HOMEOWNERS REQUEST FOR CHANGE
As required by the Lakes at Hazel Dell Covenant, this form must be printed, filled out
and submitted by any homeowner wishing to make exterior changes to his/her home or
property. These structure/color additions/changes include, but are not limited to: decks,
fences, retaining walls, hot tubs, play structures, basketball goals, mail boxes, patios,
shutters, trim, doors, windows...

***Roof Shingle replacement will only be approved if you are installing charcoal,
brown or dark gray shingles. Black shingles will not be approved.
