The applicant seeks site plan and design approval for a 4 story, 236,231 sq. ft. orthopedic hospital, surgery center,
medical office building, and 664 space parking garage on 10.8 acres. The site is located at approximately 10800 Illinois
Street. The site is zoned MC/Meridian Corridor. Filed by Marty Rosenberg of Methodist Sports.

Petitioner, please note the following:
1. This item will be on the Wednesday, July 17, 2019 meeting agenda of the Technical Advisory Committee (TAC),
which begins at 9:00 AM. Petitioner, please have a representative attend this meeting. For your reference, here
is a link to the current TAC members list.
2. Mailed and Published Public Notice needs to occur no later than Tuesday, July 30, 2019. Due to the site’s
location, published notice (legal ad) is required within The Current in Carmel.
a. Please contact The Current at least 11 days prior to the public notice deadline, in order for them to
publish your legal ad on time (317-489-4444 or legals@youarecurrent.com).
b. The placement of a notice of public hearing sign on the property is required.
c. First class mailings (with Certificate of Mailing) to all adjoiners is required. Contact the Hamilton County
Transfer and Mapping Office for the list of adjoining properties. Please note that the list of adjoining
properties must not be obtained sooner than 45 days prior (Sat., July 6) to the public hearing.

d. Make sure the public notice meeting time to the newspaper, public notice sign, and letters to neighbors

reads 6:00 PM and that the location reads Carmel City Hall Council Chambers, 1 Civic Square, 2 Floor,
Carmel, IN 46032.
3. The Filing Fee and 9 Informational Packets must be delivered to Plan Commission Secretary Joe Shestak no later
than Noon, Friday, August 9, 2019. Checks can be made out to The City of Carmel or we can accept credit card
by phone for a small fee.
4. For the information packets, please print them at 8.5 x 11 size and use tabs to separate the sections. 11 x 17
sheets can also be used, but they must be folded down to 8.5 x 11. If you choose to bind your packet, please do
not use a 3 ring binder. Spiral, wire, or stapling are acceptable methods. Please include the following in your
information packets, if applicable to your project:
a. Cover Page (including project name, docket number, Plan Commission meeting, meeting date, contact
info)
b. Brief Description of the project
c. Location Map
d. Site Plan and a Site plan laid over an aerial
e. Engineered site plan, including bike rack locations
f. Architectural renderings of building and garage
g. Architectural elevations of building and garage with materials called out
h. Materials sheet (photos of material samples)
i. Dumpster enclosure plan
j. Floor plans
k. Lighting information – site, garage, and building (including cut sheets for all fixtures)
l. Lighting photometric plan
m. Landscaping plan
n. Signage plan
o. Any other supporting information or drawings
p. Findings of Fact Forms (page 9 of the DP application)
5. Proof of Notice (Certificates of Mailing, newspaper affidavit, etc.) needs to be received by this Department no
later than Noon, Thursday, August 15, 2019. Failure to submit Proof of Notice by this time will result in the
tabling of the petition.
6. This item will appear on the Tuesday, August 20, 2019 meeting agenda of the Plan Commission (under “Public
Hearings”), which begins at 6 PM in the Council Chambers of City Hall. (A representative must be present.)
7. This item will also appear on the Tuesday, September 3, 2019 meeting agenda of a Plan Commission Sub-
Committee, which begins at 6 PM in the Caucus Rooms of City Hall. (A representative must be present.)

9. Review comments from the Planning/Zoning Dept. will be issued soon via ProjectDox.
