the nature of construction we cordially request the following inspections to be

Per the conditional permit release requirements I have attached a copy of the architects
signed Certificate of Substantial Completion.
As the access to all areas of construction is under lock and key, please coordinate the

The Work performed under this Contract has been reviewed and found, to the Architect's best
knowledge, information and belief, to be substantially complete. Substantial Completion is the stage
in the progress of the Work when the Work or designated portion is sufficiently complete in
accordance with the Contract Documents so that the Owner can occupy or utilize the Work for its
inlended use. The dale nf Substantial Completion of the Project or portion designated above is the
date of issuance established by this Certificate, which is also the date of commencement of
applicable warranties required by the Contract Documents, except as stated below:

A list of items to be completed or corrected is attached hereto. The failure to include any items on
such list does not alter the responsibility of the Conlractor to complete all Work in accordance with
the Contract Documents. Unless otherwise agreed to in writing, the date of commencement of
warranties for items on the attached list will be the date of issuance of the final Certificate of
Payment or lhe date of final payment.
Cost estimate of Work that is incomplete or defective:

The Carmel/Clay Zoning Ordinance provides for a
penalty if any construction is continued without
having anyone of the following inspections
scheduled, performed, and approved. This penalty
will be based on the fee structure in Ordinance Z-
289, as amended. This penalty also applies to any
construction that is found to be OCCUPIED before a
Certificate of Occupancy is issued.
This is to certify that a building permit has been issued on this new construction in
accordance with the Ordinances of the City of Carmel! Clay Township.
