AMENDMENT TO THE CITY OF CARMEL
EMPLOYEE HEALTH BENEFIT PLAN
WHEREAS, the City of Carmel ("Plan Sponsor"), by action of its governing body, adopted the City of
Carmel Employee Health Benefit Plan (the "Plan") effective February 1, 1992, and subsequently modified
the Plan by a full restatement effective January 1, 2019; and
WHEREAS, authority to amend the Plan is granted therein; and
WHEREAS, Plan Sponsor wishes to amend the restated Plan.
NOW, THEREFORE, effective January 1, 2019, the Plan is amended as follows.
Under ELIGIBILITY PROVISIONS, TERMINATION, the section on retiree coverage is amended to read
as follows:
CONTINUATION OF COVERAGE FOR RETIREES AND EARLY RETIREES
An employee who qualifies as a retiree or an early retiree (hereinafter jointly referred to as "retiree") who is
not eligible for Medicare may elect to continue coverage for himself and his Plan-eligible dependents if he
notifies the Plan Administrator of his intent, in writing, within thirty (30) days after his retirement date. An
employee will have the opportunity to elect a plan change at the time he requests retiree coverage.
Dependents and coverages may not be added or deleted except as allowed elsewhere under the Plan's
eligibility rules.
A retiree with at least 20 years of active service to the City who has terminated coverage through the Plan,
and who is not yet eligible for Medicare, may re-enroll in the Plan if he has maintained continuous coverage
through another employer-sponsored health insurance program or an individual ACA-compliant plan
offered by a carrier licensed by the State of Indiana (or, if the retiree has moved out of Indiana, the
retiree's state of residence) Eligible dependents who are going to re-enroll must meet the same criteria.
An eligible retiree can rejoin the plan in the following circumstances:
1. During an initial enrollment period, with coverage effective April 1, 2019;
2. Any subsequent year during the annual open enrollment period; or
3. Following a change in family status, provided application is made within 30 days of the change.
In all other respects the Plan remains unchanged.
