Morphey has submitted a Time Extension Request for an Additional 41 Calendar Days to be added to the contract. This Request is due to delays in receiving INDOT ROW permits for work on project. (See Attachment A) B. EWA 08 has been submitted for work on a Citizens Water service line that was in conflict with the storm sewer on Line PR -C Station 31+70. The Additional work is for boring and new line to the meter and attaching to the existing meter. The additional cost to the project is $4,500.00 and 3 additional calendar days added to the contract. (See Attachment B) C. EWA 07 has been submitted for Structure Modification completed in the field due the size of the pipe was a 30"00" box under 961 street instead of the 30" CMP at the outflow. The additional cost to the contract is $1,614.03 and 1 additional calendar day added to the contract. (See Attachment Q. D. EWA 10 has been submitted for Added Milling on Line PR -C from station 28+75 to station 34+00. The additional cost to the contract is $3,650.00 and 1 additional calendar day added to the contract. (MCI) mobilized off the project from 05-16-18 through 05-29-18. In light of these discussions, MCI would request adding 41 calendar days to the following substantial and final completion dates: Shelborne Road Trail, from Inverness Boulevard to 106"' Street Substantial Completion: Work Completed — No Change Shelborne Road Trail, from 96t6 Street to Inverness Boulevard Substantial Completion: +41 Calendar Days 96th Street Trail from Michigan Road to Ditch Road Substantial Completion: +41 Calendar Days Final Completion, all Sections: +41 Calendar Days This 41 calendar day request accounts for the delays experienced while INDOT ROW permits were being issued for the Michigan Road intersection included in our Contract. In addition to this 41 calendar day request, 10 additional calendar days have been previously requested in pending extra work agreements (EWA 07 -10). Books, PE, CPESC Construction Department Manager Butler, Fairman & Seufert, Inc. 8450 Westfield Blvd., Suite 300 Indianapolis, IN 46240-8302 Re: EWA 08 — Water Service, Far Side, Citizens Water In Reference to: Verbal RFP for Relocating Citizens Water Services &I While installing the proposed storm sewer improvement on Line "PR -C" a Citizens Energy Group (CEG) water service was found to be in conflict at —Sta. 31+70 Ln "PR -C". The service line was temporary spliced around our Contract's storm sewer improvement. However, CEG does not allow permanent splices and requires a new service line from the water main to the meter pit. Per our verbal discussions, Morphey Construction Inc. proposes boring a new service line across 96th street to make the proper repairs.
