HomeMy WebLinkAboutBPW-03-04-91-01InsuranceA RESOLUTION ESTABLISHING A POLICY CONTINUING EMPLOYEE BENEFITS FOR HEALTH, ACCIDENTAL DEATH AND DISFIGUREMENT AND LIFE INSURANCE Whereas, the City of Carmel, Indiana provides certain insurance benefits to its employees; and Whereas,situations arise whereby employees may take a temporary leave of absence, intending to return to their city employment at the end of their leave of absence; and Whereas, clarification is required as to whether a continuation of these benefits should take place while the employee is on leave of absence; NOW, THEREFORE, BE IT RESOLVED BY THE CARMEL BOARD OF PUBLIC WORKS AND SAFETY THAT: Any employee who takes an approved leave of absence from his/her city employment and,prior to taking that leave of absence,gives notice to their Department Head of their intent to return to their city employment at the conclusion of the leave of absence, shall be entitled to continued coverage of those benefits provided by the city, in the same percentage of contribution as if the employee had not taken their leav~ of absence. Any employee taking a leave of absence, who fails to pay their proportionate share of any insurance benefit, in a timely manner, may lose the continuing coverage the city provides. In the event any employee fails to return to his/her employment as previously promised, the city may seek reimbursement from the employee for the cost of the benefits provided. In any event,no employee shall receive any benefit payments for a period more than one hundred twenty days (120) from the date the approved leave of absence began, unless otherwise approved by the Board of Public Works.
