June 15, 2016, for leased premises at 11939 North Meridian Street, Carmel, Indiana.
Dear Mssrs. Wagner & Reese:
Pursuant to Article 18 of the Lease Agreement entitled, “Exterior Building Signage,” Tenant is hereby notified that
Landlord has approved the attached eye4group specifications (OR-7174 dated 06/11/21) for Wagner Reese’s
lettering to be installed at Tenant’s expense on the existing monument sign at 11939 North Meridian Street, Carmel,
Indiana, 46032, by eye4group as depicted on the specifications. Tenant and its sign vendor shall comply with all laws
including zoning rules, regulations, and any applicable building codes.
Please have eye4group forward its Certificate of Insurance to me at mikem@citimarkinc.com evidencing at least
$1,000,000 in commercial general liability coverage naming 11939 Office, LLC, and Citimark Management Co., Inc.,
as additionally insured before commencing work.
Please notify Citimark at least 48 hours in advance of installation.

2016 Minimum Standard Detail Requirements for ALTA/NSPS Land Title Surveys, jointly established and
adopted by ALTA and NSPS, and includes Items 1, 2, 3, 4, 7(a), 7(b)(1), 7(c), 8, 9, 11, 13, 14, 16, 17, 18, 19
and 20 of Table A thereof. The fieldwork was completed on 12-17-2019.

SURVEYOR'S REPORT
In accordance with 865 IAC 1-12-1 et seq., the following observations and opinions are submitted regarding
the causes and the amounts of uncertainty in the locations of the lines and corners found or established during
this survey as a result of:
A.)availability and condition of reference monuments,
B.)occupation or possession lines,
C.)clarity or ambiguity of the record descriptions for the site and its adjoiners, and
D.)the relative positional accuracy of the measurement.
A.The following monuments were found or re-established marking the corners common to the Northwest
Quarter of Section 35, Township 18 North, Range 3 East. Unless otherwise noted, there is negligible
uncertainty associated with the locations of these corners.
Southeast corner: Harrison Monument found
Northeast corner: Harrison Monument found
Southwest corner: Harrison Monument found
B.Unless otherwise noted, there is no physical evidence of occupation or possession along the boundary
lines of the subject parcel.
493.91' Line: A gas line marker is on line near the east end. A handhole is 1.9 feet south of the line. The
south edge of a planter/landscape area ranges from 1.2 feet south to 0.8 of a foot north of the line.
251.67' Line: A sign is as much as 4.7 feet west of the line.
359.76' Line: Several utilities cross the line near the west end and the middle of the line. A transformer is
5.7 feet south of the line.
327.00' Line: A buried telephone line ranges from 1.3 feet east to 4.4 feet west of the line.
C.The record descriptions for the site and its adjoiners were obtained from the office of the Recorder of
Hamilton County (the latest research date was December 19, 2019). The record descriptions for the
adjoiners were mathematically retraced to disclose any gaps and/or overlaps between the boundary lines
of the site and its adjoiners. The following observations are made as a result of this retracement:
There is a strip of land in Pennsylvania Street that we could not find a right-of-way dedication for. The
strip is between the east deed line and the east line of the City of Carmel parcel. The strip is not included
in the exception to the deed legal description.
A portion of the west side of the subject parcel was sold to the State. This portion is not excluded from
the deed legal description.
The Surveyed Parcel legal description excludes the strip, the City of Carmel parcel and the State of
Indiana parcel.
D.The relative positional accuracy (uncertainty due to random errors in measurement) of any point on the
survey relative to any other point on the survey at the ninety-five percent (95%) confidence level is within
the specifications for an Urban Survey (0.07 feet + 50 ppm) as defined in 865 IAC 1-12-7.
As a result of the above observations, it is my opinion that the uncertainties in the locations of the lines and
corners established during this survey are as follows:
Due to variances in reference monuments: 0.0 feet;
Due to inconsistencies in lines of occupation: see “B” above;
Due to discrepancies in the record descriptions: 0.0 feet; and
Due to random errors in measurement: within the specifications for an Urban Survey (0.07 feet + 50
ppm) as defined in 865 IAC 1-12-7.
SURVEYOR'S NOTES
1.This plat of an ALTA/NSPS LAND TITLE SURVEY is based upon the Near North Title Group
commitment for title insurance having Commitment No. IN1911691 and the Commitment Date of
November 15, 2019.
2.No comment is made regarding the following Exceptions in Schedule B, Part II of the commitment for title
insurance: 1-13, 18 and 19.
3.The following notes are keyed to the Exceptions in Schedule B, Part II of the commitment for title insurance.
Item 14: Sewer Easement per Easement Record 3, Page 926 - The easement falls on the subject parcel along
the north line.
Item 15: Easement for Landscaping and incidental purposes per Instrument Number 2000-09082 - The
easement falls on the subject parcel along the east line.
Item 16: Limitation of Access per Instrument Number 2014006009 - The west lines of the subject parcel are
limited access lines.
Item 17: Easement for electric and/or telecommunication lines and incidental purposes per Instrument
Number 2016059659 - The easement falls on the subject parcel along the west and south lines.
4.The purpose of this survey was to prepare an ALTA/NSPS Land Title Survey for the site.
5.The accuracy of any flood hazard data shown on this survey is subject to map scale uncertainty and to any
other uncertainty in location or elevation on the referenced Flood Insurance Rate Map. Flood Insurance Rate
Map having Community-Panel Number 180081 0208 G and 180081 0209 G (both revised November 19,
2014) of the National Flood Insurance Program indicates this site to be within zone “X” (areas determined to
be outside 500-year floodplain).
6.Basis of Bearing System: Indiana State Plane Coordinate System, East Zone.
7.The locations of any underground utilities shown on this plat are based upon above ground evidence
(including, but not limited to, manholes, inlets, and marks made on the ground by others) and construction
plans for the site. There may be underground utilities for which there is no above ground evidence or for
which the above ground evidence was not observed (i.e. buried or paved over).
8.The dimensions of the buildings are exterior (“footprint”) dimensions and the area of the building is the
“footprint” area.

10.This plat of an ALTA/NSPS LAND TITLE SURVEY represents a survey made under my supervision
and in compliance with “Rule 12” of TITLE 865 of the INDIANA ADMINISTRATIVE CODE
(establishes minimum standards for the competent practice of land surveying) and the MINIMUM
STANDARD DETAIL REQUIREMENTS and CLASSIFICATIONS for ALTA/NSPS LAND TITLE
SURVEYS (a copy of which is attached hereto and is hereby made a part hereof) with corners established as
shown.
