A RESOLUTION ESTABLISHING A POILCY CONTINUING
EMPLOYEE BENEFITS FOR HEALTH, ACCIDENTAL DEATH
AND DISFIGUREMENT AND LIFE INSURANCE
DURING LEAVES OF ABSENCE
WHEREAS, the City of Carmel, Indiana provides certain insurance
benefits to its employees; and
WHEREAS, situations arise whereby employees may take a leave of
absence from their employment;
NOW, THEREFORE, BE IT RESOLVED BY THE CARMEL BOARD OF PUBLIC WORKS
AND SAFETY THAT:
Any full-time permanent employee who takes an approved leave of
absence from City employment, and prior to such leave gives written
notice to their department head of their intent to return to their
City employment at the conclusion of the leave, or those employees
who have been determined by a certified pension board to be disabled
and are therefore necessarily on such a leave, may continue coverage
of health, accidental death and disfigurement and life insurance
plans of the City while on leave in accordance with the following
provisions:
The City shall pay seventy-five percent (75%) of the applicable
monthly premium and the employee shall pay the balance of
twenty-five percent (25%) for the initial period of ninety (90)
days. After such initial period continued coverage is at the
option of the employee in accordance with the Public Health
Service Act, at a one hundred percent (100%) cost to the
employee.
In the event the employee fails to return to his/her employment
as previously promised, the City may seek reimbursement from
said employee for the amount of the City's contribution to the
premium during the initial ninety (90) day period.
The employee's portion of the premium shall be paid to the
Clerk-Treasurer's office in a timely manner (payment due 1st of
each month) or the employee may lose the continuing coverage
provided by the City.
This resolution applies to all full-time employees in all
departments (including police and fire personnel) and agencies in
the City of Carmel.
This resolution shall supersede any and all resolutions heretofore
passed by the Board of Public Works governing the City's obligation
for the provision of such health, accidental death and disfigurement
and life insurance coverages and shall constitute the City's policy
from this date forward.
