You are directed to make the following changes in this Contract: 4 The purpose of this change order is to address changes to the contract. Work covered in this change order includes the following: Sawcut for Temporary HMA Widening During removal of the existing median shoulder along Keystone Parkway,the contractor discovered inadequate pavement thickness remaining at the edge of the removal.The solution was to cut along the edgeline,farther into the mainline pavement,in search of sufficient thickness.Removing this additional strip of existing pavement exposed sufficient thickness of pavement to remain in place.This required a total of 1498 LFT of saw cutting. Light Pole,Remove Along the west side of the Penske parcel,twelve light poles remained within the new right of way limits.It was not in the contract for these light poles to be removed.In order for construction work to proceed,the contractor could not wait for the parcel owner to remove the light poles themselves.James H.Drew provided a price to remove these poles and move them to a storage location on the property.Three additional light poles in two other parcels,one at the Burger King parcel and two at the AZR parcel,also needed to be removed.James H.Drew agreed to perform the work at the same price they quoted for removal of the Penske light poles.The total number of light poles removed is 15 EACH. Temporary Construction Panel Sign—Relocate James H.Drew had planned to remove an existing box truss sign structure on SB Keystone Parkway at I-465.City of Cannel requested that the temporary construction panel sign that had been ground mounted on SB Keystone be relocated to the existing box truss for better visibility during early construction phases.Due to a miscommunication,JH Drew had a crane scheduled to remove the box truss.This crane was used to relocate the temporary panel sign while on site. Renascent Additional MOB Due to delays in Right of Way acquisition,Renascent was unable to complete demolition of all four parcels with one mobilization.These delays resulted in two additional mobilizations for the contractor,one for the Burger King/Chase Bank parcel and one for the Marathon parcel. Maintaining Traffic-Priority Way RAB With the delays to the start of construction on the Keystone Parkway Corridor contract,the Priority Way Intersection contract was completed first.This delay resulted in phasing issues between the two projects.Additional MOT was required on the newly constructed Priority Way Roundabout in order for the traveling public to safely transition from the roundabout into the MOT setup currently in place along 96th Street between Priority Way and Keystone Parkway. Houses and Buildings,Remove,Parcel No.10-Additional The demolition plan for Parcel 10 showed leaving the block walls and brick building in the SE corner of the parcel.This plan also indicated leaving a 40 FT strip of asphalt pavement along the east side and a 25 FT strip of asphalt pavement along the south side of the parcel.During the permit process,it was questioned as to why it was necessary to leave these areas intact.Upon further investigation with the Design Engineer and City of Cannel,it was deemed that leaving these areas was not necessary. Therefore,Renascent was instructed to demo these areas as well.This was not in their original bid,so a price was submitted and approved for this additional work. 16 IN Gate Valve It was in the original scope of work for the TRICO sanitary sewer force main in the NW corner of the 96th Street and Keystone Parkway intersection to be relocated by the Contractor.This work was originally planned to occur in Phase 3, but it was in conflict with a storm sewer structure that was to be installed in Phase 2.It was also discovered that the location of the tie-in would fall under the ramp that was to be built in Phase 2.Therefore,it was concluded that part of Version 2016-08-31 AR-6 the relocation had to occur in Phase 2.This required some re-design of the force main relocation process,which resulted in the need for a new 16 IN Gate Valve in order to phase the relocation. Box Culvert Access Points Castings for Storm STR 810A and a box culvert access point just to the SE had to be adjusted approximately 2.5-3.0 FT in order to match the proposed grade in the area.This adjustment was not accounted for in the re-design of the box culvert structure.Additional pre-cast sections as well as labor and equipment make up the additional cost of this work. Overhead Sign Structure,Monotube—Additional Cost Due to changes in the panel signs covered in Construction Change No.9,the height of sign structure GDO-3 uprights were adjusted to compensate.Also,the span of sign structure GDO-3 was adjusted to achieve clear zone compliance. This cost change is material costs passed on from the supplier plus appropriate markups. Construction Engineering—Additional SurveyTech has incurred additional costs for additional staking due to Construction Changes,utility relocations,and property acquisition.Additional staking on the SW corner of the project for IPL relocation was also recently required. This item covers that additional costs of this work. Connect Penske Lot Drain to Box Culvert(Time and Materials) The construction plans indicated connection of Penske Chevrolet storm drains to new ditch structures.The storm drain in question(west of entrance)had no such direction in the construction drawings.This Penske storm drain could not be connected to the new ditch structure(STR 496)due to elevation restrictions.After discussion at a Progress Meeting, City of Cannel agreed to proceed with direct connection to the box culvert.The attached proposal covers the T&M work necessary to make this connection. Sanitary ARV Structure—Adjust to Grade The casting for the Sanitary ARV structure(installed by others)at MSE Wall 5 had to be lowered in order to match the grade of the moment slab in this area.The attached proposal covers the time and materials required to complete this work.
