weather permitting. In addition,adequate signage in both directions will be required
both in advance of path reconstruction(116t Street and at northern entrance to Dellfield Blvd. West
Drive)and at the actual closure location.
9. Why is the proposed asphalt path along Hazel Dell located so far to the west? Its current location
doesn't appear to match that of the entrances from section 1.
10. What are your plans for removing/relocating existing trees within Hazel Dell Parkway right of way?
Please coordinate these relocations with our office and/or The Department of Community Services.
11. We would request a modification to the note on Sheet L101 of Section 4 plans. Please change the note
to"Hazel Dell Road Planting Plan to be Coordinated with Cannel D.O.C.S.and/or Department of
Engineering".
12. Any lane closures associated with the construction of the entrance need to be coordinated with the City
of Carmel Engineering Department. In addition,proper advance signage will need to be utilized if lane
closures will occur. (i.e.lane closed ahead,merge left) It was difficult to determine if lane closures
will be utilized,please indicate this on the maintenance of traffic plans.

13. The pavement in the existing travel lanes of Hazel Dell will need to be protected during the
construction of the entrance.
14. Please confirm that the section of pavement for the acelldecel lanes matches the existing pavement
section for Hazel Dell Parkway.
15. Please notify the contractor that no vehicles are to be parked within Hazel Dell Parkway right of way,
and that motor vehicles are not to be driven on the asphalt path.
16. Carmel Utilities will provide a separate review of water and sanitary sewer issues associated with the
project.
