State of Indiana, Register No. S0083, does hereby certify to Manor Healthcare Corp., a Delaware Corporation, Beverly Enterprises-tndiona, Inc., o California Corporation and Commonwealth Land Title Insurance Company, as follows: I. This plat and the survey on which it is based were made in accordance with the "Minimum Standard Detail Requirements for ALTA/ACSM Land Title Surveys" jointly established and adopted by the ALTA and ACSM in 1992, including Item Nos. 1, 2, 3, 4, 6, 7(a), 7(c), 8, 9, 10, 11, 13 and 14 of Table A (Optional Survey Responsibilities and Specifications); and meets the accuracy requirements of an Urban Survey, as defined therein. 2. DATE I BY SURVEYOR'S REPORT In accordance with Title 865, Article 1.1 Chapter 12, Section 1 through 34 of the Indiana Administrative Code, the following observations and opinions are submitted regarding the various uncertainties in the locations of the lines and corners established on this survey as a result of: a Variances in the reference monuments; b Discrepancies in record descriptions and plats; c Inconsistencies in lines of occupation and; d Random errors it measurement (theoretical uncertainty); The theoretical uncertainty (due to random errors in measurement) of the corners of the subject tract established by this survey is within the specifications for a Class C. Survey (0.50) feet as defined in IAC 865. This is a retracement survey of 3 tracts of real estate described in Instrument #8724249. The Southeast Quarter of Section 26 as surveyed, very nearly (0.2' more or less) matches the quarter section survey recorded in Plat Book 10, Page 146. The quarter section recorded in Plat Book 10, Page 146 is the basis of the record descriptions used for this survey. The bearings and distances in the record description (Instrument #87-27249) were honored for this survey. An east -west uncertainty of 1.8' more or less, to 2.0' more or less, exists along boundary line 563-564_ A north -south uncertainty of 0.4 feet, more or less, exists between Point #556 and 557 due to on existing fence. An east -west uncertainty of 0.3' more or less, exists between Point #557 and 559 due to an existing fence line. East -west uncertainties, 3.2 feet, more or less, 6.7 feet, more or less and 3.9 feet, more or less, exist between Point #564 and 563 due to overlaps with various adjoiner's deeds. Two east adjoiners (Deed Record 211, Page 71 and Deed Record 175, Page 321) may "overlap" or "gap" with bounder; line 563-564 These descriptions are written from the centerline of U -S. HiThway #31 and when plotted from the existing centerline of U S Highway #31, a "gap in excess of 20 feet, more or less, exists with these porcels. Dimensions from adjoining properties indicate U.S. Highway #31 as currently located may be 34 feet, more or less, east of a previous centerline location.
