1. SHOULD THERE BE ANY QUESTIONS REGARDING THE CONTR4CT DOCUMENTS, EXISTING
CONDITIONS ANDIOR DESIGN INTENT, THE CONTRACTOR(S) SHALL BE RESPONSIBLE FOR
OBTAINING CLARIFICATION FROM THE ARCHITECT PRIOR TO BID SUBMITTAL AND PROCEEDWG
WITH ANY WORK OR RELATED WORK IN QUESTION. THE ARCHITECT SHALL HAVE THE SOLE
AUTHORITY AND RESPONSIBILIN TO DETERMINE THE INTENT OF THE CONTRACT DOCUMENTS SO
FAR AS THE INTENT IS REASONABLE INERRABLE FROM THE CONTRACT DOCUMENTS.
2. ALL CONTRACTOR(S) OR SUPPLIER�S) REQUESTS FOR SUBSTITUTIONS OF SPECIFIED ITEMS
SHALL BE SUBMITTED IN WRITING, ACCOMPANIED BY THE ALTERNATNE PRODUCT INFORMATION,
TO THE ARCHITECT. ALL SUBSTITUTIONS SHALL BE SUBMITTED TO THE ARCHITECT NOT LATER
THAN FNE (5) WORKING DAYS PRIOR TO BID OPENING DATE. SUBSTITUTIONS SHALL BE
CONSIDERED ONLY IF THEY DO NOT SACRIFICE QUALITY, APPEARANCE AND FUNCTION. UNDER
NO CIRCUMSTANCES WILL OWNER BE REQUIRED TO PROVE THAT A PRODUCT PROPOSED FOR
SUBSTITUTION IS OR IS NOT OF EQUAL QUALITY TO THE PRODUCT SPECIFIED.
3. THE CONTRACTOR(S) SHALL BE RESPONSIBLE FOR FAMILIARIZING THEMSELVES WITH THE
PROJECT SCOPE OF WORK, BUILDING STANDARDS, SCHEDULE AND DEADLINES. THE
CONTRACTOR(S) SHALL FURTHER BE RESPONSIBLE FOR ADVISING OWNER OF ALL LONG LEAD
TIME ITEMS AFFECTING THE PROJECT SCHEDULE AND SHALL, UPON REQUEST FROM OWNER,
SUBMIT ORDER AND DELIVERY DATE CONFIRMATIONS FOR SUCH LONG LEAD TIMES.
4. THE CONTRACTOR(S) SHALL COORDINATE WITH OWNER ACCESS TO THE SPACE AND THE
ONGOING CONSTRUCTION WORK. DISRUPTIVE OR POTENTIALLY HAZARDOUS CONSTRUCTION
ACTIVITIES SHALL BE COORDINATED TO OCCUR AFTER NORMAL OFFICE HOURS, SO AS TO
MINIMIZE DISTURBANCE TO THE CURRENT BUILDING OCWPANTS.
5. THE CONTRqCTOR(S) IS RESPONSIBLE FOR FIELD VERIFYING ALL CONDITIONS AND DIMENSIONS
PRIOR TO THE START OF WORK REPORT ANY DISCREPANCIES TO THE ARCHITECT IMMEDIATELY.
DO NOT SCALE DIMENSIONS ON THE DRAWINGS.
DESIGN COMPLIANCE REVIEW
6. THESE CONTRACT DOCUMENTS ARE INTENDED TO DESCRIBE ONLY THE SCOPE AND
APPFARANCE OF THE REAL PROPERTY IMPROVEMENTS,INCLUDING THE PERFORMANCE AND
LEVEL OF QUALITY EXPECTED OF ITS COMPONENTS. THESE CONTRACT DOCUMENTS DO NOT
ATTEMPT TO INSTRUCT THE CONTRACTOR(S) IN THE DETAILS OF HIS TRADE. THEY ARE
PERFORMANCE SPECIFICATIONS IN THAT THEY REQUIRE THAT ALL MANUFACTURED ITEMS,
MATERIALS AND EQUIPMENT BE WSTALLED IN STRICT ACCORDANCE WITH THE MANUFACTURER'S
RECOMMENDED SPECIFICATIONS, EXCEPT IN THE CASE WHERE THE CONTR4CT DOCUMENTS
ARE MORE STRINGENT. ANY MISCELLANEOUS ITEMS OR MATERIALS NOT SPECIFICALLY NOTED,
BUT REQUIRED FOR PROPER INSTALLATION, SHALL BE FURNISHED AND INSTALLED BY THE
CONTRACTOR(S).
