NO.: CITY PO NO.: CITY PO DATE: 100876 August 27, 2018 You are directed to make the following changes in this Contract: Field Drilled Holes in concrete - End Bent No. 2 - work is for Hamilton County Bridge #36 and has been approved by the Hamilton County Bridge Program Manager. II. The following referenced Documents further describe the changes outlined in Paragraph I, and are to be considered a part of this Change Order: R.F.P.: WDC No.: Field drilled holes are required by the plans for retro fitted threaded tie bar assemblies for end bent #2. However, there is no pay item for this work. In accordance with the Indiana Department of Transportation, specification reference 705-51863, there should have been an pay item included in the original schedule of values. The email from the design engineer of record supports this. Herein is the price for this work based on actual work performed by the contractor. Field Drilled holes are typically a separate pay item with respect to INDOT work and there is no item on the original schedule, no quantity listed in the Miscellaneous quantities on the bill of materials for the superstructure work, no note in the plans stating this work is complimentary to other items, and nothing indicating so in the technical specifications included in the CIB.
