Dear Bill,
As we have discussed with Drew Pattyn, our Task Force would like to extend our lease with you
for an additional two years. We have received from Mr. Pattyn, a Fourth Lease Amendment and
agree to the terms set forth within, including the only change being the increase in rein.
Enclosed please find a purchase order for the amount of rent due beginning April 2002 and
cominuing through March 2003. We will continue to pay you on a monthly basis·

Administrative Assistant, a call at 571-2522.

FOURTH LEASE AMENDMENT
THIS FOURTH LEASE AMENDMENT is made and entered into by and between The
Peak Group, Inc., an Indiana Corporation CLessor'') and H.C.D.T.F./City of Carreel ("Lessee").
WITNESSETH:
WHEREAS, Lessor and Lessee entered into a certain Lease Agreement dated January 10,
1996 (the "Lease'), whereby Lessee leases t~rom Lessor certain premises consisting of
approximately 2,550 square feet of space and located at ~_Location unidentified for security
purposes (the "Leased Premises"); and
WHEREAS, Lessee leased an additional 3,400 square feet of space on April 5, 2000
(5,950 total square feet) and
WHEREAS, the Lease, by its terms (and subsequent First, Second and Third
Amendment(s) te~s), expires on January 31, 2002 and
WHEREAS, Lessor and Lessee now desire to amend this Lease to allow Lessee to renew
the existing (5,950 square feet) "Leased Premises" for an additional term of two years, and
NOW, THEREFORE, in consideration of the premises and mutual covenants herein
contained, Lessor and Lessee hereby enter into this Fourth Lease Amendment.
1. Amended as follows:
· Lessee will lease the Lease Premises, on an "AS IS" basis.

2. Effective Date. The effective date for the provisions of this Fourth Lease
Amendment shall be the date which the last party hereto executes same.
3. Definitions. The terms used in this Fourth Lease Amendment shall have
the definitions set forth in the Lease.
4. Incorporation. This Fourth Lease Amendment shall be incorporated into
and made a part of the Lease, and all provisions of the Lease not expressly modified or amended
hereby shall remain in full force and effect.
IN WITNESS WHEREOF, Lessor and Lessee have executed this Fourth Lease Amendment this
