ONE CIVIC SQUARE / COUNCIL CHAMBERS The meeting was called to order by Mayor Ted Johnson. Board member Rick McKinney was in attendance along with Clerk-Treasurer Susan W. Repp. Mayor Johnson moved to approve the minutes APPROVAL OF MINUTES: from the meetings of August 18, 1993, September 22, 1993 and September 30, 1993. The motion was seconded by Rick McKinney. Bids for the traffic signal project at Rangeline Road & 131 BIDS: Street, Main Street & Guilford Road and Rangeline Road & Napanee Drive were opened and read by the Board. James Drew Company $60,470.75 Hoosier Company $54,614.41 Zeal Company $44,230.22 Midwestern $47,319.75 Signal Construction $47,963.50 The bids were taken under advisement by City Engineer, Tom Welch and later return with a recommendation that Zeal be awarded the contract. The Mayor so moved. The motion was seconded by Rick McKinney. All members voted aye. Motion carried. Bids for Chemicals, Salt and a One Ton Dump Truck were opened and read by the board. See attached tabulation sheet for bids. The bids were taken under advisement by Lee Dolen of the Carmel Utility Company. Board members signed the final plats for Bayhill, PLAT SIGNATURES: Section 2, Stonewick, Section 3 and Windpointe, Section 2. REQUEST FOR RECOMMENDATION TO THE CITY COUNCIL OF ANNEXATION OF Jim Nelson, Attorney for the petitioner SHADYBROOK SUBDIVISION: presented the annexation to the Board and asked for a favorable recommendation. Mayor Johnson moved to give favorable recommendation to the Council for annexation of Shadybrook. The motion was seconded by Rick McKinney. All members voted aye. Motion carried. REQUEST FOR PERMISSION TO PLACE PARK BENCHES IN FRONT OF BUSINESSES Mayor Johnson moved to grant permission to IN OLD TOWN CARMEL: place park benches in front of businesses in old town Carmel. The motion was seconded by Rick McKinney. All members voted aye. Motion carried. The petitioner, Sally Van Overmeriren, stated the benches would be the same style. Mayor Johnson moved to approve the PERFORMANCE BOND RELEASES: release of performance bonds as follows: Smokey Hollow Associates, Storm Sewers, $85,700 (Smokey Ridge, Section 5/6)Water Mains, $72,000 San. Sewers, $109,200 Steven H. Henke (Oakbrook) Storm Sewer, $17,200 Waterstone (Bayhill, Section 2) Streets, $71,525 Curbs, $29,100 Water Main, $72,400 San. Sewers, $63,500 Waterstone (Stonewick, Section 3) Streets, $62,350 Curbs, $25,525 Water Main, $77,975 San. Sewers, $74,100 Waterstone (Windpointe, Section 2) Streets, $34,225 Curbs, $14,225 Water Mains, $41,150 San. Sewers, $104,100 Smokey Hollow Assoc. (Smokey Ridge, Section 2/3) Street Top Coat, $30,500 Smokey Ridge, Section 4 Street Top Coat, $8,350 Mayor Johnson moved to grant REQUEST FOR USE OF CITY HALL: permission to use the gazebo for a Halloween Party on October 31, 1993 from 4 to 6:00 P.M. sponsored by the Carmel Clay Parks and Recreation. The motion was seconded by Rick McKinney. All members voted aye. Motion carried. Mayor Johnson moved to allow a flu FLU SHOTS FOR CITY EMPLOYEES: shot program sponsored by St. Vincents for city employees. The motion was seconded by Rick McKinney. Johnson stated the program would be strictly on a volunteer basis and that St. Vincents would charge $50 per hour for a nurse to administer the vaccine. All members voted aye. Motion carried. Mayor Johnson moved to CARMEL POLICE DEPARTMENT/PAY INCREASE: approve a pay increase for Cynthia Knauer's to $882.28, effective October 9, 1993. The motion was seconded by Rick McKinney. All members voted aye. Motion carried. Mayor Johnson made a motion, SEWER AND WATER AVAILABILITY: seconded by Rick McKinney to approve sewer and water availability for the following: Windpointe, Section 3: Sewer - 27 E.D.U.'s not to exceed an average daily flow of 8,100 gallons. Water - 27 E.D.U.'s not to exceed an average daily flow of 8,100 gallons. Bayhill, Section 3A: Sewer - 22 E.D.U.'s not to exceed an average daily flow of 6,600 gallons. Water - 22 E.D.U.'s not to exceed an average daily flow of 6,600 gallons. Bayhill, Section 4: Sewer - 25 E.D.U.'s not to exceed an average daily flow of 7,500 gallons. Water - 25 E.D.U.'s not to exceed an average daily flow of 7,500 gallons. All members voted aye. Motion carried. CANCELLATION OF BID AWARD BY PHYSIO-CONTROL FOR 5-LIFEPAK 300 AUTOMATIC DEFIBRILLATORS IN THE AMOUNT OF $34,651.25/REQUEST FOR Mayor Johnson made a motion to approve the RE-ADVERTISING: cancellation of the bid to Physio-Control, 11811 Willow Road, N.E., Redmond, Washington for 5 defibrillators and to allow the Chief to re-advertise for that same equipment. The motion was seconded by Rick McKinney. All members voted aye. Motion carried. The Mayor explained that the company has failed to deliver the merchandise. Mayor Johnson made a motion to approve the following CONTRACTS: contracts. The motion was seconded by Rick McKinney. All members voted aye. Motion carried. (See attached for copies of the contracts.)