7. ALL WORK IS TO BE COMPLETED IN STRICT ACCORDANCE WITH ALL APPLICABLE CODES,
ORDINANCES, RULES, REGULATIONS AND STANDARDS, INCLUDING, BUT NOT LIMITED T0, THE
INDIANA BUIL�ING CODE, THE INDIANA FIRE CODE, THE INDIANA ELECTRICAL CODE, THE INDIANA
PLUMBING CODE, THE INDIANA MECHANICAL CODE, THE INDIANA ENERGY CONSERVATION CODE,
AND APPLICABLE TRADE STANDARDS. ALL APPLICABLE RULES AND REGULATIONS ARE TO BE
THE MOST CURRENT ADOPTED.
S. THE CONTR4CTOR(S) IS RESPONSIBLE FOR OBTAINING ALL LOCAL PERMITS REQUIRED FOR
COMPLETION OF THIS PROJECT.
9. CONTRACTOR SHALL PROVIDE ALL NECESSARY LABOR, MATERIALS, AND EQUIPMENT FOR THE
WORK IN THESE DOCUMENTS, UNLESS CLEARLY NOTED "NOT IN CONTRACT" (NIC).
10. ALL WORK IS TO BE COORDINATED BY THE GENERAL CONTRACTOR TO ASSURE ADEQUATE FIT,
FINISH, SYSTEM OPER4TION, AND FULL COMPLETION OF THE WORK, WCLUDING SERVICE
REQUIREMENTS OF OWNER'S FURNISHING, FIXTURES AND EQUIPMENT.
11. WHERE EXISTING WORK IS TO BE REMOVED OR ALTERED, RESTORE ADJACENT FINISH TO
ORIGINAL CONDITION. REFINISH ALL FR4MES, BASE AND TRIM WHERE CUTTING HAS EXPOSED
1. THE CONTRACTOR SHALL SUBMIT SHOP DRAWINGS OF CUSTOM FABRICATION OR MANUFACTURE AND PHYSICAL SAMPLES OF ALL FINISH MATERIALS SPECIFIED TO THE ARCHITECT FOR REVIEW AND
APPROVAL. BY SUBMITTING SAID SHOP DRAWINGS AND SAMPLES, THE CONTRACTOR ASSERTS THAT THE CONTRACTOR HAS REVIEWED THE SUBMITTALS AND FOUND THAT THEY ARE IN COMPLIANCE
WITH THE REQUIREMENTS OF THE CONTRACT DOCUMENTS, ANY DEVIATIONS FROM THE REQUIREMENTS OF THE CONTRACT DOCUMENTS MUST BE EXPLICITLY EXPRESSED ON THE TRANSMITTAL OF
THE SUBMITTAL.
2. ALL SUBMITTALS MUST BE DELNERED TO THE ARCHITECT WITHIN TWO WEEKS OF THE CONTRACTOR'S RECEIPT OF THE NOTICE TO PROCEED.
3. THE ARCHITECT WILL REVIEW THE SHOP DRAWWGS AND SAMPLES FOR CONFORMITY WITH THE MFORMATION CONTAWED IN THE CONSTRUCTION DOCUMENTS AND RETURN THE SUBMITTALS TO THE
CONTR4CTOR WITHIN TEN 10 WORKING DAYS OF RECEIPT EXCEPT AS MAY OTHERWISE BE PROVIDED FOR BY OWNER. ALL MATERIALS MUST BE ORDERED WITHIN FNE 5 WORKING DAYS OF THE

SUBMITTAL REVIEW. IF THERE IS A DELAY IN THE DELIVERY OF ANY MATERIALS WHICH PUTS THE OVERALL CONSTRUCTION CONTRACT IN JEOPARDY THE CONTRACTOR MUST ADVISE THE ARCHITECT
THEREOF WITHIN TEN (10) WORKING DAYS OF THE RECEIPT OF THE SUBMITTAL REVIEW. THE OWNER WILL NOT AGREE TO AN EXTENSION OF TIME FOR THE [PROJECT COMPLETION DUE TO MATERIAL
DELIVERY DELAYS IF THE ABOVE CONDITIONS ARE NOT MET.
