AGREEMENT FOR PROFESSIONAL SERVICES
THIS AMENDMENT TO THE AGREEMENT FOR PROFESSIONAL SERVICES ("Agreement=') entered into
by and between the City of Carmel and United Consulting Engineers, Inc. (the "Professional"), as City Contract dated
June 1, 2016 shall amend the terms of the Agreement by adding the additional services to be provided by
Professional consistent with the Scope of Work attached hereto and incorporated herein as Exhibit "A". The terms
and conditions of the Agreement shall not otherwise be affected by this Additional Services Amendment and shall
remain in full force and effect.

RE: Professional Service Fees: Trail Design
Spring Mill Road Trail from 106th Street to 1-465
Dear Mr. Kashman:
The following Fee Proposal for the Spring Mill Road Trail from 106th Street to 1-465 has
been prepared in accordance with the email provided by Dave Richter on December 6,
2018.
SCOPE OF SERVICES
SURVEY
The topographic survey will follow all the requirements of the "Indiana Department of
transportation Design Manual, Part III — Location Surveys (June 15, 1998), and all the
requirements of Title 865 IAC 1-12 et sequential. The limits of the survey will be as follows:
1) Spring Mill Road (south): Beginning 25 feet south of the north end of the Spring
Mill Road bridge over 1-465 and extending 625 feet north to a point 125 feet north
of the center of the roundabout at Spring Mill Road and Illinois Street. The
reference alignment shall be established on the section line. The coverage
width shall be from the crown of the road of Spring Mill Road to 100 feet west of
the crown of the road.
2) Spring Mill Road (north): Beginning at a point 75 feet north of the center of the
roundabout at Spring Mill Road and Illinois Street and extending 2,450 feet north to
a point 75 feet north of the centerline of Millridge Drive. The reference alignment
shall be established on the section line. The coverage width shall be from the
crown of the road of Spring Mill Road to 75 feet east of the crown of the road.
3) Sufficient information will be gathered by use of a data collector to provide complete
topography and equivalent cross-sections at 50 foot intervals along all referenced
alignments.
TOTAL SURVEY LENGTH:
Total Survey Length = 3,075 feet (0.58 miles)

GEOTECHNICAL INVESTIGATION
UNITED CONSULTING shall make or cause to be made a geotechnical investigation
including the necessary borings, sub -surface exploration and the analysis thereof in
accordance with the INDOT Geotechnical Manual. A copy of the document is on file
with the Indiana Department of Transportation and it is incorporated herein by reference
and is made a past hereof.
WATERS OF THE U.S. REPORT
UNITED CONSULTING shall conduct a wetland determination at the project site which
shall include examination and documentation of the existing conditions, include soil
characteristics, drainage ditches, hydrology, vegetation, and general topography.
REGULATORY PERMITS
UNITED shall coordinate, apply for, and track the status of all applicable project permits
until received. Regulatory permits for this project include the following: IDEM Rule 5,
IDEM Section 401, and USACE Section 404.
TRAIL DESIGN PLANS
UNITED CONSULTING shall prepare plans, specifications and estimate of cost, which shall
be in accordance with the accepted standards for such work and in accordance with the
following documents in effect at the time the plans are submitted: Indiana Design Manual,
American Association of State Highway and Transportation Officials " A Policy on Geometric
Design of Highways and Streets", Guide for the Development of Bicycle Facilites, City of
Carmel Standard Drawings, Indiana Department of Transportation's Standard
Specifications, Road and Bridge Memoranda except as modified by supplemental
specifications and special provisions, if any, and shall be competed to the point required to
fulfill the requirements for Preliminary Plans, 95% Plans, and Final Contract Documents.
UTILITY COORDINATION
UNITED CONSULTING shall identify and notify all affected utilities and coordination
plans for utility relocations in accordance with 105 IAC 13.
UNITED CONSULTING shall prepare all required utility relocation agreements between
the City of Carmel and the utility.
BIDDING SERVICES
UNITED CONSULTING shall prepare the necessary contract Bid Documents including
Specifications, Standard Drawings and Bidding Forms.
UNITED CONSULTING shall advertise the project's Bidding in the local news
publications.
UNITED CONSULTING shall answer Contractor questions and issue Addenda, as
required.

