Release CITY OF CARMEL, INDIANA BOARD OF PUBLIC WORKS & SAFETY A RESOLUTION ESTABLISHING REOUIREMENTS FOR MEDICAL RELEASE WHEREAS, from time to time, employees of the City of Carmel become ill, are injured on or off the job, or otherwise prohibited from work due to illness or injury; and WHEREAS, it is in the best interests of the employee and the City that a medical release be obtained certifying the employee's return to work; NOW, THEREFORE, BE IT RESOLVED by the Carmel Board of Public Works and Safety that: 1. Any employee of the City of Carmel who is absent from work more than three (3) consecutive days due to illness or injury may not be permitted to return to work until such time as a written release, permission slip, or certification is received by the relevant department head from the employee's attending physician certifying that employee's fitness to return to work. Any employee attempting to return to work without receiving written permission from his or her attending physician may be subject to termination and may not receive compensation for that time worked in violation of this directive. 2.
