2.EXISTING UNDERGROUND UTILITIES SUCH AS WATER, SANITARY, STORM,
GAS, ELECTRIC, ETC. AS SHOWN ON THE PLANS HAVE BEEN DETERMINED
FROM THE MOST ACCURATE INFORMATION AVAILABLE. ALL INFORMATION
MUST BE VERIFIED IN THE FIELD BY THE CONTRACTOR PRIOR TO THE START
OF CONSTRUCTION. ANY DISCREPANCIES SHALL BE IMMEDIATELY
REPORTED TO THE ENGINEER. THE INFORMATION IS GIVEN FOR THE
CONVENIENCE OF THE CONTRACTOR. THE OWNER AND ENGINEER DO NOT
ASSUME RESPONSIBILITY OF THE ACCURACY OF THE LOCATIONS SHOWN. IT
SHALL BE THE CONTRACTOR'S RESPONSIBILITY TO CONTACT ALL UTILITY
COMPANIES AND THEIR FACILITIES SHALL BE LOCATED PRIOR TO ANY
WORK.
3.BEFORE STARTING ANY CONSTRUCTION, THE CONTRACTOR SHALL EXPOSE
ANY UTILITIES OR OTHER OBSTRUCTIONS TO BE CROSSED BY THE PROPOSED
SEWER AND SHALL NOTIFY THE ENGINEER IMMEDIATELY IF THERE ARE
ANY CONFLICTS WITH THE PROPOSED SEWER GRADE.
4.CALL J.U.L.I.E. (1-800-892-0123) PRIOR TO ANY AND ALL DIGGING.
5.THE CONTRACTOR SHALL BE RESPONSIBLE TO CONNECT OR RE-ROUTE ANY
EXISTING UNDERGROUND SANITARY OR STORM SEWERS ENCOUNTERED,
NOT INDICATED ON THE PLANS WHETHER ACTIVE OR NOT DURING
CONSTRUCTION AS NECESSARY.
6.ALL AFFECTED OFF-SITE AREAS SHALL BE RESTORED WITHIN TWO WEEKS
OF DISTURBANCE.
7.ANY WORK WITHIN THE INDOT ROW SHALL BE CONDUCTED IN
ACCORDANCE WITH INDOT SPECIFICATIONS.
8.ALL GRADES ARE FINISHED GRADE ELEVATIONS.
9.ALL ELEVATIONS ARE TO GUTTER GRADE UNLESS NOTED OTHERWISE.
10.ALL OPEN SPACE AREAS SHALL BE DRESSED WITH TOPSOIL AND SEEDED OR
SODDED PER THE CONTRACT. MAXIMUM SIDE-SLOPE IN OPEN SPACE AREAS
SHALL BE 4H TO 1V.
11.THE CONTRACTOR SHALL MAINTAIN PUBLIC ROADWAYS CLEAN OF ANY
AND ALL MATERIALS AND DEBRIS WHICH RESULT FROM CONSTRUCTION
OPERATIONS.
12.THE CONTRACTOR SHALL REVIEW AND UNDERSTAND THE GEOTECHNICAL
INVESTIGATION REPORT AND ITS EFFECT ON THE CONSTRUCTION OF THE
PROPOSED SITE IMPROVEMENTS. THE CONTRACTOR SHALL ADVISE THE
OWNER AND ENGINEER OF ANY POTENTIAL ADJUSTMENTS TO THE
GRADING PLAN.
13.STRIP 6" MINIMUM TOP SOIL FROM ALL AREAS WHICH WILL BE FILLED OR
CUT INCLUDING WASTE OR BORROW AREAS. ADDITIONAL STRIPPING MAY
BE REQUIRED TO ENSURE REMOVAL OF ORGANICS AND SOFT SEDIMENTS.
14.AREAS WHICH WILL RECEIVE EMBANKMENT SHALL HAVE TOP 12-INCHES
DISKED AND COMPACTED TO 95% STANDARD PROCTOR DENSITY.
15.ALL FILL MUST BE COMPACTED TO A DENSITY NOT LESS THAN 95%
STANDARD PROCTOR DENSITY WITH MOISTURE LIMITS SET FORTH IN SOILS
REPORT.
16.ALL LOCALIZED AREAS WHICH CANNOT BE COMPACTED OR WHICH SHOW
EVIDENCE OF PUMPING ACTION MUST BE UNDERCUT AND RECOMPACTED
WITH ON-SITE FILL MATERIAL.
17.ALL STRUCTURAL FILL AREAS SHALL BE BENCHED.
18.MAINTAIN CUT AND FILL AREAS TO ACCOMMODATE SURFACE DRAINAGE.
19.STOCKPILE TOP SOIL FOR SHOULDERING AND SEED/SOD BEDS.
20.FINISHED GRADE TOLERANCE IS 0.20 FOOT FOR NON-PAVED AREAS.
21.FINISHED GRADE TOLERANCE IS 0.10 FOOT FOR PAVED AREAS.
22.MINIMUM THICKNESS OF TOP SOIL IS 6-INCHES ON ALL LANDSCAPE AREAS.
23.BACKFILL TO TOP OF ALL CURBS.
24.THE PROJECT SITE IS PRIVATE OWNED, THE GRADING SYSTEM IS DESIGNED
FOR PRIVATE USE.
25.NO EARTH DISTURBING ACTIVITY MAY COMMENCE WITHOUT AN
APPROVED STORM WATER MANAGEMENT PERMIT.
26.DAMAGE TO THE EXISITNG RIGHT-OF-WAY SHALL BE RESTORED/REPAIRED
TO THE SATISFACTION OF THE CITY AT THE COMPLETION OF THE PROJECT.
THE CONTRACTOR IS ENCOURAGED TO INSPECT THE RIGHT-OF-WAY WITH
THE CITY PRIOR TO THE START OF CONSTRUCTION TO DOCUMENT THE
EXISTING CONDITION OF THE RIGHT-OF-WAY.
27.ALL PAVING WITHIN THE EXISTING AND PROPOSED CITY RIGHT -OF-WAY
SHALL CONFORM TO THE REQUIREMENTS OF THE DEPARTMENT OF
ENGINEERING. THE CONTRACTOR SHALL CONTACT THE DEPARTMENT OF
ENGINEERING TO SCHEDULE A PRE-CONSTRUCTION MEETING TO REVIEW
THE DEPARTMENT'S CONSTRUCTION REQUIREMENTS, STAFF NOTIFICATION
REQUIREMENTS, REQUIRED INSPECTIONS FOR CERTAIN STAGES OF THE
WORK AND TO REVIEW THE AUTHORITY OF THE DEPARTMENT AS IT
RELATES TO WORK WITHIN THE EXISTING AND PROPOSED RIGHT-OF-WAY.
