Dear Mr. Kilmer:
We have reviewed the drawings submitted for this project for the November 16,2005 Technical
Advisory Committee meeting. We offer the following comments:
GENERAL INFORMATION
1. The project site is located within current City of Carmel Corporate Limits. The following
jurisdictions will apply.
2. Jurisdictions:
. Street and Right of Way - City of Carmel
. Water-Indianapolis Water Company Service Area (IWC)
. Sanitary Sewer-Clay Township Regional Waste District (CTRWD)
. Storm Sewer - Hamilton County (primary), City of Carinel (Secondary).
3. Board of Public Works and Safety Approval Requirements:
. Commercial Curb Cut. When appropriate, Board approval for the curb cuts should be
requested. Please provide 81/2 x 11 exhibits of the curb cuts and include all dimensional
data including throat width, overall Width at the street, radii, etc. Show all existing and
opposing curb cuts and the alignment between existing and proposed.
. Consent to Encroach Agreements for any permanent improvement to be installed within
dedicated right of way and/or dedicated easements.
. Dedication of right of way if applicable.
Any submission to the Board requires prior final approval by the Carmel Clay Plan
Commission and/or the Board of Zoning Appeals and completion of review by the
Technical Advisory Committee. All written requests to be place on the Board's agenda
must include the appropriate Docket Numbers and the date (or dates) of approval by the
Plan Commission and/or BZA.
4. Water and Sanitary Sewer Availability (acreage) and Connection Fees
. Current Water and Sanitary Sewer Availability Fees
Not required by the City but may be required by the CTRWD or the IWC.
. Current Water and Sanitary Sewer Connection Fees
Not required by the City but may be required by the CTR WD or the IWC.

We request that all comments and comment letters generated by this office be answered in
writing and be accompanied by a drawing reflecting requested revisions. Final drawings will
not be approved for construction until all issues have been resolved. The design engineer
must certify all drawings submitted for final approval. This office will require a minimum of
three sets of approved drawings after all issues have been resolved and the construction
drawings have been approved. The drawings will be stamped approved and signed by the
City Engineer. The Developer/Owner Will receive one set which is to be maintained on the
construction site at all times. Our Construction Manager will receive one set and one set will
be maintained in the Department of Engineering. If more than the minimum three sets are
requested to be approved, the additional sets (maximum total of two additional) must be
submitted with the three required sets.
6. Please be advised that any installation of signs, walls, irrigation systems, etc. within dedicated
right of way and/or dedicated easements Will require a Consent to Encroach Agreement
between the OwnerlDeveloper and the City of Carmel. This Agreement requires Board of
Public Works and Safety approval.
7. I am including copies of the following with this correspondence:
. Commercial Project Approval Procedures.
. Permit Data, Contacts, etc.
. Commercial Permitting Procedures.
I will send a copy of our Commercial Irrigation System Requirements upon request.
BONDING REQUIREMENTS
8. Individual Performance Guarantees will be required for the following subdivision
improvements:
. Streets (base, binder and surface). You may bond perimeter street improvements
separately.
. Curbs
. Water Mains
. Street Signs
. Sidewalks (builders lot sidewalks and comm~n area right of way sidewalks-asphalt
paths/perimeter street right of way sidewalks~asphalt paths may be bonded
separately).
Note: Storm sewers/Storm Drainage and Monuments & Markers bonding is under the
jurisdiction of the Hamilton County Surveyor's Office. Sanitary sewers are Clay Township
Regional Waste District bondingjurisdiction.
The amount of the Performance Guarantee is based upon a certified Engineer's Estimate
for 100% of the cost of labor and materials to construct the individual improvements, to
be provided by the design engineer. Please provide detailed Eneineers Estimates for
'each improvement includine unit costs. Quantities. materials and types of materials.
etc. Upon completion and release of individual Performance Guarantees, a three-year
Maintenance Guarantee will be required. The Maintenance Guarantee amount is based
upon 15% of the Performance amount for Streets and Curbs and 10% of the Performance
amount for all other improvements. Performance Guarantees may be Performance or
Subdivision Bonds or Irrevocable Letters of Credit. Please reference the enclosures for
more detailed explanation of our procedures.

and drainage calculations to Greg llko of Crossroad Engineers.
These comments represent the Department of Engineering's initial review of the construction
plans for this project. We request that all responses to our comments be provided in writing.
Failure to provide written responses may result in the delay of the review process.
It is critical that our office be made aware of all modifications made on the plans being
resubmitted, particularly if any such changes are considered "new" or fall outside of our previous
reviews. Please provide revised plans including all revisions. Please notify us of any changes
and specifically state any changes, including changes resulting from Plan Commission, BZA or
other committee meetings.
All bonds and performance guarantees mi.1st be posted prior to Engineering Department approval
of construction plans. Also, Board of Public Works and Safety approvals and any other
governing agency approvals (if required) must be obtained prior to Engineering Department
approval.
The Department reserves the right to provide additional comments based on subsequent reviews.

The following comments are regarding this months TAC items. Additional information, such as the District
Application and Specifications, are available at our website wWw.ctrwd.org.
Clarian North Expansion - The existing sanitary sewer main extension must be dedicated to the District before
sanitary sewer permits may be issued in this area. Sanitary sewer construction plans need to be submitted for
review to the District. The District has not received sewer service plans. A District project application needs to be
completed.
Grandin Hall Section 2 - Outside service area.
College Park Baptist Church Parking Lot Expansion - Plans were received and reviewed. No action needed.
Stratford Assisted Living at the Village of West Clay - Plans and District application have been received and
reviewed. Comment letter to follow shortly. An IDEM construction permit for sanitary sewer will be needed. All
IDEM submittal information should be provided following the District's review and approval of final plans.
Kendal Wood - The plans and application have been received last week. An IDEM construction permit for
sanitary sewer will be needed. All IDEM submittal information should be provided following the District's review
and approval of final plans.
Sanders Plat -- Not received by District. Please submit for review.
Mayflower Storage Condos - Plans have been received and reviewed. Additional information will be requested.
A CTRWD Application is needed.

Dear Mr. Scimia:
This letter is in response to your Development PlanlADLS and Secondary Plat applications for Clarian North
Hospital-Medical Office Building II. The following issues must be resolved:
Comments and concerns in regard to the Development PlanlADLS are as follow:
1. Please submit an application for Development Plan (DP) Review. The DP application will be
processed with this docket item (05100038 DP/ADLS). The completed application and fee
($772.50 + $5968.85) must be submitted to the Department of Community Services.
2. Please provide pedestrian connections from the proposed parking lot to the trail to the west,
northeast, and southeast.
3. As per the Alternative Transportation Plan, a ten-foot pedestrian path is required along Illinois
Street. Please provide.
4. A sidewalk is indicated around the parking lot addition, however, the DP filed with 29-03 Z
indicates a multiuse trail. Please indicate the sidewalk as a multiuse trail.
5. Identify and label all required rights-of-way (Illinois Street is 100 feet, and 1 16th Street is 140
feet).
6. A copy of the landscape plan approvalletter/e-mail from the Urban Forester is required prior to
approval.
7. The east fayade of the building will be highly visible from US 31. Staff recommends the use of
bays, similar to the existing structure. In addition, buildings within the US 31 Overlay District are
required to have 8 corners. Please revise the east elevation to address this concern.
Please wait to submit revised materials until after the Technical Advisory Committee (TAC) has met on these
applications. The resubmitted materials should include edits addressing the above connnents along with any
requirements that arise from the TAC meeting. Additional comments may be mad e requested revisions
have been submitted.
