HALL O & M MANUAL Owner Acknowledgement Agreement ("Agreement") For good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, the undersigned owner ('Owner') hereby submits this Operation and Maintenance Manual ('Manuar) to the Cay of Cannel, Indiana ('City') as a written acknowledgement of Owners warranty and agreement to institute, maintain, and follow the water quality Best Management Practices CBMPs) listed below, and to follow and abide by the inspection schedule and maintenance activities listed in this Manual. The Owner also hereby agrees to provide, at Owners cost all additional maintenance, repair, and/or replacement services reasonably necessary to maintain the function and longevity of the BMPs from and including the date this Agreement is executed by Owner to and including the date on which a new Agreement is filed with the City by another party who assumes all of the obligations and responsibilities of Owner as set forth herein. The BMP owner agrees to pay all the fees required by the City of Cannel, including annual inspection fees, and /or any additional fees required. The adjacent property owners are responsible to maintain all improvements, including the detention and pavement section to include snow plowing, street cleaning, resurfacing, detention basin and storm sewer system maintenance per the approved and recorded 0 & M Manual. The adjacent owners will share an equal portion of the expense in repair and maintenance par the recorded covenants, conddions and restrictions for this development This condition shall be a burden upon the adjacent owners until such time that the City of Carmel shall eMend the alley to 4th Street and modify the drainage system under said alley. The City of Camlel has a rightoRentry to inspect and maintain the BMPs as necessary Submittal of annual inspection reports to the City of Carmel Engineering Department is required. The first report is due one year after construction is completed, with subsequent reports due each year within the same month of the initial report If there are any deficiencies found during the inspection, these should be addressed. If the inspection report is not received within the month it is due, if there are deficiencies which were not included in the report, or if any deficiencies included in the report are not addressed in a timely manner, the BMP owner faces enforcement action from the City. The City of Cannel representatives have the right to enter the properly to inspect the BMPs. JAMES D. HALL A water Quality BMP is a terrain feature or manmade structure that stops dirt and debris from entering a storm system. Narrative 1) Sumped storm structures numbered 603, 605 will have the SNOUT outlet, oil and debris filters attached. These will inhibit the introduction of large floatables into the underground detention basin. The sumped inlets will allow the settling of fine material prior to water entering the underground detention basin. ADDITIONAL 3' stone Infiltration Trenches are places on each side of the alley to also fitter runoff from pavement and adjacent lots. 2) Vegetated Swales and Dry Detention basin will filter surface flows trapping fines and most suspended solids via infiltration before the waters enter into the underground detention basin. SEE INSERT MAP SECTION II — MINIMUM STANDARDS OF CARE For storm sewer inlets: Inspect castings for debris and remove immediately. At a minimum, inspections to occurs after each major rain event or monthly if no rain occurs. During the fall season, remove all fallen leaves from vegetated swales and dry detention basin. This may need to be done often until all leaves are down for the season. For vegetated swales and dry detention basin, pick up and dispose of large debris regularly. Mow areas sparingly (2-3 times per year max) while maintaining a grass height of 3 — 4 inches. For the Structures 603, 605 see the following manufactures recommendations for the SNOUT Shield. SNOUT/SUMP structures to be maintained by a vacuum truck annually if debris is found during the annual inspection. A waste removal company should be contacted to conduct the yearly inspections on Structures 603, 605 and the Outlet Control Structure. The main- tenance items have been adapted from multiple stormwater programs, including the Rouge River Detention Basin Maintenance Manual, Georgia Stormwater Management Manual, the Vermont Stormwater Management Manual, and the Stormwater Manager's Resource Center. The checklists are designed m help identify key components of BMPs that require ongoing maintenance as well as a basic schedule of when the maintenance should occur. The checklists have been divided into those items essential for the general operation and functionality of the BMP and those items that optional and may enhance the BMP. It is suggested that the inspection be undertaken by a licensed PE and/or a person knowledgeable about the design and function of the BMP.
