This evaluation is intended to assess the level of compliance with 327 lAC 15-5 (Rule 5). It is also intended to identify areas
where additional measures may be required to control erosion and sedimentation. All practices recommended in this report
should be evaluated as to their feasibility by a qualified individual with structural practices designed by a qualified engineer.
All erosion and sediment control measures shall meet the design criteria, standards, and specifications outlined in the "Indiana
Handbook for Erosion Control in Developing Areas" or similar guidance documents.
Current Site Information:
Site development is completed. All disturbed areas are seeded except for 30' behind the curbs. Home construction has
not started in this section.
The Following Items Have Been Evaluated and Assigned a Designation of:

Construction Site Management for Erosion and Sediment Control
S (1) Disturbed areas have been adequately protected through seeding or other appropriate erosion and

In response to your comment letter dated May 1, 2002 concerning the above referenced
project, the following is offered to your comments:
"1. Change "Final Plat" to "Secondary Plat.""
The change has been made.
"2. Change note of "Prepared by" to "Surveyor.""
The change has been made.
"3. Change note of "Developed by" to "Subdivider"."

"4. Change note of "owned by" to "Owner"."
Complete.
"5. Do pedestrian paths run through Blocks A & D? If so, please add pedestrian
access easements."
A 10 foot pedestrian access easement has been added.
6. Remove "When contracted by developer" from the Monumentation Legend.
Has been removed from the Monumentation Legend.

Continuation labels have been corrected on all sheets.
10. Note: Addresses are not required on a secondary plat. If ever a change were
necessary it would require a secondary plat amendment, which is both costly and
time consuming. As a matter of policy our Department recommends that
addresses not be placed on the secondary plat (sheet 5 only).
The addresses are not included on the Secondary Plat.
11. Label Block "A", sheet seven, what is designation of small block on sheet seven,
southeast comer? Label this block separately.
The southeast corner on sheet seven is included in Block "A" and has been
labeled as such.
12. Add a marker between lots 80 & 81 on Sheet 7.

Dear Mr. Corey:
This letter serves to acknowledge receipt of a transmittal received 5/24/02 containing the
plans for the above-mentioned project. After reviewing the plans the Highway
Department has the following comments:
1. Please remove the road and road dimensions, swale locations and asphalt path from
the right of way on the plat. Only the dimensions and labels for the right of way need
to be included.
2. The testing and inspection will need to be executed for Section 2 before any
construction can begin.
3. Bonds/Letters of Credit will need to be submitted at the time of final plat approval.
The bondslletters of credit should match the provided estimate for the work.
4. It appears that all comments have been satisfactorily addressed for the construction
plans and secondary plat. Therefore, the Highway Department request seven sets of
plans for final approval. The plans should not be submitted until all comments from
the Hamilton County Surveyor's office have also been addressed.

I have reviewed the plans for this project and have the following comments.
Procedural Items:
1. Please provide the signed form to make the stonn sewers regulated.
2. Provide the signed Non-enforcement Permit form. with the appropriate check,
3. Please forward an engineers estimate or oOPY of the contract for 100% of the
constmction costs for storm sewers (including SSD), erosion control, and monuments
and markers,
4. Submit original letters of credit or bonds for the above amounts.

S. A 6" field tile has been located south of the intersection of Towne Road and 146111.
This field tile crosses Town Road and comes onto this site 3 t north of the existing
culvert There may be further discussions regarding this tile,

Please submit a revised Secondary Plat with the above-mentioned changes. Once these changes
have been made and comments addressed, please produce a mylar copy for signature by this
Department. Two paper copies of the plat must be filed with the mylar prior to recording. I will
return the signed mylar to you for signature by the County Commissioners. Once recorded, our
Department needs three paper copies and a mylar of the recorded plat. Our approval of the
secondary plat ( signature) will be forthcoming upon revision of the plans according to the items
listed above.

Dear Mr. Moen:
This letter serves to acknowledge receipt of a transmittal received 4/25/02 containing the
plans for the above-mentioned project. After reviewing the plans the Highway
Department has the following comments:
1. Please note that decorative signs, sprinkler systems, trees, landscaping mounds,
light poles or other such amenities are not permitted in the right of way.
2. the secondary plat for Section 2 will need to be submitted for review and approval.
3. a testing and Inspection will need to be executed for Section 2. the necessary
documents will be forwarded to SCM Development for execution.
4. Before any approvals are granted for the Secondary Plat, an Engineers Estimate
broken into the following categories will need to be submitted, along with a
Performance Bond or Letter of Credit in the amount of 100% of the estimate:
* Stone Base
* HAC Binder
* HAC Surface
* Concrete Curbs
* Street Name and Regulatory Signs

I have reviewed the plans for this project and have the following comments.
Procedunlltems:
1. Please provide the signed fonn to make the stonn sewers regulated.
2. Provide the signed Non-enforcement Permit fonn with the appropriate check.
3. Please forward an engineers estimate or copy of the contract for 100% of the
construction COsts for storm sewers (including SSD), erosion control, and monuments
and markers.
4. Submit original letters of credit or bonds for the above amounts.
ConstruettoD Plus:
S. A 6" field tile has been located south of the intersection of Towne Road and 146\11.
This field tile crosses Town Road and comes onto this site 3' north of the existing
culvert. There may be further discussions regarding this tile.
6. The other construction plan items previously discussed have been satisfactorily
addressed.

Following are CTRWD comments on the agenda items in our service area for tomorrow's meeting. Please feel free to copy
and distribute this email and attach it to your meeting minutes.
Parkwood West ADLS
We have no comments relative to ADLS approval but there remain a number of issues to work out with the developer
regarding sanitary sewer service. A lift station is required to service this property; the force main must go under 1465 to tie
into gravity sewers to the north so a lengthy State approval process can be anticipated. Building permits should NOT be
issued without the District's connection permit.
Brookstone Park of Carmel, Section 2
The plans are being reviewed by the District's consulting engineers, Samuel L. Moore & Associates, and will be submitted
to IDEM for State permit approval when that review is completed. Building permits should NOT be issued without the
District's connection permit.
Pahud Residential Subdivision
We have no comments about the Primary Plat but the developer needs to contact the District to discuss how this parcel
will receive sanitary sewer service since no construction drawings have been provided for review by the District. Building
permits should NOT be issued without the District's connection permit.
East 96th Street Office Campus
The plans have been reviewed by the District's consulting engineers, Samuel L. Moore & Associates, and they have
requested additional revisions before the District can submit them to IDEM for State permit approval. The developer's
engineer has been advised of the plan deficiencies. Building permits should NOT be issued without the District's
connection permit.
Woods of Williams Creek subdivision (Primary Plat)
We have no comments about the Primary Plat but the developer needs to contact the District to discuss how this parcel
will receive sanitary sewer service since construction drawings have not been provided for review by the District. Building
permits should NOT be issued without the District's connection permit.