3. REVIEWED SHOP DRAWINGS AND SCHEDULES PREPARED BY OTHERS SHALL NOT BE CONSTRUED TO BE PART OF THE CONTRACT DOCUMENTS. THE ARCHITECT ASSUMES NO RESPONSIBILITY FOR
DRAWINGS, SCHEDULES, ANDIOR SPEGFICATIONS FOR WORK ON THE PROJECT PREPARED OR ACCOMPLISHED BY OTHERS.
4. SUBMITTALS REQUIRED FOR THIS PROJECT INCLUDE:
CASEWORK
PLUMBING FIXTURES
LIGHT FIXTURES
ELECTRICAL DEVICES
NURSE CALL AND COMMUNICATIONS
DOORS,FRAMESANDHARDWARE
FINISH FLOORING AND BASE SAMPLES
WALLCOVERING AND PAINT SAMPLES
CERAMIC TILE SAMPLES
HVAC EQUIPMENT. GRILLES AND DIFFUSERS
ACCESSIBILITY GUIDELINES (NOT TO SCALE)
ANY UNFINISHED SURFACES. REPAIR ANY DAMAGE TO EXISTING WORK AND RESTORE TO
ORIGINAL CONDITION.
12. CONTRACTOR(S) SHALL KEEP WORK AREAS AND ADJACENT PUBLIC CORRIDORS AND FACILITIES
FREE OF DEBRIS. MATERIALS STORED ON SITE ARE THE RESPONSIBILIN OF THE CONTRACTOR
AND SHALL BE STORED IN A LOCATION APPROVED BY OWNER.
13. ALL FINISH CARPENTRY IS TO BE COMPLETED IN STRICT ACCORDANCE WITH THE AMERICAN
WOODWORKING INSTITUTE (AWI) STANDARDS, PREMIUM GRADE, UNLESS OTHERWISE
SPECIFICALLY NOTED.
14. DURING THE ENTIRE PERIOD OF CONSTRUCTION, ALL EXISTING EXITS, EXIT LIGHTING, FIRE
PROTECTIVE DEVICES AND ALARMS SHALL BE CONTINUOUSLY MAINTAWED.
15. ALL PLUMBWG ITEMS ANDIOR EQUIPMENT ITEMS ARE TO BE COORDINATED AND VERIFIED WITH
INTERIOR ELEVATIONS PRIOR TO ROUGH-IN. ANY DISCREPANCIES SHALL BE COORDINATED WITH
THE ARCHITECT.
16. THE CONTR4CTOR(S) SHALL COORDINATE WITH OWNER, OR THEIR AUTHOR2ED AGENT
(HEREAFTER JOINTLY REFERRED TO AS "OWNER"), TO DETERMINE ALL EXISTING MATERIALS TO
BE SALVAGED AND RETAINED BY OWNER. THE CONTRACTOR(S) SHALL REMOVE ANY
SALVAGEABLE MATERIALS FROM THE SPACE AND STORE IN LOCATIONS AS DIRECTED BY OWNER.
ALL OTHER MATERIAL$ RUBBISH AND DEBRIS SHALL BE REMOVED AND PROMPTLY DISPOSED OF
BY THE CONTRACTOR(S) AT AN OFFSITE LOCATION AND IN ACCORDANCE WITH ALL APPLICABLE
LAWS AND REGULATIONS. THE CONTRACTOR(S) SHALL TAKE ALL NECESSARY PRECAUTIONS TO
ASSURE THAT ALL PROPERTY IMPROVEMENTS (I.E. PARTITIONS, DOORS, FRAMES, CABINETS,
CEILING GRID, CEILING TILE, AND THE ELECTRICAL AND MECHANICAL DEVICES, ETC.) WHICH ARE
TO REMAIN,ARE PROTECTED FROM DAMAGE.ANY DAMAGED PROPERTYIMPROVEMENTS SHALL
BE REPAIRED OF REPLACED BY THE CONTRACTOR.
