Vine & Branch sign or will this email suffice?
I approve SavATree Inc to re -face the old sign at 4721 (anf 4621 which is their USPS address) E 146th St, Carmel, IN

Here is a link to Cityworks, Carmel's ePermitting site. The Sign Permit Application can be found under Create
Application > Sign Permits > Commercial. Please then complete the application to the best of your ability and hit
submit. Below is a list of documents I will need for the review. Because the sign was simply refaced a fee of $87.00 will
be assessed. If you have any questions please let me know.
• Written property owner approval.
• Written business owner approval.
• Aerial or site plan showing the location of the sign
• Rendering or spec sheet with sign dimensions.
• A landscape plan for the sign.
