All proposals MUST include a Surveyor’s/Plot Plan with the area of proposed change clearly marked aswell as photos of the proposed improvement. Proposals will not be considered without all of thisinformation.The homeowner is responsible for obtaining any necessary building permits
You may apply for more than one change per each form (i.e. a fence and a deck)If you are requesting to replace or exchange a “street tree” in the right of way area in front of your home,you may only replace it with the same type of tree. This is due to restrictions set forth by theMunicipalities in your area.
By signing below, I acknowledge and agree to all of the following:
1. I am required to obtain architectural approval before making any improvements or additions to my home.
2. I am obligated to allow sufficient time for the processing of my request.
3. I have read and will fully comply with the Declaration and the Design Guidelines in their entirety. It is my
responsibility to research and adhere to all guidelines specified by the Design Guidelines. ModificationsCommittee shall not bear any responsibility for ensuring the structural integrity or soundness of approvedconstruction or modifications, nor for ensuring compliance with building codes, zoning or PUD ordinances,
and/or other governmental requirements.
4. Prior to excavation, I am responsible for location of all underground utilities by calling 811 or going towww.811now.com. Digging should not commence until underground utility locations are marked.
5. I am responsible for all clean-up of the project. I will make sure that all debris, sod, soil, and so on areremoved and hauled to the proper waste sites.
6. If the project causes injury to or encroachment upon adjacent property or public property, includingdrainage easements I am responsible for any necessary repairs and the removal of any encroachments.
7. I am responsible for damages to residential or common area irrigation systems that may result from the
project.
8. I acknowledge that if any work does not conform to the approval of the Modifications Committee or to theDesign Guidelines, I am liable for all costs necessary to bring the work into compliance, including legal fees if
necessary. This approval action does not relieve me from any Declaration or Design Guidelines requirements.

The proposed structure consists of a deck area (lower and an upper deck), Swim spa, Two patio
areas, a Pergola, Basket ball hoop and trees. The lower deck area will consists of a Fire table. Gaslines will be drawn from the main meter to provide natural gas to the fire pit and a grill in the lowerdeck area. Electrical lines will be drawn to supply power to the Swim spa. The upper deck area willsurround the Swim spa from three sides. Necessary railing will be installed per Carmel city code.Three sets of drawings are attached;1) Lot plan with proposed construction showing distances from the T.O.B.B.S.L (Top of bankbuilding set back line), B.M.P.E (Best Management Practice Easement) and R.D.E. (RegulatedDrainage Easement) lines.2) 2D drawing showing a plan view and a rear view of the construction3) 3D drawing showing a Isometric view of the construction with a Table consisting of dimensions,area, weight and material information of each component in the proposed construction. See Page-2 of this drawing for the table. This drawing also includes the distances from the T.O.B.B.S.L (Topof bank building set back line), B.M.P.E (Best Management Practice Easement) and R.D.E.(Regulated Drainage Easement) lines.
