HomeMy WebLinkAboutIndiana University Health/HR/Fourth Amendment to Master Services Agreement for Designated Health Care ServicesIndiana University Healthb,4 Workplace Services JAS FOURTH AMENDMENT TO CITY OF CARMEL, INDIANA MASTER SERVICES AGREEMENT FOR DESIGNATED HEALTH CARE SERVICES This Fourth Amendment to the Master Services Agreement for Designated Health Care Services (this "Amendment") is entered into on July 1, 2018 (the "Fourth Amendment Effective Date"), by and between the City of Carmel, Indiana, a municipality (hereinafter "City") and Indiana University Health, Inc. (hereinafter "IU Health"). Recitals A. IU Health and City entered into a Master Services Agreement for Designated Health Care Services dated February 1, 2013 (the "Agreement'). B. IU Health and City subsequently amended the Agreement, effective January 25, 2016 ("First Amendment'). C. IJ Health and City subsequently amended the Agreement, effective January 1, 2017 ("Second Amendment'). D. IU Health and City subsequently amended the Agreement, effective December 20, 2017 ("Third Amendment'). E. The parties desire to further amend the Agreement to incorporate additional services. Amendment NOW, THEREFORE, the parties agree as follows: 1. Definitions. Capitalized terms used in this Amendment but not defined in this Amendment shall have the meanings assigned to them in the Agreement. 2. Term. Effective on the Fourth Amendment Effective Date, the second sentence of Section 2 of the Agreement, Term, is hereby deleted and replaced with the following: "Except for the term of Service Line Attachment #5, the term of any particular SLA shall coincide with the term of this Master Services Agreement." 3. Service Line Attachments. Effective on the Fourth Amendment Effective Date, Service Line Attachment #5 Urgent Care Enhanced Access, attached hereto, shall be added to the Agreement. 4. Effective Amendment. The parties agree that this Amendment has been duly prepared and executed in accordance with the terms of the Agreement. 5. No Other Modifications. Except as otherwise set forth in this Amendment, all other provisions of the Agreement shall remain unchanged. Huesing, iTP/Treasw e Date: —71 CITY OF CARMEL, INDIANA by and through its Board of Public Works and Safety Ziw Jaiffies Brainard, Presiding Officer Date: Date: Lori W Board Member Christine Pauley ClerkTreasurer Date: 17/��/! 3 SERVICE LINE ATTACHMENT# 5 URGENT CARE ENHANCED ACCESS Addition of Enhanced Care Services Subject to the provisions of the Agreement and this SLA, IU Health Business Solutions agrees to provide eligible employees of the City of Carmel with access to IU Health Urgent Care Centers located at: Broad Ripple, Plainfield, Brownsburg, Hazel Dell, Lawrence, and Greenwood: 1) Services: Effective July 1, 2018, eligible members of the City of Carmel health plan will have access to all IU Health Urgent Care Centers listed above when the City's Primary Plus Employee Health Center is not open. Eligible members must present their Enhanced Access identification card in order to receive services at the cost outlined below. If the Enhanced Access identification card is not presented at the time of service, all charges will be processed through the City's health plan. Hours of access are: Monday, 5pm-8pm Tuesday, 6pm-8pm Wednesday, 5pm-8pm Thursday, Sam-lpm and 5pm -8pm Friday, 1pm -8pm Saturday -Sunday, 8am-6pm. *Hours may be changed if both parties agree in writing. 2) Term: The services provided under this SLA are effective July 1, 2018, to June 30, 2019, after which the parties will review outcomes and make any modifications necessary for the plan to continue or terminate as appropriate. 3) Cost of Services: Plan members will have no out of pocket cost. All charges will be billed to City of Carmel as utilized on a monthly basis. The cost of visits completed at the 1U Health Urgent Care Centers via the Enhanced Care model, will be 62% of billed charges. El
