The undersigned affirms that the information included in this Application is correct and acknowledges a final
inspection is required and does hereby authorize City Staff permission to make site inspections as may be
necessary. Further, if the Application is approved, the undersigned will comply with all the requirements of the
City of Carmel Unified Development Ordinance and certifies that the measures proposed to mitigate adverse
impact will be completed in a timely fashion. The contractor/property owner is responsible for properly locating
the property lines; locating pins or surveying property. Also, the contractor/property owner is responsible for
contacting Indiana811 to locate underground utilities, and for scheduling the required final inspection.
Please contact the Building and Code Services office at 317-571-2444 to schedule your final inspection.
If an inspection is not called in after completion, a late fee of $596.00 may be applied.

I, the undersigned Registered Land Surveyor, hereby certify to the best) of my information, knowledge and belief, that
the included plat correctly represents a retracement survey performed under my direction during May of 2018 of the
following described real estate:
Lots 97-100 & 110-111, in Westmont, Section 2A, a subdivision in Hamilton County, Indiana, the plat of which is
recorded as Instrument Number 2014032885 in Plat Cabinet 5, Slide 226 in the Office of the Recorder for Hamilton
County, Indiana.
THEORY OF LOCATION
The purpose of this survey is to establish the rear lines and rear corners of said Lots 97-100 & 110-111. The
improvements on said lot were not located unless they lie within five (5) feet of the lot or property line or were
specifically requested to be located.
The record plat of Westmont, Section 2A does illustrate on the face of the document that original subdivision
monumentation was established to control the location of the within lot corners. Aluminum welds were found
throughout the subdivision within the centerline of the streets as depicted on said plat. These monuments had only
negligible discrepancies in the horizontal angle and horizontal distance from the record plat geometry and were held.
The monument designated as '1"was held as the basis of this survey and the bearing between it and the monument
designated as "2"was held as the basis of bearing and checked with monument designated as "3". Bearings and
distances shown on the within plat are both measured and record, unless otherwise noted.
SURVEYOR'S REPORT
In accordance with Title 865, Article 1, Chapter 12, Section 1 through 30 of the Indiana Administrative Code, the
following observations and opinions are submitted regarding the various uncertainties in the locations of the lines and
corners established on this survey as a result of:
A. Availability and condition of reference monuments:
Aluminum welds (marking the center points at surface no Firm Number stamped on weld) were found within the center
of Lasino Run and Danube Drive as shown on the plat of survey.
These monuments were set in place by the platting surveyor and are original or section corners set and maintained by
the County Surveyor. Said monuments were found flush with the surface and have been in place for approximately four
years.
Monuments set per this survey were set flush with the adjacent surface unless otherwise noted.
B. Occupation or possession lines:
The possession along the West lines of the subject real estate are to the street improvements of Lasino Run and
Danube Drive. The possession along the North, South and East lines of the subject real estate are indistinguishable.
C. Clarity or ambiguity of the record descriptions used and/or adjoiner's descriptions:
Unless discussed above, there are no discrepancies between the record land descriptions of the subject real estate and
the adjoiners. Similarly, the lines of the subject real estate ore contiguous with the adjoiners' lines.
D. The relative positional accuracy of the measurements:
The relative positional accuracy (due to random errors in measurement) of the corners of the subject real estate
established on this survey is within the specifications for an Suburban Survey (0.13 feet plus 100 parts per million) as
defined In IAC 865.
E. Positional uncertainty.
As a result of the above observations, it is my opinion that the uncertainties in the location of the lines and corners
established on this survey are as follows:
Due to Inconsistencies in lines of occupation: EQUAL to the differences shown between the property line evidence found
and the property lines established by this survey.
Due to variances in reference monuments: Negligible
Due to D?screponcies in the record descriptions: None observed.

the adjoiners. Similarly, the lines of the subject real estate are contiguous with the adjoiners' lines.