EXISTING CONDITIONS CHANGE ORDERS
1. EXISTING CONDITIONS SHOWN ON THESE DRAWINGS ARE BASED UPON DOCUMENTS MADE
AVAILABLE TO THE ARCHITECT BY OWNER. UNLESS OTHERWISE NOTED,AS-BUILT
ARCHITECTURAL CONDITIONS HAVE NOT BEEN FIELD VERIFIED AND MAY VARY FROM THOSE
SHOWN.
2. PRIOR TO SUBMITTWG THE CONSTRUCTION BID, THE CONTRACTOR SHALL BE RESPONSIBLE
FOR VISITING THE SITE TO FAMILIARIZE HISIHERSELF WITH THE EXISTING CONDITIONS OF THE
SITE. NO ADDITIONAL FUNDS WILL BE APPROVED FOR WORK OMITTED FROM THE
CONSTRUCTION BID DUE TO LACK OF VERIFICATION BY THE CONTR4CTOR EXCEPT FOR
WORK ASSOCIATED WITH "HIDDEN" CONDITIONS WHICH COULD NOT BE REASONABLY
ASCERTAINED PRIOR TO CONSTRUCTION.
3. IF THE CONTRACTOR BELIEVES THAT THERE ARE CERTAIN "HIDDEN" CONDITIONS THAT WILL
AFFECT THE CONSTRUCTION OF THE WORK AS DESCRIBED IN THESE CONTRACT
DOCUMENTS, CONTRACTOR SHALL NOTIFY THE ARCHITECT.
AS-BUILT DRAWINGS
1. DURING THE COURSE OF CONSTRUCTION THE CONTR4CTOR WITH KEEP TRACK OF ALL
DEVIATIONS FROM THE CONTRACT DOCUMENTS. AT THE CONCLUSION OF THE PROJECT, AND
BEFORE FINAL PAYMENT IS MADE, THE CONTRACTOR WILL DELNER TO THE ARCHITECT A
MARKED-UP SET OF DRAWINGS SHOWING ALL SAID DEVIATIONS.

17. THE CONTR4CTOR(S) SHALL COORDINATE WITH OWNER FOR DELIVERY TO THE SPACE OF ALL
BUILDING MATERIALS AND SHALL ARRANGE FOR USAGE OF THE FREIGHT ELEVATOR (OR
ANOTHER DESIGNATED ELEVATOR). IF DELIVERY OF BUILDWG MATERIALS INVOLVES THE USE OF
FACILITIES OTHER THAN THE AVAILABLE ELEVATORS, THE CONTRACTOR(S) SHALL COORDINATE
SUCH ACTNITIES IN A MANNER WHICH POSES NO HAZARD TO ANY PERSONNEL WITHIN OR
AROUND THE BUILDING OR BUILDING SITE.
18. LOCATION OF DUMPSTER MUST BE COORDINATED WITH AND APPROVED BY OWNER.
19. COORDINATE WITH OWNER AND FACILITY STAFF PROJECT PHASING TO MINIMIZE DISRUPTION OF
EXISTING FACILIN SERVICES.
20. THE RENOVATED AREAS SHALL BE ISOLATED FROM THE OCCUPIED AREAS DURING ALL PHASES
OF CONSTRUCTION USING AIRTIGHT BARRIERS, AND EXHAUST AIR FLOW SHALL BE SUFFICIENT
TO MAINTAIN NEGATIVE AIR PRESSURE IN THE CONSTRUCTION ZONE.
21. THE CONTRACTOR(5) SHALL PRESERVE ALL PRINTED INSTRUCTIONS AND WARR4NTIES THAT
ARE PROVIDED WITH EQUIPMENT OR MATERIALS USED AND DELNER SUCH PRINTED MATERIAL
TO THE OWNER IN THE FORM OF AN "OPER4TIONS AND MAINTENANCE" LOOSE LEAF BINDER
PRIOR TO FINAL COMPLETION. THE CONTRACTOR(S) SHALL INSTRUCT OWNER IN THE PROPER
USE OF THE EQUIPMENT FURNISHED.
