November 17, 2021 Mr. Daren Mindham Principal Urban Forester City of Carmel One Civic Square Carmel, Indiana 46032 RE: PZ19060019 DP ADLS Franciscan Orthopedic Center of Excellence Daren: Due to availability, certain species originally specified for the project noted above are not available through nurseries and substitution are required. Those substitutions are as follows: ORIGINALLY SPECIFIED SPECIES PROPOSED SUBSTITUTIONS TREES (1) Que-r Quercus rubra Red Oak (1) Nys-s Nyssa sylvatica Sour Gum (16) Pic-g Picea glauca ‘Globosa’ Dwarf Globe Spruce (16) Pic-g Picea glauca White Spruce SHRUBS (73) Die-l Diervilla loniceri ‘Kodiac Orange’ (73) Fot-g Fothergilla gardenia Dwarf Fothergilla Please let me know if there are any concerns with these substitutions. We consider these substitutions to be equal in both plant quality and quantity. Nyssa sylvatica was already approved during the original plan submittal process and adding one more does not take the plant quantity beyond the 20% threshold. There are other oaks provided on the project, as well. Thank you for your time concerning this matter. Alyssa P. At that time, there were four alternates shown on the plans which included the following: • Alternate Exterior Sports Training including the synthetic turf field, batting cage, tennis and half-basketball court (located west of the Medical Office Building) • Alternate Dining Area north of hospital • Valet Parking north of the parking garage • Alternate Fitness Trail/Path on the east side of the site The project phasing has resulted in the Exterior Sports Training elements to be incorporated into this phase of construction and included with the Medical Office Building, Hospital, and Parking Garage Construction. The Valet Parking will not be constructed at this time. This will be constructed in the future. Additionally, the Fitness Trail/Path will not be constructed at this time. The layout of this trail may be revisited as the land north of this phase of construction is developed so that a more comprehensive trail/path system may be established. The accompanying plans show the revised documents reflecting the current phase of construction. We appreciate your time and consideration. cc: File, Alex Jordan (Carmel Engineering) 3939 Priority Way South Drive,Suite 200 r I pe Indianapolis,IN 46240 AUG 3 % . 317 77 K FAX: 317.706 6451 p www.cripe.biz — J Date: 8/12/19 Cripe Job #: 180235-20000 To: City of Carmel RE: Franciscan Orthopedic Center of Plan Commission Excellence 1 Civic Sq., Carmel, IN 46032 Docket# 19060019 DP/ADLS Attention : Joe Shestak- (317) 571-2419 We are sending you the following items via: Courier Shop Drawings Prints Plans Samples Specifications Copy of Letter Change Order Attached Report X Other Copies Date Description 10 8/12/19 Plan Commission Packet 2 8/12/19 Digital Copy Plan Commission Packet 1 8/12/19 Check THESE ARE TRANSMITTED as checked below: For Approval Submit Approved as Submitted Copies for Approval X For Your Use Return Approved as Noted Copies for Distribution x As Requested Not Approved Reviewed for Compliance Signature Requested Resubmit Corrected Prints For Review and Comment Remarks: c: Scanned File Signed: Jennifer Lasch,jlasch@cripe.biz Transmittal Only: Notice: The drawing/computer files listed above are the property of Cripe Architects + Engineers and are transmitted for reproduction use only. The project falls in the incorporated area and MS4 jurisdiction of the City of Carmel. 2. The proposed project DOES NOT fall in a Carmel Wellhead Protection Zone. 3. The proposed project falls in the Williams Creels Watershed, 4. Please direct all stormwater questions outside the parameters of the regulated drain to the City of Carmel Engineering Department. 5. Please provide an outlet request form for the direct discharge to the Meridian Suburban Regulated Drain, 6. This project will require a utility crossing permit for the construction of the Sanitary Sewer System across the Meridian Suburban Regulated Drain. Please submit a crossing permit request with payment to our office. 7. The scope of work for this project includes the reduction of a 75 foot easement to 30 feet. This will require a nonenforcement agreement to be approved by the Hamilton county Drainage board. 8. The scope of work for this project includes the reconstruction of a portion of the Meridian Suburban Drain. This will require the following items: Petition for regulated drain relocation, Engineer's estimate and all associated bonds for 120% of the construction cost. 9.
