n -applicable response or in the case of answering "no"to any of the above statements, have attached a letter and
er documentation stating the reason for not complying with the above said requirements. Submitting plans along
this stamped and signed document obligates me, the petitioner, to agree to the following requests by the
ailment of Engineering hereafter referred to as the reviewer. The petitioner will nobly the reviewer in the event of
zhanges to the submitted plans and is required to submit revised drawings for approval that highlight those
