the appropriate documents and then complete the Applicant Upload Task This kicks the system over
to me for review. If you click on the Project Tasks button in the op right hand corner of the screen
this will bring up the Applicant Upload Task link. A new window will appear with prompts to
complete the task. Please let me know if you have any questions.

Mr. Poynter,
I hope all is well with you. I drove by the store recently and noticed that the façade was changed
(see attached). An administrative ADLS Amendment is required because of this unapproved change.
The petition will not be seen by any entity, but will instead be only reviewed by myself. There is a
small filling fee of $143.00.
An application can be applied through Cityworks, Carmel’s ePermitting site. Once submitted I will
send an invitation to ProjectDox, Carmel’s eReview site. Please then upload all task to the site and
complete the Applicant Upload Task to move the system over to me for review.
