FILING FEES Commercial Variance—Additional $353.00 Total Due: $353.00 Filing Dates&Deadlines-Petitioner, please note the following: 1. This item does not need to be on a meeting agenda of the Technical Advisory Committee (TAC). 2. All Mailed, Posted, and Published Public Notice needs to occur no later than end of day Tues., Nov. 6 (but may occur earlier). Published notice (legal ad) is required within The Indianapolis Star; please contact them at least 3 days prior to the public notice deadline in order for them to publish your ad on time (877-925-0228 x 3,or publicnotices@indystar.com and grsc-legals@gannett.com ). Also, letters to adjoining property owners& placement of a public hearing sign on the property is also required by the BZA Rules of Procedure,Article VI, Section 6 (see Application and Calendar). Make sure the meeting time to the newspaper,on the sign, and in letters to neighbors reads 6:00 PM and the location reads Carmel City Hall Council Chambers, 1 Civic Sq., 2nd Flr, Carmel, IN 46032. 3. The Filing Fee and 7 Informational Packets must be delivered to BZA Secretary Joe Shestak no later than Noon Fri., Nov. 16. Failure to submit Informational Packets by this time will result in the automatic tabling of the petition. 4. Please include the following in your Informational Packets: a) Page 11—Ballot Sheet(leave blank except for Docket No. & Petitioner) b) Page 12—Findings of Fact sheet (question sheet filled out) c) Statement of Variance request (short paragraph explaining what you are asking for) d) Statements of Support(short paragraph explaining reasons supporting the request, including analysis and comparisons of what is allowed vs. what is proposed) e) Location Map f) Reductions of all plans(site plan, exterior architectural building elevations, signage design and signage details/specs,etc.)on 8.5"x11" or 11"x17" paper. 1 g) Any other pertinent information for the Board to explain this request ksuch as photographs of site&context, letters of support, etc.). 5. The Proof of Notice needs to be received by this Department no later than Noon,Wed., Nov. 21. Failure to submit Proof of Notice by this time will result in the tabling of the petition. 6. This Item will appear on the Mon., Nov. 26 meeting agenda of the Board of Zoning Appeals Regular Meeting, under Public Hearings, at 6:00 PM. 7. The petitioner needs to provide a Ballot Sheet and a fully filled-out Findings-of-Fact sheet for each docket/petition the night of the meeting for the Board's use (Sheet 12 of the application). On Ballot sheets, only fill out docket number, petitioner, and date (Sheet 11). 8. Either the Petitioner or someone representing the Petitioner must be present at the meeting to present the item to be heard. If no one appears, it will be tabled. 9. Please make checks payable to The City of Carmel. 2. Provide a copy of the Official List of Adjacent Property Owners from the Hamilton County Auditor's Office. (Make sure to check 'Carmel BZA'on their form.) 3. Provide a copy of the Official List of Adjacent Property Owners from Marion County/Indianapolis,too. 4. Make sure the public notice meeting time to the newspaper and neighbors reads 6:00 PM. 5. The Indianapolis Star needs to have your legal notice submitted to them before Noon,Wed., Oct. 31, in order to publish it on time (please verify this with them, since their deadlines might be changed due to Election Day, Nov. 6.). Please contact The Indianapolis Star as soon as you can to make sure you can get your public notice legal ad printed on time, at 317-444-7163 or publicnotices@indystar.com and grsc-legals@gannett.com. (Use the Notice of Public Hearing form in the Application as a template for what to have published in the newspaper&what to mail out to the adjoining neighbors. 6. The notice to the neighbors should be done by first class mail with Certificates of Mailing (not Certified Mail). Please provide us with the certificate of mailing receipts for the file on or before Nov. 21. 7. Please provide the filled out and notarized Placement of Public Notice Sign affidavit once the sign is in place. 8. Please provide the filled out and notarized Petitioner's Affidavit of Notice of Public Hearing form. 9. Please provide the filled out Notice of the Public Hearing form/template. 10. Please provide proof of notice, such as Publisher's Affidavit from the newspaper, and Certificate of Mailing receipts from the Post Office, once you receive them. 11. Please submit a site plan that shows the ground sign location, in addition to the building location,and their setbacks. 12. Please respond in writing to my follow-up preliminary review comments issued by email on 10-08-2018. 13. Please make sure to list out the true practical difficulties and hardships associated with having to comply with the zoning ordinance. 14.