22. GENERAL CONTRACTOR IS TO PROVIDE A THOROUGH CONSTRUCTION CLEANING OF ALL
SURFACES OF THE WORK AREA PRIOR TO SUBSTANTIAL COMPLETION.
23. ALL WORK SHALL BE WARRANTED SATISFACTORY, IN MATERIALS AND WORKMANSHIP, FOR ONE
(1) YEAR, OR FOR THE PERIOD OF WARRANTY CUSTOMARY OR STIPULATED FOR THE TRADE,
CR4FT OR PRODUCT, WHICHEVER IS LONGER. THEREFORE, ONLY COMPETENT MECHANICS
CAPABLE OF PRODUCING GOOD WORKMANSHIP CUSTOMARY TO THE TRADE SHOULD BE USED.
COMMENCING WORK BY A CONTRACTOR(S) OR SUBCONTRACTOR(S) CONSTITUTES ACCEPTANCE
OF THE CONDITIONS AND SURFACES CONCERNED. IF ANY SUCH CONDITIONS ARE
UNACCEPTABLE, THE GENERAL CONTRACTOR SHOULD BE NOTIFIED AT ONCE AND NO WORK
DONE UNTIL CONDITIONS ARE ACCEPTABLE.
24. ALL DIMENSIONS ARE SHOWN ON THE DRAWINGS. ANY DIMENSIONS NOT SHOWN OR DEEMED
QUESTIONABLE ARE TO BE VERIFIED WITH THE DESIGNER. DO NOT SCALE DIMENSIONS ON THE
DRAWINGS. ALL DIMENSIONS ARE TO THE FACE OF GYPSUM BOARD, MASONRYICONCRETE
WALLS, OR CENTERLINE OF COLUMNS; UNLESS NOTED OTHERWISE.
25. SUBMIT THE REQUIRED NUMBER OF SUBMITTALS, FOR DESIGNER REVIEW AND APPROVAL, PWS
ONE EXTRA COPY FOR DESIGNER'S RECORDS, PRIOR TO MATERIAL ORDER PLACEMENT AND
INSTALLATION. DESIGNER WILL NOT BE RESPONSIBLE FOR MATERIALS THAT ARE ORDERED
ANDIOR WSTALLED WITHOUT DESIGNER'S PRIOR SUBMITTAL REVIEW AND APPROVAL.
CONTRACTOR IS TO SIGN EACH SHOP DRAWING SUBMITTAL INDICATING IT HAS BEEN REVIEWED
AND THAT THE CONTRACTOR COORDINATION HAS BEEN PERFORMED.
26.MAKE ALL DEMOLITION CLEAN AND COMPLETE IN A MANNER SUITABLE TO ACCEPT NEW
SPEGFIED FINISHESIMATERIALS. REFER TO PL4NS, SPECIFICATIONS AND DETAILS FOR SPECIFIC
INFORMATION REGARDING APPLICATION OF NEW MATERIALS.
27. THE GENERAL CONTRACTOR SHALL PROVIDE TEMPORARY DUST PARTITIONS ANDIOR
TEMPORARY DOORS AS DEEMED NECESSARY BY OWNER. THEY SHALL BE MAINTAINED AND
REPAIRED IMMEDIATELY IF ANY DAMAGE OCCURS.
28. PATCH AND REPAIR ALL FLOOR AND WALL HOLES, CRACKS, IMPERFECTIONS, ETC. TO ORIGINAL
STATE PRIOR TO MATERIAL INSTALLATION. APPLY FULL FLOOR CRACK SEALANT MEMBRANE TO
ENSURE CONCRETE SLAB CRACKING IS CONTAWED PRIOR TO FLOOR TILE WSTALLATION.
29. PREPARE ALL EXISTING WALLS AND FLOORS SMOOTH TO RECENE NEW FINISHES; PATCH AND
REPAIR AS REQUIRED. REFER TO PLANS AND MANUFACTURER SPECIFICATIONS FOR SPECIFIC
INFORMATION REGARDING APPLICATION REQUIREMENTS OF NEW FINISH MATERIALS.
