THIS AMENDMENT TO THE AGREEMENT FOR PROFESSIONAL SERVICES ("Agreement") entered into
by and between the City of Carmel and VS Engineering. Inc. (the "Professional'), as City Contract dated June 1.
2016 shall amend the terms of the Agreement by adding the additional services to be provided by Professional
consistent with the Scope of Work attached hereto and incorporated herein as Exhibit "A". The terms and conditions
of the Agreement shall not otherwise be affected by this Additional Services Amendment and shall remain in full force
and effect.

L PROJECT DESCRIPTION
Project Name: Jordan Woods Stormwater Improvements
Owner: City of Carmel, Indiana
Engineer: VS Engineering, Inc.
A. Project Location and Overview
The City of Carmel proposes to construct a regional detention pond located within the Woodlands
County Club golf course. The project is located north of the Jordan Woods Subdivision located
north of 106`h Street and west of Keystone Parkway.
This Amendment #4 is to provide for the professional services of Timothy Liddy Associates, a
golf course landscape architect that was requested by the Woodlands Country Club to ensure the
integrity of the golf course is maintained through the installation of the pond.
II. GENERAL REQUIREMENTS
The general scope of services and deliverables are described in the attached scope provided by
Timothy Liddy Associates.

Golf Design Proposal
Scope of Work
Timothy Liddy / Associates, Inc. will prepare preliminary design and construction drawings for the
remodeling of Golf Holes #10 and #16 at Woodland Country Club in Carmel, Indiana.
1.0 Program / Analysis
1.1 Consultation with the owner and Mr. Dye to review the site, complete the planning approach and
establish schedules for reviews, events and presentations. Obtain insights, goals, issues, opportunities
and priorities.
1.2 Preparation of a base sheet of existing conditions from a survey provided by the owner. The survey
will have one foot contours and all existing physical features located on the site.
2.0 Design Development Phase (Schematic Design)
2.1 Based on the essential components for the goals and objectives, Timothy Liddy / Associates will draw
preliminary plans with Mr. Dye, Woodland Country Club and VS Engineering.
2.2 Timothy Liddy/Associates. Inc. will prepare schematic plans for approval by Woodland Country Club
and VS Engineering. The plans will include preliminary layout plans, golf course grading plans, golf
course drainage plans, golf course grassing plans, and necessary feature details of the the greens, tees,
fairways, cart paths, bunkers and lakes.
3.0 Construction Documents
3.1 Timothy Liddy lAssociates, Inc, shall provide preliminary construction documents based on approved
design development documents, adjustments in project scope, quality or budget by the owner, and
conditions imposed by permits issued for the project, as applicable. These drawings will include a
preliminary grading and drainage plan prepared in AutoCad for use by the VS Engineering to obtain
necessary permits.
3.2 The Construction Documents shall consist of drawings and specifications setting forth in detail the
requirements for the construction of the project, including the respective responsibilities of owner,
contractor and golf course architect, together with appropriate bidding and proposal Information for
contractors. The golf course architect shall determine the format and graphic character. Such documents
shall be sufficient for a contractor experienced and knowledgeable in golf course construction to provide a
qualified bid and complete the work.
3.3 These drawings will include: Golf course layout plans, golf course grading plans, golf course drainage
plans, golf course grassing plans, golf course irrigation plans, specifications, bid and contract forms.

3.4 The drawings do not include any necessary permit drawings such as erosion control or state and
federal environmental permit requirements. All plans will be prepared in AutoCad for use by VS
Engineering in preparing drawings for necessary permits and engineering support.
4.0 Contract Terms
The work described above shall be accomplished on a fee not to exceed $80,000, broken into phases of
either lump sum or hourly not to exceed as shown in the below summary table. It will be invoiced monthly
and as follows:

The owner, for direct reimbursable expenses, will reimburse Timothy Liddy / Associates, Inc. for: travel,
graphic reproduction expenses and overnight mail incurred expressly for the project. It shall be billed at
cost plus 5% for handling and carry cost. Direct costs go toward the above contract amount and are
included in the not to exceed amount of $80,000.00.
