ADDITIONAL SERVICES AMENDMENT TO
AGREEMENT FOR PROFESSIONAL SERVICES
THIS AMENDMENT TO THE AGREEMENT FOR PROFESSIONAL SERVICES ("Agreement") entered into
by and between the City of Carmel and COMPANY NAME (the "Professional"), as City Contract dated June 2, 2021
shall amend the terms of the Agreement by adding the additional services to be provided by Professional consistent
with the Scope of Work and Fee Summary attached hereto and incorporated herein as Exhibit "A". The terms and
conditions of the Agreement shall not otherwise be affected by this Additional Services Amendment and shall remain
in full force and effect.

Smoky Row Road Reconstruction
Old Meridian Road to Rangeline Road
ROAD DESIGN
Roadway Plan Development: The project includes the widening and reconstruction of Smoky
Row Road from just north of Old Meridian Road to just west of Rangeline Road.
The following design items are not included in the original design scope and are being
requested as part of this supplement:
1. After completion of preliminary geometric layout with the proposed roadway and the
North End Development, geometric revisions to the layout were requested to add
parallel parking stalls, revise curb cut locations and reconfigure the location of the
proposed path based on the inclusion of parking stalls. These parking stalls were
circulated for review and revised a second time based on comments from the Developer.
Items impacted by this revision include:
a. Updates to the base design files
b. Creation and updates to exhibits to be utilized for client and Developer review
c. Updates to Autoturn templates for turning movements at roundabout, Kensington
Place and Woodland Terrace.
2. After completion of the geometric review submittal, a new roundabout configuration was
provided by the City for review. BLN reviewed this configuration, as well as, the
possibility of lowering the inscribed circle diameter from the proposed in the geometric
review submittal. It was determined that the originally proposed configuration would be
kept, with minor changes implemented. Minor revisions including the removal of truck
aprons are not included in this request, as they are included in the original fee proposal.
The following items are scoping items which were outside the scope of the original
proposal:
a. Investigation into impacts to public safety, specifically the design of the splitter
island on the east leg and the width of the pedestrian refuge provided by the
proposed alternatives.
b. Investigation and simulation of truck turning movements for WB-40, WB-50 and
WB-65 for the alternative configurations.
c. Investigation and computations of fastest path speeds for alternative
configurations.
d. Investigation into revisions to the construction footprint including right-of-way
impacts and utility impacts.

3. It was requested that the roadway alignment be revised to introduce a horizontal curve
to accommodate a skew for the bridge over Cool Creek to avoid an existing transmission
pole. Subsequently, the bridge was then narrowed further so no fill would be placed
around the base of the existing transmission pole to avoid relocation of the existing pole.
This change impacted the following design and/or plan elements:
a. Roadway alignment revisions
b. Roadway profile grade revisions
c. Side ditch profile revisions
d. Typical section revisions
e. Roadway plan sheet revisions
f. Proposed cross section revisions
g. Drainage area computation revisions
h. Storm sewer configuration revisions
i. Storm sewer computation revisions
j. Roadside safety computation revisions
k. Stopping sight distance computation revisions
I. Quantity computations revisions
4. Revisions to roadway geometry at the northwest quadrant of the Cool Creek Bridge were
requested to accommodate plans for development and lessen the amount of proposed
right of way needed. This change impacted the following design and/or plan elements:
a. Side ditch profile revisions
b. Proposed cross section revisions
c. Storm sewer configuration revisions
d. Storm sewer computation revisions
e. Quantity computations revisions
5. Due to the addition of the parallel parking stalls, revisions to the profile grade and the
introduction of a superelevated (remove adverse crown) segment (— Sta. 103+00 to
108+00) was needed to minimize the grade differential between the path and the
proposed stairs leading to the future North End Development while minimizing impact
to the south side of the roadway. Additional cross sections were modeled at each Lot in
the proposed development for coordination with the developer's Engineer of Record.
This change impacted the following design and/or plan elements:
a. Roadway profile grade revisions
b. Typical section revisions
c. Introduced additional superelevation computations
d. Introduced additional superelevation diagram
e. Proposed cross section revisions
f. Drainage area computation revisions
g. Storm sewer configuration revisions
h. Storm sewer computation revisions
i. Quantity computation revisions

6. The proposed rail transition at the Cool Creek Bridge utilized timber guardrail for the
transition instead of a standard transition. A detail was completed for this transition for
inclusion into the road plans. This change impacted the following design and/or plan
elements:
a. Additional detail generated for timber rail transition
b. Submittal for review to Bridge Engineer
It was assumed that this work would be completed by the Bridge Engineer when the
original fee was developed.
7. Maintenance of traffic was originally planned to utilize two main phases of construction
with a detour in place throughout the duration of each phase. The two phases were to
accommodate the Cool Creek Bridge construction and the roundabout and road
reconstruction. Upon discussion with the City, it was determined that full closure of the
roadway could only be implemented outside of the Carmel Clay School year and that
during the school year, one eastbound lane shall remain open at all times. This led to a
revision to the maintenance of traffic to be a combination of phased construction for the
road and roundabout along with a full closure with detour for the construction of the Cool
Creek bridge. This configuration now consists of six phases of construction. This change
impacted the following design and/or plan elements:
a. MOT Typical section revisions / additions
b. MOT Design file revisions / additions
c. MOT Plan sheet revisions / additions
d. Quantity computation revisions
These changes required the development of 26 additional plan sheets to accommodate
the additional MOT phases.
8. The proposed crossing of the Monon Trail was included in the scope as a tabletop
crossing, with little to no impact to the adjacent roadway. Upon further detailed analysis
of the existing profile grade along Smoky Row Road, it was determined that the sight
distance from the adjacent roadway, Greenford Trail, would be limited by raising the
profile using a table top crossing. Therefore, the profile grade of Smoky Row Road will
be adjusted to provide adequate sight distance. This change is anticipated to impact the
following design and/or plan elements:
a. Roadway profile grade
b. Roadway plan sheets
c. Roadway profile sheets
d. Proposed cross sections
e. Drainage area / proposed storm sewer computations
f. Quantity computations
g. Construction cost estimate