UNITED CONSULTING shall review Contractor Bid Packages and prepare a written
recommendation to the City of Carmel for selecting the lowest responsive bidder and
provide the City of Carmel with certified Bid Tabulations.
TITLE SEARCH
UNITED CONSULTING shall provide a Title and Encumbrance Report and Guarantee
of Title Certification prepared by and INDOT pre -qualified abstractor. This shall be
completed for 3 parcels.
RIGHT-OF-WAY ENGINEERING
UNITED CONSULTING shall provide Right -of -Way Engineering services required to
secure the right-of-way required for the Spring Mill Road Trail project. These services
shall be in accordance with the INDOT Right -of -Way Engineering Manual. Right -of -Way
Engineering services shall be completed for 2 parcels.
RIGHT-OF-WAY STAKING
UNITED CONSULTING shall provide in the field stake -out locating the new right-of-way
line for the partial takings included in the parcels. The stake -out shall be made using
wooden hubs located at appropriate property lines, changes in bearing and every 100
feet on station. This will be performed as required as appraisers, buyers, and property
owners request field visualization of proposed acquisitions.
RIGHT-OF-WAY ACQUISITION SERVICES
These services will be provided by others.
SCHEDULE
No work under this Agreement shall be performed by UNITED CONSULTING until a
written notice to proceed is provided by the City of Carmel.
All work by UNITED CONSULTING under this Agreement shall be completed and
delivered to the City of Carmel for review and approval, if required, within the time
periods shown in the following schedule:
1) Field Survey and Processing shall be completed 45 calendar days after receipt
of the Agreement Commencement Date.
2) Completed geotechnical investigation report within 45 calendar days after
approval of Preliminary Plans.
3) Waters of the U.S. Report shall be completed within 45 calendar days of the
Agreement Commencement Date.
4) Regulatory Permit applications shall be submitted within 21 calendar days after
approval of Preliminary Plans.

a. Preliminary Plans within 90 calendar days after the completion of the
design survey.
b. 95% Plans within 90 calendar days after approval of Preliminary Plans.
c. Final contract documents within 60 days after approval of 95% plans.
6) Utility Coordination
a. Request for Utility Identification within 14 calendar days after the
Agreement Commencement Date.
b. Request for Utility Verification within 28 calendar days after completion
of the design survey.
c. Request for Utility Conflict Review within 14 calendar days after approval
of the Preliminary Plans.
d. Request for Utility Work Plans within 7 calendar days after approval of
95% plans.
7) Bidding Services
a. Bid Advertisement shall be completed within 21 calendar days after
completion of the Bid Documents.
b. Bid Tabulations and a Recommendation to Award shall be completed
and delivered to the City of Carmel within 3 calendar days after the Bid
Opening by the City of Carmel.
8) Title and Encumbrance Reports (Title Search) shall be completed within 21
calendar days from approval of Preliminary Plans.
9) All right-of-way engineering documents shall be completed within 21 calendar
days from receipt of Title and Encumbrance Reports.
10) Right -of -Way Staking shall be completed within 10 calendar days from City of
Carmel authorization.
COMPENSATION
UNITED CONSULTING shall receive as payment for the services performed under this
Agreement the total fee, not to exceed $226,700 unless a modification of the Agreement
is approved in writing by the City of Carmel.
UNITED CONSULTING will be paid Lump Sum for the services performed under this
Agreement as follows:

FUTURE PEDESTRIAN CONNECTION OVER 1-465
The pedestrian connectivity project for Springmill Road in Carmel, IN will need to extend
south of the roundabout with Illinois Street across the INDOT bridge crossing 1-465 (1465-
131-07719) immediately west of the US31 interchange. The existing bridge is two span
structure, 300 feet long, with a pier in the median between eastbound and westbound 1-465
traffic. There are two likely alternatives for making this crossing for pedestrians, Alternate
No. 1 would be to widen the existing bridge and Alternate No. 2 would be construction of a
stand-alone pedestrian structure to the west of the existing roadway bridge. A pedestrian
tunnel was ruled out immediately due to anticipated cost, future maintenance and safety.
Alternative No. 1 will involve widening of the existing substructure and superstructure in order
to add a pedestrian path to the west side of the bridge. At least one additional line of steel
beams will be required to complete the widening.
Alternative No. 2 will be construction of a separate bridge, most likely a prefabricated steel
truss, on new foundations immediately to the west of the existing roadway bridge.
Based on cost review of previous pedestrian bridges and bridge widenings, it is anticipated
that Alternative No. 1 (Widening of the existing bridge) will result in less cost, reduced future
maintenance, less impact to surrounding properties and provide a cleaner aesthetic for the
site.
UNITED reviewed the recently released INDOT RFP 1812-03 Project Development Services
— Road — Added Lanes on 1-465 from W. 86Th Street to US 31 and Interchange Modification
at I-465 and 1-865 in Marion, Boone, and Hamilton Cos. to determine the proposed work
required for the referenced structure. Currently, the proposal recommends a bridge deck
overlay only.
This proposal does not include fee or scope for any work on the Spring Mill Road bridge
over 1-465.
